摘要
目的 研究硝酸甘油 (nitroglycerin ,NG)和丁丙诺啡(buprenorphine ,BU)两药单用及合用抗心肌缺血的药理性预适应的延迟相保护作用。方法 ♂大鼠分为 5组 ,Sham组静注等容积NS ,DIP组行肠系膜上动脉结扎 10min处理 ,BU组静注盐酸丁丙诺啡 1 0mg·kg- 1,NG组静注NG 0 3mg·kg- 1,B +N组分别给上述剂量BU和NG。 2 4h后各组行 3 0min冠脉结扎缺血 /2h再灌。各组均于术后 15min、缺血期和再灌期连续监测心率、血压、ST 段和心律失常情况 ;测定缺血 3 0min和再灌 2h血浆LDH、CK ;再灌 2h后行HE染色与TTC染色定性和定量测定心肌坏死情况。结果 与Sham组比较 ,DIP组可降低心脏缺血期ST段抬高幅度 (P <0 0 1) ,推迟VPC出现时间 ,缩短持续时间 (P <0 0 5) ,降低二联律发生率 ,降低缺血期血浆LDH水平 (P <0 0 5) ,缩小心肌坏死面积 (P <0 0 5)。BU组可降低心脏缺血期ST段抬高幅度 (P <0 0 1) ,推迟VPC出现时间 ,缩短持续时间 (P <0 0 5) ,降低二联律和室颤的发生率 (P <0 0 5) ,降低缺血期血浆LDH水平 ,缩小心肌坏死面积 (P <0 0 5)。NG组可降低ST段抬高幅度 (P <0 0 1) ,缩短VPC持续时间 (P <0 0 1) ,降低二联律、室速和室颤的发生率 (P<0 0 1) ,明显缩小心肌坏死面积 (P <0 0 1) ,减轻心肌坏死?
AIM To study the delayed cardioprotection eff ec ts of pharmacological preconditioning of nitroglycerin (NG) and buprenorphine (B U) used alone or in combination on myocardial ischemia in rats. METHODS Male Wistar rats were randomized to 5 groups. Normal saline (NS) was inf used intravenously with the same volume of NG and BU in Sham group. The anterior mesenteric artery was ligated for 10 min in delayed ischemic preconditioning (D IP) group. BU (1 0 mg·kg -1 ) and NG (0 3 mg·kg -1 ) was administere d intravenously alone or in combination in BU, NG and B+N group respectively. Th e ischemia and reperfusion injury were induced by ligation of left arterior desc ending coronary artery for 30 minutes and followed 2 hours reperfusion at 24 h a fter first operation. Heart rate, blood pressure, ECG ST-segment and arrhythmia s were recorded continuously throughout the whole test. The plasma LDH and CK we re measured. on 30min after ischemia and 2 h after reperfusion. HE and TTC stain ing were performed to determine the myocytes necrosis at the end of test. RESULTS Compared with sham group,DIP group reduced ST-segment during ischemia ( P<0 01),delayed the onset of ventricular premature contraction (VPC),shor tened the duration of VPC (P<0 05),decreased the incidences of bigeminy,l owered the plasma LDH concentration during ischemia (P<0 05),reduced the size of infarction of myocardium ( P<0 05). BU group reduced ST-segment during ischemia(P<0 01),delayed t he onset of VPC,shortened the duration of VPC(P<0 05),decreased the incid ences of bigeminy and VF(P<0 05),reduced the size of infarction of myocard ium,lowered the plasma LDH concentration during ischemia,reduced the necrosis of myocardium. NG group reduced ST-segment during ischemia significantly (P <0 01),shortened the duration of VPC (P<0 01),reduced the incidences of bigeminy,ventricular tachycardia (VT),VF (P<0 01), reduced the size of in farction of myocardium significantly(P<0 01),reduced the necrosis of myoca rdium. B+N group reduced the ST-segment significantly (P<0 01),decreased the incidences of bigeminy,VT,VF,reduced the size of infarction of myocardium (P<0 01),lowered the level of LDH in plasma(P<0 01). The onset and the duration of VPC and the incidences of VA were all better than those of NG or BU alone in B+N group (P<0 05). CONCLUSION NG and BU had th e significant delayed cardiaprotection effects of pharmacological preconditionin g on myocardial ischemia in rats. Combined used BU and NG were more effectively than used them alone.
