摘要 目的评价EH复合型颈椎螺纹融合器(EHTFC)的有效性和安全性。方法颈前椎间盘摘除后,在恢复颈椎间隙高度和颈椎生理弧度的基础上,用环锯进一步去除骨赘,植入EHTFC。30例中,21例接受1个节段的固定,9例接受2个节段的固定。结果30例无术中并发症,后无感染。随访6~30个月,EHTFC无塌陷、变形或移位,周围无透亮带。术前JOA评分为5~1分;术后评分为12~17分,术后改善率为83%。结论EHTFC能提供即刻的生物力学稳定性,恢复维持椎间隙高度及颈椎生理弧度,EHTFC能完全有效地替代自体髂骨植骨,避免取骨部位的发症。 Objective To evaluate the effectiveness and safety of the cervical threaded fusion cages of EH composite type (EHTFC). Methods After a standard anterior cervical discectomy and on the base of recovery of the cervical interbody height and normal cervical lordosis, the further decompression was performed with trepanation, and then EHTFC was inserted. Results Of thirty patients, 19 were male and 11 female, the median age was 49 years. Twenty-one patients underwent fixation on one segment,and 9 patients on two segments. All patients were free of intraoperative complications and postoperative infection. They were followed up for 6 to 30 months, and graft recipient site-related complications, including graft collapse, graft extrusion or impaction on the spinal canal,or related foramens, did not occur. There was no radiolucent band around EHTFC. The preoperative scores of JOA were 5 to 13 points and 12 to 17 points postoperatively with a mean recovery rate of 83%. Conclusions EHTFC not only provides immediate biomechanical stability, bus also restores and sustains cervical interbody height and lordosis. We substitute EHTFC for autogenous iliac graft absolutely and effectively and avoid the complications of donor site.
