摘要 目的:观察不同方法干预高危儿中枢性协调障碍(zentralecoordinationsstorung,ZKS)的效果。方法:选择年龄4~13个月的ZKS患儿101例,随机分为4组,分别为药物治疗组、运动发育治疗组、高压氧治疗组、综合治疗组。药物治疗组:26例,单纯采用脑蛋白水解物注射液;运动发育治疗组:26例,联合使用Bobath法、Vojta法及上田法;高压氧治疗组:27例,单纯使用高压氧;综合治疗组:22例,综合应用药物、高压氧、运动发育疗法及引导式教育。疗程均为3个月。结果:综合应用药物、高压氧、神经发育疗法及引导式教育干预高危儿中枢性协调障碍患儿疗效满意,总有效率达95%,药物治疗组、运动发育治疗组、高压氧治疗组疗效与综合治疗组比较,差异有显著性意义(χ2=4.89,3.91,4.57,P<0.05);各组治疗前发育商比较,无显著性差异,具可比性(P>0.05),治疗后发育商值综合治疗组(61.24±4.35)>运动发育治疗组(57.41±6.42)>药物治疗组(57.32±8.19)>高压氧治疗组(56.33±5.48)。4组治疗前后比较,差异有显著性或非常显著性意义(t=2.434,2.740,P<0.05;t=2.828,7.880,P<0.01)。结论:早期综合干预方法的介入有利于提高高危儿ZKS干预效果。 AIM:To observe the effect of various measures in early intervention treatment of infant with zentrale coordination sstorung (ZKS). METHODS: A total of 101 infants with ZKS, at the age of 4-13 months, were randomized into 4 groups: drug treatment group, kinetogrowth treatment group, hyperbaric oxygen treatment group and combined treatment group. Twenty six infants in drug treatment group were treated with Lizhusailie injection; twenty six infants in kinetogrowth treatment group were treated by Bobath, Vojta and Shangtian processes; twenty seven infants in hyperbaric oxygen treatment received hyperbaric oxygenation and twenty two infants in combined treatment group were treated by medicine, hyperbaric oxygenation, therapeutics of neurophysiology and conductive education. Treatment course of all the above mentioned was three months. RESULTS: Combined application medicine, hyperbaric oxygen, neurodevelopmental therapy and conductive education were effective in the intervention of infants with ZKS, with the total efficiency rate of 95%. Drug treatment group, kinetogrowth treatment group, hyperbaric oxygen treatment group showed significant difference as compared with combined treatment group (χ2 =4.89, 3.91, 4.57, P< 0.05); development quotients in the four groups had no significant difference before treatment(P >0.05), and the quotient value was:61.24±4.35 for combined treatment group > 57.41±6.42 for kinetogrowth treatment group >57.32±8.19 for drug treatment group >56.33±5.48 for hyperbaric oxygen treatment group.Differences in the four group showed significant or great significant meaning before and after treatment(t=2.434,2.740,P< 0.05;t=2.828,7.880,P< 0.01). CONCLUSION: Earlier combined intervention can improve the intervention effect of infants with ZKS.
