摘要
目的:探讨不同剂量、不同给药时间低分子量肝素(LMWH)对兔髂动脉内膜损伤后内膜增生和血管平滑肌细胞增殖的影响。方法:将96只纯种雌性家兔分为空白对照组6只(A),实验对照组18只(B)、小剂量(60u/Kg)LMWH组36只(C)、大剂量(120u/Kg)LMWH组36只(D)。根据不同给药时间,C、D组分别分成三个亚组。三组均经左侧股动脉,近心端插入3FFogarty球囊导管,反复拉动导管制备髂动脉内膜损伤模型。分三批于用药后72h、14d和28d提取髂动脉标本;利用计算机成像系统测量内膜厚度,抗SMCα-ActinABC免疫组织化学方法行5-BrdU单抗标记和Verhoff方法联合测定SMC增殖指数。结果:A组内膜厚度平均41.35±7.06μm;未见5-BrdU标记的阳性细胞。B、C、D组内膜厚度均明显高于A组(P<0.01)。C、D组的内膜厚度和SMC增殖指数高于B组,有显著性差异(P<0.05),并可见5-BrdU标记的阳性细胞。C和D组比较,前者的内膜厚度和SMC增殖指数均小于后者,但两者间无统计学差异,P>0.05。结论:兔髂动膜球囊导管损伤后,内膜明显增生,平滑肌细胞增殖;LMWH可抑制其血管内膜增生和平滑肌细胞增殖,尤其是小剂量、损伤后早期用药。
Objective To investigate the effects of LMWH with two dosages and different time on vascular intimal hyperplasia following balloon injury in rabbit iliac artery. Methods 96 rabbits were randomized into four groups: control group (A , n=6), experimental control group (B, n=18), low-dose LMWH group (C , n=36, 60u/Kg) and high-dose LMWH group (D, n=36,120u/Kg). C and D were divided into 3 subgoups respectively according to the different time of LMWH administration. Balloon injury of the iliac arterial intima was performed by 3F balloon catheter through left femoral artery in all group. The animals were killed at 72h , 14d and 28d after ballon injury respectively and the iliac arterys were sectioned. The vascular intimal thickness was measured with computer imaging analysis system. The hyperplasia index of SMC was measured by 5-BrdU-labeled and Verhoff’s method. Results The intimal thickness in group A was 41.35±7.06 um ; and no positive 5-BrdU-labeled cell was detected. The intimal thickness in group B , C and D were thicker than that in group A ( P<0.01. There were positive 5-BrdU-labeled cells observed. The intimal thickness in group C and D were thicker than that in group B , there being singnificant difference (P<0.05 . There was no singnificant diference between group C and D (P>0.05), although the hyperplasia index and thickness of SMC in goup C are larger and thicker. Conclusion LMWH is able to inhibt vascular intimal and SMC hyperplasia following balloon injury in rabbit iliac artery , especially low-dose and earlier administration of LMWH.
