摘要
本试验采用有创性血流动力学测定方法评价静脉注射米力农对充血性心力衰竭患者的血流动力学的影响。8例慢性充血性心力衰竭患者,在注入米力农负荷量50μg/kg后,心输出量和心脏指数分别较前升高了46.3%和48.5%,肺毛细血管嵌压较前下降了43.8%,体循环阻力和肺循环阻力分别较前下降了39.4%和48.0%;在4h的维持用药期间,血流动力学的改善持续存在。未见明显不良反应。结果表明短期静脉注射米力农能迅速、有效、安全地改善充血性心力衰竭患者的血流动力学。
To evaluate the hemodynamic effects of intravenous milrinone. 8 patients with chronic heart failure were studied in this test.Milrinone was given 50 μg/kg as a loading injection followed by an infusion with 0.5 μg· kg ̄(-1)·min ̄(-1) for 4 hours.It was found that after infusion of the loading dose, cardiac index increased from 2.27 to 3.371/m2/min(P<0.01), pulmonary capillary wedge pressure decreased from kPa 2.28 to 1.28(17.13 to 9.63 mmHg)(P<0.01),systemic vascular resistance decreased from 1959.04 to 1187.87 dynes,sec/cm ̄5(P<0.01), and pulmonary vascular resistance decreased from 206.39 to 107.42 dynes.sec/cm ̄5(P<0,01). The early improvement in hemodynamics was sustained throughout the 4-hour infusion period. Development of tolerance was not observed, nor were any clinical symptomatic side effects. The results suggest that short-term infusion of milrinone is an effective and safe medication to bring rapid and prolonged improvement in hemodynamics in patients with chronic heart failure.
