实验性兔后肢淋巴水肿模型构建 被引量:4

An improved experimental model of obstructive lymphedema for observation

摘要 目的模拟临床肿瘤根治术后放疗患者的淋巴水肿形成过程,建立并观察一种改良的肢体淋巴水肿动物模型。方法应用传统的淋巴结根治与60钴照射相结合,制作兔后肢淋巴水肿模型。术后观察解剖形态、体积变化,并行淋巴核素造影、组织病理检查。结果动物死亡率为7.8%,在成活动物中模型成功率达100%,产生相应的组织形态变化及淋巴造影表现,其水肿形成规律与临床患者相似;兔后肢淋巴解剖与人较接近,大淋巴管直径约为0.6-0.9 mm。结论该模型成功率高,成本较低,便于观察;由于淋巴管口径较粗,尤其适用于显微淋巴外科的研究。 Objective Simulating the lymphedema of patients after radical resection of tumor and radiotherapy, an improved model with obstructive lymphedema in a limb was established and observed. Methods Modelling the clinical process, traditional radical resection of lymph vessels and nodes followed by local irradiation of Co was undertaken to propare the rabbit model of lymphedema in a hindlimb. The volume variation was measured after operation. In addition, lymphoscintigraphy, biopsy and anatomic observation were made at postoperative 24 weeks. Results The successful ratio was 100% in animals alive with a mortality rate of 7.8% . The corresponding variation of tissue morphology and lymphoscintigraphy were observed at postoperative 24 weeks. The process of edema was similar to the clinical patients after radical resection of tumor and radiotherapy. The anatomy of the rabbit's hindlimb was relatively similar to that of human beings, with the diameter of big lymph vessels about 0. 6 - 0. 9mm. Conclusion The experimental model preparation is relatively cheap and with a high successful ratio. The rabbit is easy to capture and feed, as well as convenient to observe. It is especially suitable to lymphatic microsurgical research on big lymph vessels.
