摘要
采用硅橡胶及针织涤纶制成的哑铃型人工食管,将20只犬的中下段食管置换。对其通过功能及病理修复过程进行观察。由于哑铃状的特殊造型,能够实现较为长期固定,取得了全组动物平均存活509天,最长存活达7年零7个月。
ABSTRACT
The collar-button esophageal prosthesis is made of silicons rubber on a dacron framework and divided into two types: type A ( 140 mm long,
end diameter 24 mm, midsection diameter 14 mm)and type B (180 mm long, end diameter 28mm, midsection diameter 16 mm).
In 20 dogs, the prosthesis was inserted by intraluminal telescoping of its ends after resection of middle ( lower ) segmjnt of esophagus, the distances between the cut esophageal ends on completion of anastomos's being 80-120 mm.
The survival time was 2-2788(509) days. Death outcome was due to anastomotic leakage, obstruction or pneumonitis. All animals were subjected to postmortem examination.
The prosthesis acted as a stent for the fibrous wall formed on the 5th post-operative day and afforded reliable protection for two weeks. The prosthesis was securely fixed because of the collar-button enlargements. In 11 of 14 dogs, the prosthesis was kept in its original posit'on 53-2349 days later. In the 3 other dogs, the prosthesis was dislocated. In one 3 months later with stenosis of fibrous wall and in two ( 2-6 yrs ) without any stenosis.
It is concluded that the collar-button esophageal prosthesis has the merit of long term fixation preventing stenosis and disadvantage of increased resistance to food passage.
