摘要
目的:分析研究髋关节手术损伤臀上血管后并发臀内侧上部坏死的解剖学基础。方法:在40侧经动脉内灌注红色乳胶标本,解剖观察梨状肌上孔、臀上血管神经走行及分支分布,髂内动脉造影观察吻合情况。结果:梨状肌上孔为半圆形,中点高(11.6±2,4)mm,下界宽(22.3±4.2)mm;梨状肌上孔内侧缘骨壁厚(7.5±1.2)mm;臀上动脉外径厚(2.3±1.3、)mm;外径宽(4.8±1.0)mm;臀上动脉深支分布臀中小肌;浅支外径厚(0.9+0.6)mm;外径宽(2.7±0.8)mm,分布臀大肌内侧上部。臀上静脉1支型占30.0％,2支型占70.0％;臀上静脉浅支外径厚(0.6±0.2)mm,外径宽(0.9±0.6)mm。结论:术中将臀大肌向内推臀上动、静脉挤压至梨状肌上孔内侧缘骨壁上,致臀上血管断裂,并发臀内侧上部软组织坏死。
Objective: To provide the anatomical data on the necrosis of medial superior part of gluteal region due to injuring the vessel via suprapiriformis foramen during the operation of hip jiont. Methods: The shape of suprapiriformis foramen, the path of superior gluteal vessel, nerve and the distribution of those branchs were observed on the 40 sides of adult cadaver perfused with the red latex, and the vascular anastomosis of internal iliac artery was observed by the method of arteriography. Results: The shape of suprapiriformis foramen was half-circle, its vertical diameter of middle point was 11.6±2.4 mm, its transverse diameter of lower wall was 22.3±4.2 mm, and its thickness of medial wall was 7.5±1.2 mm. The vertical diameter of superior gluteal artery was 2.3 ±1.3mm,its transverse diameter was 4.8 ±1.0 mm.The deep branch of superior gluteal artery distributed to the middle gluteal muscle and the least gluteal muscle. The vertical diameter of the superficial branch of superior gluteal artery was 0.9 ±0.6 mm, its transverse diameter was 2.7 ±0.8 mm.The superficial branch of superior gluteal artery distributed to the medial superior part of the greatest gluteal muscle. The percentage of one branch type of superior gluteal vein was 30.0%, two branch type of Superior gluteal vein was 70.0%. The vertical diameter of the superficial branch of superior gluteal vein was 0.6 ± 0.2mm, its transverse diameter was 0.9 ±0.6 mm. Conclusions: During the operation, when greatest gluteal muscle is pushed to medial direction and superior gluteal artery and vein are extruded to the medial wall of suprapiriformis foramen, superior gluteal vessel will be ruptured, and the necrosis of medial superior part of gluteal region broke out simultaneously.
