主动脉压力时间常数测定在冠心病诊断中的应用

TIME CONSTANT OF AORTIC ARTERIAL PRESSURE IN THE DIAGNOSIS OF CORONARY ARTERY DISEASE

摘要 本文采用改良的Simon氏法,测得14例冠心病病人的主动脉压力时间常数为1.54±0.32秒,时间常数与心动周期的比值(T)为1.91±0.28.发现时间常数和T值与核素心功能的左室射血分数(LVEF)、射血时间(ET)、充盈率(FR)呈负相关,其中与LVEF相关性最好(r=-0.831和r=-0.852),同时与左室收缩压峰值,主动脉压亦呈正相关.单纯时间常数的数值小多提示严重心肌缺血,而T值增大则提示合并有小动脉硬化所致的高血压,因此时间常数可以作为衡量左室后负荷,辅助诊断冠心病或合并高血压病的有用指标. The time constants of aortic pressure were measured in 14 patients with coronary heart disease with an average value of 1.54±0.32s, the T value, i. e. time constant/cardiac cycle being 1.91±0.28. Both were found to be correlated negatively with LVEF, ET and FR, positively with the peak value of systolic pressure of LV, and best correlated with LVEF(r=-0.813, r=-0.852). The low value of time constant in patients with simple coronary heart disease usually suggests serious myocardia ischemia while the high one usually indicates accompanied hypertension caused by arteriole sclerosis. Therefore, time constant can be used as a useful index both in evaluating the afterload of the left ventricle and in diagnosing coronary heart disease or hypertension.
