摘要
本文报道5例黄色肉芽肿性肾盂肾炎(XGPN)超声表现。根据其病理变化声像图可分为两型:①弥漫型或混合病变型4例。表现为肾形态失常、体积显著增大,肾内结构紊乱,可探及多个无回声或/和低回声区。此型常伴有梗阻、结石、感染相应的声像图改变。②局灶型1例。声像图表现为肾轻度增大,肾内可见均匀或不均匀实性(或以实为主)结节回声。无合并梗阻、结石和感染相应的声像图改变。作者认为本病声像图缺乏特异性,晨尿离心找泡沫细胞和超声引导下组织学活检对本病术前诊断有重要意义。本文对局灶型患者进行成功的经验性非手术治疗方法进行讨论。
Sonographic features of Xanthogranulomatous Pyelonephritis (XGPN) in 5 cases were reported. According to their pathologic changes, the sonographic findings of XGPN could be devided into two patterns. 1, Diffuse type or mixed type (4 cases). The renal volume was increased and renal contour was abnormal. The normal echo—structures were diffusely changed. In some part of the lesion, low level echo or echo—free areas could be found, and often associated with renal hydronephrosis, stone or infection appearance. 2. Focal type (1 case). A solid tumor nodule could be seen but other sonographic changes above mentioned were rare. The authors considered sonographic appearance of XGPN was not specific. Urine culture, microscopic finding of foam cells and tissue—core biopsy under ltrasonic guidance were valuable for preoperative diagnosis. A empirical but successful conservative treatment in one case with focal type was discussed.
