摘要 目的 评价INNO LiPA乙型肝炎病毒基因分型的方法 ,并探讨了基因型与临床疾病的关系。方法 随机选取 113例慢性HBV感染者外周血采用INNO LiPA和S基因序列分析两种方法测定HBV基因型。结果  1 INNO LiPA基因分型法与S基因序列分析法测定的基因型结果比较 ,符合率 82 3% (93/ 113) ,误判率 3 5 % (4/ 113) ,B/C型混合感染检出率 5 3% (6 / 113)。 2 LC组和HCC组C基因型比率高于AsC组 ,差异有显著性 (分别为 92 9%比 5 2 8% ,X2 =7 0 3,P <0 0 1和 88 9%比 5 2 8% ,X2 =3 91,P <0 0 5 ) ;LC组C基因型比率高于CHB组 ,差异有显著性 (92 9%比 6 1 1% ,X2 =5 12 ,P <0 0 5 )。结论  1 INNO LiPA法是一种较灵敏的快速检测HBV基因型的实验方法 ,尤其检测混合基因型感染灵敏度高。 2 C基因型HBV感染与较重肝病有关。 Objective To evaluate the INNO LiPA HBV genotyping assay for determination of hepatitis B virus genotype and to investigate clinical significance of hepatitis B virus genotype Methods Sera from 113 patients chronically infected with HBV were detected HBV genotype was determined by INNO LiPA and S gene sequencing Results 1 Complete concordance between INNO LiPA and sequence analysis was observed for 93 specimens (82 3%) INNO LiPA was able to detect 6(5 3%) B/C mixed genotype infections not detected by amplicon sequencing Four specimens(3 5%) had discrepant results between the two methods 2 The rate of genotype C infection was higher in patients of LC and HCC than that in AsC(92 9% vs 52 8%, X 2=7 03, P <0 01 and 88 9% vs 52 8%, X 2=3 91, P <0 05); and there was significant difference in the distribution of genotype C between LC patients and CHB patients(92 9% vs 61 1%, X 2=5 12, P <0 05) Conclusions 1 The INNO LiPA HBV Genotyping assay appears to be useful for the rapid genotyping of HBV, particularly for the sensitive detection of mixed genotype infections 2 Genotype C HBV may induce more severe the liver inflammation than genotype B may do
