别嘌呤醇对心肌再灌注损伤的保护作用

摘要 通过对心肌氧自由基、钙含量和心肌酶直接检测,超微结构的观察,评价别嘌呤醇对开心手术心肌的保护作用。结果显示:别嘌呤醇组再灌期氧自由基信号强度、心肌钙含量及酶漏出量均低于对照组;心肌超微结构损伤亦较对照组轻。说明别嘌呤醇对心肌再灌损伤有保护作用。 This study is to evaluate the myocardial protective effect of allopurinol. Forty consecu- tive patients undergoing open heart surgery were randomly divided into two groups of 20 pa- tients each. Patients in the control group received no allopurinal;those in the other group re- ceived allopurinol at a dose of 20mgkg^(-1) day^(-1) for three days preoperatively. Before and after 30rain of aortic crossclamping,and after 15min of reperfusion,myocardium were sampled to detect its oxygen free radical (OFR)signals ,calcium contents,and ultrastructure;serum car- diac isoenzyme of creatine kinase (CK-MB) were also measured. OFR signals in the allop- urinol group were significantly lower than that in the control during reperfusion period (P< 0.01); myocardial calcium contents, CK-MB showed similar changes (P<0.01). Ultra- structure study confirmed less damage in the allopurinol group. It was concluded that allop- urinol has myocardial protective effect on cardiac reperfusion injury in patients undergoing cardiac surgery.
