摘要
目的:探讨原发性高血压(EH)患者24 h平均脉压(PP)与左心室肥厚(LVH)的关系。方法:对60例EH患者进行24 h无创性动态血压监测,并根据有无LVH分为两组:非LVH组(22例)、LVH组(38例),其中LVH伴心功能不全者17例,心功能正常者21例进行观察。结果:(1)LVH组心功能正常者24 h平均收缩压(SBP)、24 h平均PP显著大于非LVH组(P<0.01);(2)左室质量指数、相对室壁厚度与24 h平均SBP、PP呈正相关(P<0.01);(3)LVH组伴或不伴心功能不全者24 h平均SBP、PP明显高于非LVH组(P<0.01),LVH组伴心功能不全者24 h平均PP高于LVH心功能正常组(P<0.05)。结论:24 h平均PP增大在EH伴LVH的发展中起作用,且对靶器官损害程度有预测价值。
Objective: To investigate the relationship between average 24 hour ambulatory pulse pressure and Left ventricular hypertrophy(LVH) in essential hypertensive patients. Methods: Ambulatory blood pressure monitor (ABPM) was done in 60 hypertensive patients according to clinical stages, which also were divided into two groups-one group with LVH and the other without LVH in left ventricular. Results: (1)The average 24 - hour ambulatory SBP and PP were significantly increased in the group with LVH than those in the group without LVH(P< 0.01); (2) A positive correlation was found between average 24 - hour ambulatory PP and LVMI or relative ventricular wall thick(RWT)(.P < 0.01) , as well as 24 - hour ambulatory SBP and LVMI or RWT(P < 0.01); (3)The level of average 24 - hour ambulatory SBP and PP in the stage Ⅱ and Ⅲ group were marked higher than those in the stage Ⅰ group(P< 0.01), and the level of average 24 - hour ambulatory PP in the stage Ⅲ group was higher than that in the stage Ⅱ group(P < 0.05). Conclusions: The increase of average 24 - hour ambulatory PP plays the important role in the process of LVH, and could be of prognostic value in estimating the degree of target - organ damage in essential hypertensive patients.
