摘要
目的:探讨α干扰素(αIFN)加卡介苗(BCG)膀胱灌注对复发性浅表性膀胱癌术后再复发的预防效果.方法:对保留膀胱手术治疗的复发性浅表性膀胱癌患者56例,于术后随机分为2组,组Ⅰ(30例)用αIFN300万IU60mgBCG,组Ⅱ(26例)用丝裂霉素C40mg,均溶于生理盐水40mL膀胱灌注,每周1次,6次后改为每月1次,共2a.并在αIFN加BCG组灌注前后检测尿中白细胞介素2(IL2)及肿瘤坏死因子(TNF)水平.结果:56例全部随访,平均随访时间26.8(24～36)mo,αIFN加BCG组再复发率为7%(2/30),显著低于丝裂霉素C组(42%,11/26)(P<0.01).αIFN加BCG灌注后尿液中IL2及TNF水平均显著高于治疗前(P<0.01).两组并发症发生率差异无显著性(P>0.05).结论:αIFN加BCG膀胱灌注预防复发性浅表性膀胱癌术后再复发效果优于丝裂霉素C.
AIM: To investigate the efficacy of intravesical instillation of α-interferon (αIFN) plus BCG in the prophylactic treatment of recurrent superficial bladder cancer. METHODS: Fifty-six patients with recurrent superficial bladder cancer were divided randomly into two groups after tran- surethral resection of bladder tumors (TURBT) or partial cystectomy: groupⅠ (n=30) treated with BCG 60mg plus α IFN 3MU and group II (n=26) treated with mitomycin C. The drugs were mixed in 40 mL of normal saline and instilled into the bladder via a urethral catheter, once a week for six weeks and then once a month for a total course of two years. In groupⅠ, the levels of interleukin-2 (IL-2) and tumor necrosis factor (TNF) in urine were detected before and after instillation. RESULTS: All the patients were followed up successfully (mean 26.8 months, ranging 24~36 months). The follow up showed that the recurrence rate was 7% (2/30) in groupⅠ, lower than that in group II (42%, 11/26) (P<0.01) and the incidence of side effects in two groups was similar(P>0.05). Compared with those pre-therapy, the urinary IL-2 and TNF levels increased significantly after intravesical α INF plus BCG therapy (P< 0.01). CONCLUSION: The effects of intravesical instillation of α IFN plus BCG is better than mitomycin in the prophylactic treatment of recurrent superficial bladder cancer.
