摘要
目的 探讨系统性红斑狼疮 (systemiclupuserythemaitosus ,SLE)活动期与干燥性角结膜炎 (keratoconjunctivitiessicca ,KCS)、继发性干燥综合征及相关抗体的相关性。方法 对 90例SLE患者和 10 0例正常对照组的眼表检查及抗体检测资料进行分析。结果 眼表损害在所研究的SLE患者中的发生率为 2 5 6% ( 2 3 /90 ) ,正常对照组为 3 0 % ( 3 /10 0 ) ;且活动期发生率 3 4% ( 17/5 0 )明显高于非活动期的 15 % ( 6/4 0 ) ,P <0 0 5。抗dsDNA抗体、抗SSA抗体及抗SSB抗体在SLE患者的阳性率为 5 7 8% ( 5 1/90 )、 43 3 % ( 3 9/90 )及 2 8 9% ( 2 6/90 ) ,而正常对照组均阴性 ;且SLE患者活动期阳性率均明显高于非活动期患者 ,分别为 74% ( 3 7/5 0 )VS3 7 5 % ( 15 /4 0 ) ,P <0 0 1;5 6% ( 2 8/5 0 )VS2 7 5 % ( 11/4 0 ) ,P <0 0 1;以及 3 8% ( 19/5 0 )VS17 5 % ( 7/4 0 ) ,P <0 0 5。 2 3例SLE合并干燥性结角膜炎的患者抗SSA、SSB抗体阳性率分别为 10 0 0 % ( 2 3 /2 3 )和 91 3 % ( 2 1/2 3 )。 11例SLE继发性干燥综合征的患者抗SSA、SSB抗体阳性率分别为 10 0 0 % ( 11/11)和 90 9%( 10 /11)。结论 SLE活动期患者KCS发病率及抗dsDNA、SSA、SSB抗体阳性率明显高于SLE非活动期患者。抗SSA。
Objective To discuss the relationship among systemic lupus erythemaitosus(SLE) in active phase,keratoconjunctivities sicca,secondray Sjogren Syndrome and autoantibodies.Methods Analyse the clinical data of 90 cases of SLE,100 normal contrast cases in their ocular surface lesions and antibodies.Results The incidence of ocular surface lesion in the SLE cases in our study is 25 6%,3 0% in normal contrast group.The incidence in active SLE is much higher than that in nonactive SLE.The positive rate of antidsDNA,antiSSA and antiSSB is 57.8%,43.3% and 28.8% respectively in SLE group.While negative in normal contrast group.Positive rate in active SLE is much higher than that in nonactive group.They are 74% vs 37.5%,56% vs 27.5%,38.0% vs 17.5% respectively.Positive rate of antiSSA and antiSSB in23 cases of SLE complicated with keratoconjunctivities sicca is 100.0% and 91.3% respectivitely,that in 11 cases of secondary Sjogren′s syndrom is 100.0% and 90.9% respectively.Conclusions SLE in active phase have a higher occurrence rate in KCS and dsDNA-antibody,SSA-antibody and SSB-antibody than the SLE in nonactive phase.SSA-antibody and SSB-antibody can be one of the assistant diagnostic criterion in SLE complicated with keratoconjunctivities sicca and secondary Sjogren Syndrome.
