摘要
目的 探讨血清新喋呤水平与多发伤严重程度及并发症之间的关系。方法 用ELISA法检测多发伤病人伤后 2 4h、第 3、7和 14天及对照组健康人血清新喋呤水平 ,明确新喋呤水平在伤后不同时段的动态变化 ,分析其与多发伤严重程度以及与预后的关系。结果 多发伤病人血清新喋呤水平在第 3天后开始升高 ,第 7天左右达到高峰 ,以后逐渐下降 ;ISS评分值与血清新喋呤水平呈显著正相关 (r =0 5 0 8,P <0 0 5 )。MODS积分与血清新喋呤水平呈显著正相关 (r =0 795 ,P<0 0 5 )。结论 多发伤病人血清新喋呤水平与ISS值呈显著正相关 ;合并MODS者血清新喋呤水平显著升高 ,动态监测其变化 ,对预警MODS的发生和预测预后具有一定的意义。
Objective To evaluate the relationship between complication, multiple trauma degree and neopterin level in the patients with multiple trauma. Methods Thirty patients with multiple trauma and thirty healthy persons were taken in the study.The levels of serum neopterin were tested by ELISA technique on the 1st, 3rd, 7th and 14th post-traumatic days. Results The serum neopterin level in patients with multiple trauma began to rise after 3 days.It reached the peak on the 7th day,and then reduced gradually;ISS score was positive correlative with the serum neopterin level (r=0 08,P<0 0 ).The serum neopterin level in patients complicated with MODS reached the peak on the 7th day, were significantly higher than that of the patients non-complicated with MODS (P<0 0 ).MODS score was positive correlative with the serum neopterin level (r=0 79 ,P<0 0 ). Conclusion ISS score is positive correlative with the serum neopterin level,and it may be of great value to estimate the injury severity;The serum neopterin level in the patients complicated with MODS is positive correlative with MODS score. Dynamical measurement of the change of serum neopterin level may predict and estimate the occurrence of MODS and the prognosis of multiple trauma. [
