摘要
目的 了解血透对慢性肾功能衰竭患者营养状况的影响。方法 用体检 ,生化分析和调查问卷等方法对30例住院血透患者及36例门诊血透患者透析前后的营养状况进行评价。结果 30例住院血透患者透析后的淋巴细胞计数、血红蛋白、白蛋白、前白蛋白、转铁蛋白、MNA评分等较前均有显著上升 (p<0.05) ,体重指数、三头肌皮褶厚度、上臂肌围、体内脂肪含量、甘油三酯改变不明显 (p>0.05) ,而36例门诊血透患者透析后的淋巴细胞计数 ,甘油三酯及MNA平分升高较透析前有显著性差异 (p<0.05)。体重指数、三头肌皮褶厚度、体内脂肪含量、血红蛋白、白蛋白、前白蛋白、转铁蛋白改变不明显 (p>0.05) ,上臂肌围较透析前有明显下降 (p<0.05)。结论 住院血透患者予以血透治疗同时给予正规的营养支持和药物治疗和及时合理的饮食结构的调整 ,可以改善慢性肾功能衰竭患者的营养状况。而门诊血透患者血透治疗同时缺乏正规的营养支持、药物治疗和及时合理的饮食结构的调整是其低蛋白血症 ,贫血改善不理想的主要原因 ,加强门诊血透患者的饮食结构的合理调整及营养支持治疗是关键。
Objective To explore the dialysis effect on patients nutrit ion status with chronic renal failure(CRF). Methods The nutrition status of30cases with dialysis in hospital and36cases with dialysis at out-patient department were evaluated by the physical examination and the biochemical assay. Results 30cases with dialysis in hospital had higer LYM,albumin,total protein,pre-albumin,TRF,MNA score(p<0.05),and BMI,TSF,AMC,FAT%had not obvious change.36cases with dialysis at out-patient had lower AMC,and others had not obvious change. Conclusion The patients with dialysis in hospital when they were curing in dialysis had so regular cure in nutrition,medication and logical diet that their malnutrition had markedly been improved.The patient dialysis at out-patient hadn't been improved.The key was to enhance the patient's cure in nutrition and medication.
