摘要
通过前瞻性的随机单盲对照观察,验证冬虫夏草对流行性出血热急性肾损害的疗效。将23例重型、22例中型和13例轻型流行性出血热患者各随机分成两组,一组给予冬虫夏草治疗,一组给予安慰剂作为对照,结果发现:①冬虫夏草治疗可以缩短重型患者的少尿期,降低少尿期死亡率;②接受冬虫夏草治疗者,透析治疗的比例和透析次数均比对照组少;③冬虫夏草治疗可以加快重型和中型患者的肾功能恢复。从而证实了冬虫夏草对急性肾功能衰竭有良好疗效。
ur previous studies have indicatedthat Cordyceps Sinensis (CS) attenuates
theseverity and accelerates recovery in experi-mental models and human cases of nephrotoxic
acute renal injury- Based on the en-couraging results and the unique combination of
pharmacologic properties in this chi-nese traditional medicine, a protocol wasdesigned to test
the therapeutic efficacy ofCS in the management of citinical acute renalinjury with epidemic
hemorrhagic fever(EHF) . 58 EHF patients were entered in thestudy. The patients were firstly
divided intosevere, moderate and mild three subgroupsaccording to the severity of renal injury
,andthen the patients in each subgroup wererandomly assigned to CS or placebo treatment.
Entry serum creatinine levels weresimilar between two treatment groups. Incomparing the
period of oliguria in severecases between the two treatment groups,there was an average of 3.
8~ 3. 9 days inCS treatment group ,but a 6. 3- 6. 4 days inplacebo treartment group. There was
atrend toward shortening the period of olig-uria in CS treatment group(P<0. 1). Dialy-sis was
required in 20% of CS treatmentgroup and in 46. 15% of placebo group.Mortality rates of 100%
for CS group and23. 08% for placebo group were not signifi-cantly different. However, the
causes ofdeath were not similarly distributed be-tween the two groups. All 3 patients died
inoliguric stage in placebo group, while theonly death in CS group was caused by infec-tion
during polyuric stage. There were sig-nificant declines in serum creatinine over the15 days
period after CS treatment in eithersever or moderate cases. At time before,during or after the
treatment period werethere differences in serum creatinine inserve cases receiving placebo. It
is concluded that CS treatment shortens the period ofoliguria and reduces the need for
dialysisand accelerates recovery of renal function inpatients with EHF renal daniage.
