摘要
目的:研究手电筒侧照法在原发性闭角型青光眼筛检中的作用。方法:我们于1995年在广东斗门县四个镇随机抽取50岁以上人群200人,应用手电筒侧照法检查,并根据标准照片进行分级,测量眼压,检查前房角,进行原发性闭角型青光眼的筛检,全部资料作统计学处理。结果:研究显示50岁以上人群手电筒侧照法≤2级者占25.2%,有随年龄增加而增加的趋势(x^2=8.597,p<0.05),女性手电筒侧照法≤2级率高于男性(x^2=14.89,p<O.01)。以颞侧2级或以下房角(Shaffer's分级)为窄角标准,手电筒侧照法≤2级为筛检的临界点时,其敏感度为91.7%、特异度为91.5%。手电筒侧照分级≤2级眼发生原发性闭角型青光眼率为8.16%。没有发现手电筒侧照法3级或以上的眼发生闭角型青光眼。结论:本研究证实手电筒侧照法分级符合筛检试验的要求,对临床和人群的原发性闭角型青光眼筛检有一定作用,我们建议手电筒侧照法≤2级作为筛检窄房角的临界值。眼科学报 1995;11:177—179。
Purpose: To study the effect of oblique flashlight test in screening for primary angle closure glaucoma.
Method: Two hundred adults over 50 years old were sampled randomly at Doumen county of Guangdong province in 1995. 390 eyes were measured by oblique flashlight test according to grading standard photos of oblique flashlight test. Other ocular examinations consisted of intraocular pressure measurement with Schiotz tonometer, gonioscopy, and optic disc appearance. Results: The result showed 25. 2% of the examined eyes had the oblique flashlight grade ≤2, and tended to be increasing with age (x2 = 8. 597,P<0. 05) and in females (x2 = 14. 89,P<0. 01). Based on temporal anterior chamber angle ≤2 (Shaffer's grade), the obligue flashlight grade≤2 found that occludable angle sensitivity was 91. 7%, specificity was 91. 5% , and 8. 16% of the eyes had primary closure angle glaucoma. No patient was found in oblique flashlight grades ≥3. Conclusion: This study shows that oblique flashlight test conforms to screening test needs, and plays an important role in screening for primary closure angle glaucoma. We suggest that oblique flashlight grades ≤2 as cutoff point of screening for occludable angle. Eye Science 1995; 11 : 177-179.
