摘要
目的:观察筋伤膏外敷对胫腓骨闭合性骨折围手术期患肢肿胀、疼痛的影响,探索诊疗方案。方法:将符合标准的患者随机分为治疗组和对照组各45例。对照组以常规方案及七叶皂苷钠或甘露醇治疗,治疗组在此基础上给予筋伤膏外敷,观察两组患者下肢组织肿胀程度、疼痛程度(VAS评分)、术前等待时间等主要指标。结果:(1)首次出现皮肤皱褶时间:治疗组(4.54±0.78)d,对照组(5.75±1.05)d,差异有统计学意义(P<0.01);(2)肿胀程度:治疗组治疗后24h、48h、72h,肿胀度改善明显优于对照组(P<0.01);(3)疼痛评分:治疗后两组第1、2、3天,治疗组均优于对照组(P<0.01);(4)治疗总有效率:治疗组综合疗效更为显著。结论:筋伤膏在改善胫腓骨闭合性骨折围手术期肿胀、疼痛等方面疗效确切,对骨折早期患肢肿胀、疼痛作用显著。
Objective:To observe the effect of external application of Jinshang Gao on swelling and pain of affected limb in perioperative period of closed fracture of tibia and fibula,and to explore the diagnosis and treatment plan.Methods:The patients were randomly divided into treatment group and control group,45 cases in each group.The control group was treated with conventional regimen and sodium aescinate or mannitol,while the treatment group was treated with Jinshang Gao on the basis of conventional regimen.The swelling degree of lower limb tissue,pain degree(VAS score),preoperative waiting time and other main indicators of the two groups were observed.Results:(1)The first time of skin wrinkle:the treatment group(4.54±0.78)D and(5.75±1.05)D,The difference was statistically significant(P<0.01);(2)Swelling degree:the improvement of swelling degree in the treatment group was significantly better than that in the control group 24 h,48 hand 72 hafter treatment(P<0.01);(3)Pain score:on the 1 st,2 nd and 3 rd day after treatment,the treatment group was better than the control group(P<0.01);(4)Total effective rate:the comprehensive effect of the treatment group was more significant.Conclusion:Jinshang Gao has definite effect on improving swelling and pain of closed fracture of tibia and fibula in perioperative period,and has significant effect on swelling and pain of affected limb in early stage of fracture.
