摘要
目的探讨不同剂量舒芬太尼应用于无痛胃镜检查的老年患者体征及并发症的影响。方法分析2016年1月-2019年1月在克拉玛依市人民医院行无痛胃镜检查的420例老年患者的临床资料,根据其使用的舒芬太尼剂量不同分为小剂量组(n=158)、中剂量组(n=170)和大剂量组(n=92)。对比三组检查过程中生命体征的变化、麻醉用量、镜检相关时间、并发症发生率和安全性。结果中剂量组、DBP、RR、HR、SPO2优于小、大剂量组(P<0.05);中剂量组舒芬太尼用量小于大剂量组,且丙泊酚用量依次小于大、小剂量组(P<0.05);三组检查后舒芬太尼、丙泊酚血清浓度无统计学差异(P>0.05);中剂量组麻醉起效时间、胃检时间、苏醒时间、恢复时间、并发症发生率均优于小、大剂量组(P<0.05);三组安全性观察项目无统计学差异(P>0.05)。结论将剂量为0.15 ug/kg的舒芬太尼应用于胃镜检查中为最适剂量,可在保证安全性的基础上提高麻醉效果,且减少并发症发生。
Objective To investigate the effects of different doses of sufentanil on signs and complications of elderly patients undergoing painless gastroscopy.Methods The clinical data of 420 elderly patients underwent painless gastroscopy in our hospital from January 2016 to January 2019 were retrospectively analyzed.Patients were divided into the low-dose group(n=158),the medium-dose group(n=170),and the high-dose group(n=92)according to the different dosage of sufentanil.Then the changes of vital signs,anesthesia dosage,microscopic examination time,incidence of complications and safety were compared among the three groups.Results The DBP,RR,HR and SPO2 in the medium-dose group were better than that of the low-dose and the high-dose groups(P<0.05).The dosage of sufentanil in the medium-dose group was less than that of the high-dose group(P<0.05).The dosage of propofol ranking in a descending order was the high-dose group,the low-dose group and the medium-dose group,with statistic difference(P<0.05).Serum concentration of sufentanil and propofol among three groups after rescue showed no significant difference(P>0.05).The onset time of anesthesia,gastric examination time,anesthesia recovery time,recovery time and the incidence of complications in the medium-dose group were better than that of the low-dose and the high-dose groups(P<0.05).Safety related items among the three groups showed no significant difference(P>0.05).Conclusion Sufentanil with dosage of 0.15 ug/kg is the optimum dosage for gastroscopy,which can effectively and safely improve anesthesia effect and reduce complications.
