尿转化生长因子β对慢性移植肾肾病影响的研究 被引量:1

摘要 目的 探讨肾移植受者尿转化生长因子 β1(TGF β1)与慢性移植肾肾病 (CAN)的关系。方法  1999年 8月~ 2 0 0 1年 1月期间对术后满 1年、移植肾功能正常的患者进行 3年以上的前瞻性观察 ,至少每 6个月检测 1次血、尿肌酐 (Cr)、TGF β1等 ,对肾功不全者明确是否为CAN。 3年后共有 116例患者完成了全程随访 ,将其肾功能与 3年前的尿TGF β1浓度作相关性分析 ,判断二者是否相关 ;将其中 12例CAN的患者 ,比较其在肾移植 1年时的血、尿TGF β1等与其他非CAN患者有无差异。结果 患者尿TGF β1浓度与 3年后的肌酐清除率 (Ccr)之间有明显的相关性 (r =0 .43 1,P <0 0 1) ;CAN与非CAN患者在肾移植 1年时 ,尿TGF β1相对浓度分别为 2 77± 3 2 .9和 184± 40 4(pg/mg .Cr) (两者比较有显著性差异 ,P <0 0 1) ,血TGF β1浓度分别为 3 1 7± 4 8和 3 1 2± 5 1(ng/ml) (两者比较没有差异 ,P >0 0 5 )。结论 TGF β1可能在CAN的发生过程中起着重要的作用 ,CAN患者肾功能异常前尿TGF β1已显著升高 ,肾移植后早期检测尿TGF Objective To determine the association between urine TGF-β 1 concentration and chronic allograft nephropathy(CAN).Methods From August 1999 to January 2001,renal recipients who had kidney transplantation for one year and whose renal functions were normal were investigated.Their blood and urine TGF-β 1 concentrations were tested in an interval of at least 6 months.Three years later,there were 116 cases who had been followed up for the whole investigation.Correlation between their renal functions(creatinine clearance rates)and their urine TGF-β 1 relative concentrations after 1 year of renal transplantation were determined.Of the 116 cases are 12 renal recipients who were diagnosised as CAN.Their blood and urine TGF-β 1 concentrations and other factors after 1 year of renal transplantation were compared with the normal renal recipients who had no CAN.Results There were a positive correlation between long-term renal functions(creatinine clearance rates)and urine TGF-β 1 relative concentrations after 1 year of renal transplantation(r= 0.431,P<0.01).One year after renal transplantation,the urine TGF-β 1 concentrations of CAN and no-CAN recipients were 184±40.4 and 277±32.9(pg/mg.Cr)respectively.The difference is significant.The blood TGF-β 1 concentrations of CAN and no-CAN recipients were 31.7±4.8 and 31.2±5.1(ng/ml)respectively.They were not different.Conclusion The findings suggest that urine TGF-β 1 is significantly higher before renal dysfunction in patients with CAN.The level of urine TGF-β 1 in early stage after renal transplantation can predict long-term renal function.
