体外循环期间血浆神经肽Y含量变化及抑肽酶对其影响作用观察

摘要 目的观察体外循环期间血浆神经肽Y(NPY)含量的变化,并探讨抑肽酶对其影响。方法30例体外循环心内直视病人随机分为2组,对照组10例和小剂量抑肽酶组20例。分别于术前空腹,体外循环(CPB)开始后15min、30min、术后3h及术后1d取静脉血,检测血浆NPY值和血小板计数(PC)。结果两组病人NPY值于CPB开始后升高,PC于CPB开始后降低,NPY、PC于术后开始逐步恢复但未至正常基础值。对照组CPB期间血浆NPY释放增加与血小板计数下降呈显著负相关(γ=-0.648,P<0.05),实验组NPY在体外循环期间升高幅度低于对照组(P<0.05),PC的降低幅度低于对照组(P<0.05)。结论体外循环期间使用抑肽酶能有效地保护血小板,减少血小板数量的下降,抑制血浆NPY的释放,从而有利于循环功能的稳定,减少术后出血。 Objective To investigate changes of the plasma neumpeptide Y(NPY) and the effects of aprotinin on NPY during eardiopulmonary bypass (CPB). Methods In a randomized prospective study, 30 patients with open heart surgery were divided into two groups:eontrol group (n=10)and apmtinin group (n=20).Blood samples were eolleeted during the period CPB and after .The plasma NPY and platelets eount (PC) were deteeted. Results In two groups ,NPY were signifieantly inereased during CPB ,while PC were signifieantly deereased during CPB.NPY and PC were began to returned but not to normal after operation. The inereased eoneentrations of NPY were eorrelated negatively with the decreased levels of PC during CPB in the control group (r=-0.648,P〈0.05).In eomparison with eontrol group,the inereasing degree of NPY were significantly lower and the deereasing degree of PC were siginifieantly lower (P〈O.05).Conclusion Apmtinin ean effectively inhibit release of NPY and prevent the deerease of PC. So apmfinin ean protect eardiovaseular funetion, improve perfusion of peripheral tissues and reduee bleeding in patients after operations.
