摘要
目的 探讨股骨转子间不稳定型骨折的内固定方法。方法 在解剖复位的基础上 ,采用DHS加阻挡螺钉、抗旋转螺钉及固定螺钉等联合内固定治疗股骨转子间不稳定型骨折 64例。结果 随访 14～ 46个月 ,全部骨折愈合 ,患侧股骨颈较正常侧平均短缩 ( 2 .8± 0 .2 )mm ,两者之间的差异无显著性意义 (P >0 .0 5 )。结论 DHS加螺钉联合内固定是治疗股骨转子间不稳定型骨折的有效方法 。
Objective To explore the method of internal fixation on unstable intertrochanteric femoral fractures.Methods On the basis of anatomic reduction,64 consecutive patients with unstable intertrochanteric fractures were internally fixed with dynamic hip screw (DHS) combined with screws such as anti-rotation screw,blocking screw or fixation screw.Results During a follow-up period of 14 to 46 months,bone union was achieved in all cases.Even though the mean length of the femoral neck of sick side was 2.8±0.2 mm shorter than the length of the contralateral side, there was no significant difference in the length between the two sides(P>0.05).Conclusion The combined internal fixation of DHS and the screws are suitable for the treatment of unstable intertrochanteric femoral fractures,and the assisting screw internal fixation is an effective preventive measure of over-collapse for unstable intertrochanteric fractures fixed with DHS.
