摘要
对33例有第三磨牙异常的颅颌功能紊乱患者进行了、髁突位置、TMJ上腔造影及翼外肌上下头肌电检查。结果表明,髁突移位与第三磨牙异常有明显的关系,其关节弹响的主要机理可能是由紊乱导致髁突移位,继之出现盘-突关系不调所致。而另一些患者髁突移位出现在弹响对侧关节,其翼外肌上下头电位异常且类似于弹响侧翼外肌功能紊乱及非弹响但有明显疼痛者的翼外肌电位。作者认为,翼外肌功能紊乱可以是上述颅颌功能紊乱的早期病变,是导致弹响增强的另一途径。异常导致下颌运动异常可能是其主要原因。
AbstractA total of 33 craniomandibular dysfunction(CMD) patients with third molar malocclusion werestudied for their occlusion, condyle position, theTMJ arthrograph and EMG of upper and lower lateralpterygoid muscle (Lpt). The results showed that thecondyle displacement was related to the malocclusionof third molar, of which it may induce TMJ click asthe result of loss of disccondyie harmonious relationship. Most of the patients showed the abnormalEMG of Lpt. Among which some click TMJ showedno obvious condyle displacement while on the othersfelt severe pain without TMJ click. It can be consid-ered that both the malocclusion of third molar anddysfunction of Lpt may be the cause of TMJ click.
