腹主动脉末端闭塞症术后血浆ET-1和NO水平的观察 被引量:1

摘要 目的:观察腹主动脉末端闭塞症术后血浆ET-1和NO的水平,探讨围手术期血浆ET-1和NO测定的临床意义。方法:选择30例腹主动脉末端闭塞症病人,分别于手术前和手术后10d抽取空腹静脉血5ml,利用放射免疫分析和Griss法测定血浆ET-1和NO水平,同时检测30例健康同龄对照组血浆ET-1和NO水平。结果:30例病人血浆ET-1水平均高于正常对照组,而NO水平则低于正常水平。人工血管移植术后10d,26例血运得到明显改善者血浆ET-1水平明显降低,而NO水平则明显升高,恢复正常或接近正常水平。4例术后血运改善不理想者其相应血浆ET-1和NO水平则无明显变化。结论:腹主动脉末端闭塞症患者存在高ET-1血症和低NO血症,人工血管转流术后得到有效治疗者,高ET-1血症和低NO血症可得到有效纠正,而治疗效果不明显者手术前后血浆ET-1和NO水平无明显变化,手术前后观察ET-1和NO水平具有较大的临床意义。 Objective To investigate the clinical significance of determination of changes of plasma levels of ET - l and NO during peri - operative period in patients after bypass operation for atherosclerotic occlusion of distal abdominal aorta. Methods Plasma levels of ET- 1 (with RIA) and NO (with Griss method) were determined in 30 patients with atherosclerotic occlusion of distal abdominal aorta both before operation and on the 10^th day post -operatively as well as in 30 controls. Results Before operation, the plasma contents of ET - 1 were significantly higher and contents of NO were significantly lower in the patients than those in controls (P 〈0.01 ). Ten days after operation, in the 26 patients with successful operation the plasma ET- 1 contents dropped and NO contents increased to approaching normal levels ( vs before operation, P 〈 0.01 ) while plasma contents in the 4 operative failures changed little. Conclusion The higher plasma ET - 1 and lower NO levels in patients with atherosclerotic occlusion of distal abdominal aorta can be corrected after successful bypass operation. Those with unchanged plasma levels are operative failures.
