摘要
目的分析干酪坏死性附睾结核高频彩色多普勒声像特征及其与临床、病理关系。方法对资料齐全的12例经手术及病理证实的干酪坏死性附睾结核高频彩色多普勒检查结果及临床资料进行回顾性分析。结果(1)附睾结核症状隐匿,病程较长,平均年龄39.7岁,多数以单侧阴囊肿块就诊。本组资料结核菌素实验(PPD)阳性为67%,患肺结核者为42%。(2)附睾结核高频多普勒声像特征:附睾局部或全部肿大,外形失常,病变处呈不规则、不均质中等偏强或低回声,兼有局限性较小的不规则、均质低或无回声区,边界不清;高频彩色多普勒能量(CDPI):部分病灶见丰富或较丰富以动脉为主动、静脉血流信号。(3)继发改变:患侧睾丸肿大,内回声增强,患侧睾丸鞘膜积液。结论干酪坏死性附睾结核高频彩色多普勒声像与其解剖、临床及病理密切相关,超声对其诊断及鉴别诊断具有重要临床意义。
Objective To analyze ultrasonic feature of epididymis tuberculosis of caseous necrosis and its relationship with clinic and pathology. Methods Ultrasonic feature of 12 cases with epididymis tuberculosis of caseous necrosis,confirmed by surgical-pathology,was retrospectively analysed. Results 1.The epididymal tuberculosis of caseous necrosis could present images of the whole or focal epididymal enlargement and abnormal shape. 2. The lesion internal echoes were irregular, inhomogenous increased isoechoic echogenicity or hypoechoic, including bad-defined, irregular, homogenous hypoechoic or anechoic. In lesion location out of the 12 patients, 4 (33%) possessed rich color blood flow in Color Doppler Power Imaging(CDPI). 3.The testicular enlargement with increased internal echoes and the testicular hydroceles were partly ( 58%,7/12) observed by ultrasound. Conclusions The High Frequency Color Doppler and clinical features were helpful to diagnosis and differential diagnosis of the epididymis tuberculosis of caseous necrosis.
