摘要
目的探讨经尿道前列腺汽化电切术(TUVP)术中、术后常见并发症及产生原因,提高TUVP的治疗效果。方法对118例前列腺增生症患者行TUVP术,平均年龄66.5岁,术前及术后测血RT,血Na和Bs,术后随访3～6个月。结果本组无死亡病例,术中平均出血约150ml,血RT、血Na+和Bs变化不明显。发生电切综合征(TURS)先兆4例,术后尿道狭窄5例,继发出血1例,尿路感染6例,阳痿3例,急迫性尿失禁2例。结论TUVP术中严密止血,保持清晰的视野是预防TURS发生的重要措施。操作损伤是引起膀胱穿孔,尿道损伤、尿道狭窄的重要原因。术前,改善心肺功能,控制尿路感染及保持前列腺窝内平整,可提高手术的安全性,减少尿路感染的发生。
Objectiv To review the intra and postoperative complications of transurethral electrovaporization of prostate(TUVP).Methods TUVP was undertaken for 118 symptomatic BPH patients with a mena- age of 66.5.Blood routine,serum sodium were measured pre and post-operatively.The patients have been followed up at 3 and 6 months. Results The mean blood loss was 150ml.There were no obvious changes in hematocrit and serum sodium.TURS was noted in only 4.postoperative complications included urethral stricture in 5,bleeding in 2,urinary infection in 6,impotence in 3 and temporary urinary incontinence 2. Conclusion Operative injury is usually the cause of urethral stricture and a clear operative field is the key point to prevent surgical complications.
