摘要
目的 建立简便、成功率高的大鼠肾移植模型。方法 SD大鼠为供受体 ,采用自行设计的供体腹主动脉搭桥、硬膜外导管内撑法重建动脉血供。供体后腔静脉袖套套入受体左肾静脉重建静脉回流。供体膀胱瓣与受体膀胱全层缝合。结果 整个移植过程 160min ,热缺血时间 <5s ,冷缺血时间 <80min ,血管吻合时间 40min。 41例手术中 ,3 5例成功 ,手术成功率 85 .4%。术后淋巴漏 2例 ,尿漏 1例 ,输尿管入膀胱处狭窄 1例 ,术后并发症发生率 11.4%。结论 本模型具有操作相对简单、对手术器械和术者的手术技能要求低、成功率高、并发症少等优点 。
Objective To establish an easy performing and high survival rate rat kidney transplantation model.Methods SD rats were used as both kidney donors and recipients.Arterial anastomosis was established by donor arteria abdominalis bypass and using extradural catheter as inner supporting tube.Venous anastomosis:donor's cuffed vena cava caudalis was inserted into the recipient's renal vein and secured in position using a circumferential silk suture.The recipient bladder and donor bladder patch were then anastomosed.Results The mean operating time was 160 min and hot ischemic time less than 5 s,cold ischemic time less than 80 min,the mean revascularization time was 40 min.After a successful anastomosis,the kidney color became a bright pink color within 1 min.Of 41 rat kidney transplants performed,35 survived the procedure and the success rate was 85.4%.Lymphorrhagia occurred in 2 cases,urinary leak in 1 case,and ureteral stricture in another with the rate of postoperative complications was 11.4%.Conclusion The new method of rat kidney transplantation stated in this study was simple,high successful and the complications rate was low.It had no special requirement of the instrumentations and the surgeon's microsurgical techniques.It might be a good animal model of fundamental research on kidney transplantation.
