丙型肝炎病毒核酸定量测定的临床价值 被引量:13

摘要 目的 研究丙型肝炎患者血清中丙型肝炎病毒核酸(HCV-RNA)载量与抗-HCV含量以及丙氨酸氨基转移酶(ALT)水平之间的相关性与符合性。方法 用FQ-PCR法检测128份怀疑HCV感染的血清中的HCV-RNA,同时检测ALT和抗-HCV。结果 128份样本中HCV-RNA阳性率为53.9%(69/128);抗-HCV阳性率为62.5%(80/128);ALT异常率为51.0%(65/128)。HCV平均载量为8.52×105 拷贝/ml血清。抗-HCV滴度和ALT异常随着HCV-RNA载量升高而增加。结论 用FQ-PCR法检测HCV-RNA可以确定(1)抗HCV与抗-HBs不同,不是中和抗体,(2)高滴度抗-HCV血清具有传染性。(3)抗-HCV滴度与HCV-RNA浓度之间呈正相关性r=0.952, P<0.001,且有符合性和互补性,(4)对慢性丙型肝炎长期抗-HCV阳性,若出现HCV-RNA阴性或阳性可以鉴别活动性、复制性程度,(5)是评价丙型肝炎抗病毒治疗效果的重要指标。 Objective To study the correlativity and communality between serum HCV-RNA,HCV antibody and ALT levels. Methods Hepatitis C virus RNA(HCV-RNA) was measured by Fluorescence quantitative PCR(FQ-PCR) in 128 patients,and compared them with the results of ALT and HCV antibody.Results In all 128 cases,HCV-RNA positive rate are 53.9%,about 69 in 128,HCV antibody positive rate are 62.5%,about 80 in 128,and ALT abnormal rate are 51.9%,about 65 in 128.The average HCV-RNA was 8.52×10 5 copies/ml.The positive rate of HCV antibody and abnormal rate of ALT are all increase along with HCV-RNA rise.Conclusion The detection of HCV-RNA by FQ-PCR can be used to measure such facts:(1)Different with HBs antibody,HCV antibody are not a neutralize antibody;(2)Serum with high level HCV antibody have high infection;(3)HCV antibody and HCV-RNA have high communality and complementary;(4)For some chronicle hepatitis C patients with long time HCV antibody positive,HCV-RNA can be used to distinguish disease;(5)HCV-RNA is an important index to evaluated the effect of HCV patients' anti virus therapy.
