摘要
本地１４例法乐四联症根治术的麻醉处理进行了总结分析。本组病例术中平稳，无麻醉死亡，其麻醉成功的要点是：由于这类病人心肺功能低下，术中应避免使用抑制呼吸循环的药物；体温降到２５℃以下，加上心脏深低温有利于降低实质器官的耗氧；适当的过度换气可减低肺血管的阻力；保持循环稳定可减少低心排的发生。
We present our experience with complex vein-anesthesia in 14 patients with Fallot tetrad for total surgical correction. All patients received complex vein-anesthetization during the surgery,obtaining a satisfactory effect. Also the date of the status of respiration and circulation during the corrective surgery were analyzed. It is concluded that following factors are esential for the successful vein-anesthesia during the surgical correction of Fallot tetrad: l. The drugs, whice may inhibit cardiac-pulmonary function, should be avoided to use during the surgery, because of the inadeqate cardiac-pulmonary status in these patients, 2. Lower temperature (below 25℃) and the heart' s lower temperature (below 15℃) may protect the tissue from insufficiency of the oxygen;3. An adequate overventilation is beneficial in decrease of pulmonary vascular resistance; 4. Sustaining the stable circulation during the anesthesia may reduce the frequency of low cardiac output after the operation, all patients were stable during the surgery, not occured anesthesia death.
