摘要
在25例二尖瓣返流患者中,利用连续波多普勒和双心导管技术同步测量了静息和负荷状态下30例次的二尖瓣返流压差和左房压。结果显示,多普勒测量的最大返流压差与心导管测量的最大和峰间返流压差均呈高度相关(r分别为0.99和0.91),多普勒测量的收缩期左房压与心导管测量的收缩期和平均肺小动脉嵌顿压亦高度相关(r分别为0.89和0.84),表明连续波多普勒是估测二尖瓣返流压差和左房压的可靠技术。
To develop a new method for noninvasively estimating the left atrial pressure (LAP) in patients with mitral regurgitation (MR), Doppler echocardiography (DE) and cardiac catheterization (CC) were performed simultaneously in 25 cases with MR. The results showed that: 1. there was a high correlation between the brachial arterial systolic pressure (BASP) measured by a cuff sphygmomanometer and the left ventricular systolic pressur (LVSP) recorded by CC (r=0.97); 2. the peak regurgitant pressure gradient (PG—PD) measured by DE correlated highly with the peak and the pea—to—peak regurgitant pressure gradient measured by CC (r=0.99 and 0.91, respectively); 3. the Doppler—derived systolic left atrial pressure, which was calculated by subtracting PG—PD from BASP, well agreed with catheterization—measured systolic and mean pulmonary capillary wedge pressures (r=0.89 and 0.84, respectively). We conclude that our method offers a new and reliable approach to the noninvasive estimation of LAP in patients with MR.
