摘要
目的 描述不典型部位心包囊肿的影像学特点 ,提高对不典型部位心包囊肿的认识。方法 回顾性研究我院手术病理证实的 5例不典型部位心包囊肿的CT(n =5 )和MRI(n =2 )及临床手术资料。结果 4例位于右侧气管旁区 ,1例位于升主动脉根部前方。 3例CT表现为水样密度 ,2例为软组织密度。MRIT2WI表现为近似于脑脊液的长T2信号 ,T1加权像上呈长T1和等T1信号各 1例。手术见病变通过蒂 (n =4)或直接与心包相连 (n =1) ,其中 2例与心包相通。结论 ①右侧气管旁区和升主动脉前方的囊肿的鉴别诊断应包括心包囊肿。
Objective To characterize the imaging features of atypically located pericardial cysts (ALPC) and to improve the understanding of manifestations of ALPC. Methods The CT and MR images and surgical records in 5 histopathologically proved cases of ALPC were retrospectively reviewed. Results Four cysts were located in the right paratracheal region. One was in the anterior mediastinum abutting the ascending aorta. They demonstrated fluid attenuation that was close to water (n=3) or soft tissue density on CT (n=2). One appeared long T1 and long T2 signal intensities that were close to CSF on MR images and the signal intensity did not decrease on MR hydrography. One had long T2 signal intensity that was close to CSF and medium T1 signal intensity without enhancement after intravenous administration of Gd-DTPA. At surgery, the cyst was connected to pericardium by a stalk in 4 cases and directly in 1 case among which 2 were communicated with pericardium . Conclusion ①Pericardial cyst should be taken into consideration in the differential diagnosis of cystic lesions located in the paratrachea and in the anterior mediastinum abutting the ascending aorta. ②CT and MRI can be useful to show the cystic features and the relationship of the cysts with the surrounding structures and MRI is superior to CT. [
