摘要
本文报道第二次心脏瓣膜替换手术43例,占同期1100例次手术中的3.9%。文中对再次换瓣的手术适应症、主要危险因素、手术时机、手术技术等方面进行了探讨。
Between May 1976 and May 1988 total of 1100 valve replacements were performed at Fu Wai Hospital, out of these operations 43 (3.9%) were re-do cases including 33 mitral, 6 aortic and 4 mitral with aortic revalvereplacements.In this series 47 valve prosthesis iucluding 2 mechanical,36 bovine pericardial and 9 porcine aortic valves were replaced at the second operation. The new prosthesis used for the second replacement mainly were mechanical valves(43 mechanical and 4 pericardia valves). The average duration between the first and second operation was 69. 3 (5～120) months.The hospital mortlity was 25.6%. The main risk factors were (1) pre-operative heart function. Our patients always came too late after the valve prosthesis went wrong. (2) bleeding and adhesion. (3) surgical technics and longer perfusion time. The indications and surgical technics were discussed. Since 1986, only one death occured in the last 16 operations with a mortality of 6.25%.
