摘要
目的探讨高血压合并心力衰竭患者应用酒石酸美托洛尔片与盐酸贝那普利片联合治疗的临床疗效。方法选取内蒙古自治区人民医院2020年1月至2023年12月诊治的100例高血压合并心力衰竭患者作为研究对象,按随机数字表法分为观察组及对照组,各50例。对照组给予盐酸贝那普利片治疗,观察组给予酒石酸美托洛尔片联合盐酸贝那普利片治疗,两组均治疗4周。比较两组的疗效、治疗前与治疗4周后的心率和血压、心功能、6 min步行距离(6-minute walking distance,6MWT)、心肌肌钙蛋白Ⅰ(cardiac troponin,cTnⅠ)、脑钠肽(brain natriuretic peptide,BNP)水平及生活质量变化。结果观察组的治疗总有效率高于对照组(P<0.05);治疗4周后,观察组的心率、舒张压和收缩压水平均低于对照组(P<0.05);治疗4周后,观察组的左心室射血分数(left ventricular ejection fraction,LVEF)高于对照组,而左心室收缩末期内径(left ventricular end systolic diameter,LVESD)和左心室舒张末期内径(left ventricular end diastolic dimension,LVEDD)均低于对照组(P<0.05);治疗4周后,观察组的6MWT,cTnⅠ,BNP水平均低于对照组(P<0.05);治疗4周后,观察组的明尼苏达州心功能不全生命质量量表(Minnesota living with heart failure questionnaire,MLHFQ)评分低于对照组(P<0.05)。结论对高血压合并心力衰竭患者应用酒石酸美托洛尔片与盐酸贝那普利片联合治疗的疗效确切,可改善患者的心功能,使血清BNP水平得到降低。
Objective To investigate the clinical efficacy of metoprolol tartrate tablets combined with Benazepril hydrochloride tablets in the treatment of hypertension complicated with heart failure.Methods A total of 100 hypertensive patients with heart failure diagnosed and treated in the Inner Mongolia People's Hospital from January 2020 to December 2023 were selected as the study subjects and divided into observation group and control group with 50 cases each.The control group was treated with benazepril,while the observation group was treated with metoprolol combined with benazepril.The treatment course was 4 weeks.The therapeutic effects of the two groups were compared;the heart rate and blood pressure,cardiac function,6-minute walking distance(6MWT),cardiac troponin(cTnⅠ),brain natriuretic peptide(BNP)levels,and quality of life changes before and after treatment were compared.Results The total response rate in the observation group was higher than the control group(P<0.05);After 4 weeks of treatment,heart rate,diastolic blood pressure and systolic blood pressure in the observation group were lower than the control group(P<0.05);After 4 weeks of treatment,the left ventricular ejection fraction(LVEF)was higher than that in the control group,while the left ventricular end systolic diameter(LVESD)and left ventricular end diastolic dimension(LVEDD)was lower than that in the control group(P<0.05);After 4 weeks of treatment,The 6 MWT of the observation group,and cTnI,BNP was lower than that of the control group(P<0.05);After 4 weeks of treatment,the Minnesota living with heart failure questionnaire(MLHFQ)score was lower than that in the control group(P<0.05).Conclusion The combination of metoprolol tablets and Benazepril tablets in the treatment of hypertensive patients with heart failure is effective,and the cardiac function of patients is significantly improved,and the serum BNP level is significantly reduced.
