PCR检测17例急性早幼粒细胞白血病缓解后微量残留白血病 被引量:13

PML-RARα FUSION TRANSCRIPTION IN REMISSIONPATIENTS WITH ACUTE PROMYELOCYTIC LEUKEMIA

摘要 采用逆转录聚合酶链反应检测急性早幼粒细胞白血病缓解患者的残留早幼粒白血病-α维甲酸受体融合基因(PML-RARα)。缓解期在3年之内的10例中9例阳性,1例阴性,缓解期在3年以上者7例,4例阴性,3例阳性,两组阳性率相差显著,P<0.05。在PML-RARα表达阳性的患者中已有3例复发,1例复发趋势,5例PML-RARα阴性患者已随访11±4月均无复发迹象。对阴性患者放松治疗,对阳性患者继续治疗,4例原已停止治疗的患者结果阳性,继续治疗后目前随访12个月(中数),尚无复发证据。 n this study nesting polymerase chain reaction ofreverse transcription was employed to investigate thefusion transcripts of PML-RARα in 17 remissionpatients with acute promyelocytic leukemia. Afterachieving complete remission (CR) by the induction ofretinoic acid, the transcripts of the fusion gene werepositive in 9 and negative in 1 out of the 10 patientswhose CR durations were less than 3 years. However,among 7 cases with CR duration over 3 years, 4 caseswere negative and 3 positive. The difference betweenthe two groups was statistically significant(P<0. 025).Among all the positive patients, relapse was proved in 3cases and relapse tendancy in one patient. No evidenceof relapse was found in 5 negative patients at 11± 4months' follow-up. Further treatment was thereforegiven to 4 positive patients who already ceased therapy.They all remained in CR status in the following 12(median)months' observation. So we considered thatthe PML-RARα gene might represent the minimalresidual disease of APL. Its fusion transcripts wereclosely associated with relapse and the detection of itcould guide effectively clinical continuation treatment.
