摘要
目的研究弹簧圈栓塞颅内动脉瘤术中、术后可能发生的脑梗死并发症,提出防治措施。方法回顾性分析193例经弹簧圈栓塞的动脉瘤所致蛛网膜下腔出血患者,发现并发血栓形成3例,分析它们的临床特点及处理。结果术中动脉血栓形成2例(其中1例合并术中颈动脉海绵窦瘘),术后脑梗塞1例。治疗后痊愈1例,自动出院1例,无变化1例。结论弹簧圈栓塞颅内动脉瘤时可发生脑血栓形成并发症,因此要严密观察患者的变化和影像改变,及时发现并发症,并正确处理。
Objective To study the possible causes of cerebral infarction complication during or after intracranial ancurysm cmbolization with controllable coils and point out related preventive and therapeutic measures. Methods 193 subarachnoid hemorrhage patients with intracranial ancurysms cmbolizcd with controllable coils from June 2001 to March 2004 was analyzed retrospectively in regard of their clinical features and management. The thrombosis of the parent artery occurred in 3 cases. Results Among the 3 accidents, there were 2 cases with thrombosis （1 complicated concurrently with carotid-cavernous fistula） during the procedure, and 1 case after intracranial aneurysm cmbolization. Antispastic and other supporting measures were taken in all cases after the procedure. All 3 cases included 1 recovering uneventfully, 1 discharging automatically. Another 1 case got no change. Conclusion The complication of thrombosis may occur during or after the procedure of intracranial aneurysm cmbolization. Patients and imaging changes must be carefully observed in time. Try to find out complication timely and correctly judge and manage during and after embolization.
