优降糖对心肺转流下冠状动脉搭桥术中尼可地尔的血液动力学作用的影响

摘要 目的观察术前服用优降糖对冠状动脉搭桥术(CABG)中尼可地尔血液动力学作用的影响.方法选择拟行CABG的病人22例,NYHAⅢ~Ⅳ级,术前服用优降糖者为观察组,共10例;未服用者为对照组,共12例.两组术前用药、麻醉方法及术中管理与处治基本相同.术中连续监测HR、MAP、心脏指数(CI)、肺动脉嵌入压(PCWP).数据记录时点为:麻醉及建立监测后即刻(T1)、心肺转流(CPB)前(T2)、主动脉开放后60min(T3)、120min(T4)、180min(T5).结果两组病人的年龄、体重、术前射血分数(EF)、CPB及主动脉阻断时间均无显著性差异.两组T3、T4、T5时的HR均显著增快(P<0.01).对照组T3、T4时的MAP显著降低(P<0.01,P<0.05),且在T3时显著低于观察组(P<0.05);观察组各时点间无显著性差异.对照组T3、T4、T5时的CI显著增高(P<0.01),观察组各时点间无显著性差异.两组各时点的PCWP均无显著性差异;但在T3时对照组显著低于观察组(P<0.05).结论临床用量的优降糖可能削弱尼可地尔的扩血管效应及改善心肌功能的作用.
