摘要
目的探讨血清C反应蛋白(CRP)联合心肌肌钙蛋白Ⅰ(cTnI)检验在急性心肌梗死诊断中的应用效果。方法选择2018年4月至2019年4月在我院治疗的AMI患者50例作为研究对象,选择同期健康者50例作为对照组。均进行CRP、cTnI检测,测定两组CRP、cTnI水平,并对所有患者实施冠状动脉造影术,比较血清CRP诊断、cTnI诊断与联合诊断的ROC曲线下面积、灵敏度、特异度及约登指数。结果与对照组相比,观察组入院时、入院12、24、72 h血清CRP、cTnI水平均较高,差异有统计学意义(P<005);联合检测的ROC曲线下面积、灵敏度、特异度及约登指数分别为0.905、0.940、0.840、0.780,CRP单项测量为0.742、0.680、0.500、0.180,cTnI单项测量为0.853、0.840、0.680、0.520。结论血清CRP、cTnI联合检验有助于提升AMI诊断准确度,有效降低漏诊、误诊率,对早期AMI诊断、临床治疗、预后评估具有重要意义。
Objective To investigate the effect of serum C-reactive protein(CRP)combined with cardiac troponinⅠ(cTnI)in the diagnosis of acute myocardial infarction.Methods 50 AMI patients who were treated in our hospital from April 2018 to April 2019 were selected as the study objects,and 50 healthy people in the same period were selected as the control group.CRP and cTnI were measured in both groups.All patients were performed coronary angiography.The area,sensitivity,specificity and yoden index under ROC curve of serum CRP diagnosis,cTnI diagnosis and joint diagnosis were compared.Result Compared with the control group,the serum CRP and cTnI levels of the observation group were higher at admission,12 h,24 h and 72 h,the difference was statistically significant(P<0.05);the area,sensitivity,specificity and yoden index under the ROC curve were 0.905,0.940,0.840 and 0.780 respectively,the CRP single measurement was 0.742,0.680,0.500 and 0.180,and cTnI single measurement was 0.853,0.840,0.680 and 0520.Conclusion The combined test of serum CRP and cTnI is helpful to improve the accuracy of AMI diagnosis,effectively reduce the rate of missed diagnosis and misdiagnosis,and is of great significance to the early diagnosis,clinical treatment and prognosis evaluation of AMI.
