摘要 1990年3月~1992年12月对55例脊柱外科手术患者进行了脊髓诱发电位(SCEP)术中监测,无1例出现不可逆的神经损伤并发症。术前脊髓神经功能正常组27例中有2例出现了电位波幅明显变化。25例术前伴有不全截瘫,在脊髓减压后出现振幅增大、潜时加快的电位图象,术后患者神经功能障碍有了明显的改善。3例完全性截瘫者,术中脊髓诱发电位波幅消失,但在减压后有1例出现了电位波幅,术后截瘫得到了部分恢复。在硬膜外放置刺激及记录电极线记录SCEP的方法可得到大而稳定的电位波幅,能够准确地反应出脊髓功能的即时变化。 During spine surgery, 55 patients had spinal cord evoked potential (SCEP) monitoring of the spinal cord from Mar. 1990 to Dec. 1992. No one case complicated with irreversible neurological damage. Among 27 cases with normal neurological function, 2 cases showed obviously potential wave changes. All of the 25 cases with incomplete paraplegia showed increase of amplitude and acceleration of latency. After operation, neurological dysfunction of the patients were improved. 3 cases with complete paraplegia showed disappearance of SCEP wave during the operation, but one of them showed potential wave recovered after decompression and the paraplegia recovered partially later. A large and stable potential wave can be obtained if putting the stimulating and recording electrodes into the epidural space and therefore the incentive chang of spinal cord function can be accurately displayed on the screen.
