以家庭为中心提升老年慢病患者照护质量实践研究 被引量:11

目的:以哮喘为例,探讨以家庭为中心提升老年慢病患者依从性和生活质量的实践和效果。方法:选取某三甲医院2020年1月就诊老年哮喘患者152例,随机分为两组,对照组开展常规护理管理,研究组开展以家庭为中心的管理模式。比较两组严格遵医嘱... 目的:以哮喘为例,探讨以家庭为中心提升老年慢病患者依从性和生活质量的实践和效果。方法:选取某三甲医院2020年1月就诊老年哮喘患者152例,随机分为两组,对照组开展常规护理管理,研究组开展以家庭为中心的管理模式。比较两组严格遵医嘱治疗率、规范复诊率、非计划再入院率等指标,分析两组患者依从性、生活质量变化情况。结果:研究组严格遵医嘱治疗率、规范复诊率、正确使用药物率均高于对照组(P<0.05),非计划再入院率低于对照组(P<0.05);研究组疾病防治效果优于对照组(P<0.05),患者出院后2个月、6个月、12个月依从性均高于对照组(P<0.05);两组护理后生活质量评分均高于护理前,焦虑评分均低于治疗前,研究组护理后生活质量评分高于对照组(P<0.05),焦虑评分低于对照组(P<0.05)。结论:以家庭为中心的管理模式下,老年哮喘患者依从性较高,疾病防治取得较好效果,患者生活质量明显提高。在人口老龄化日益突出的当下,该模式能有效提升老年照护的可及性和质量,有一定借鉴价值。 展开更多

运用文献资料法、专家访谈法、逻辑分析法等研究初中独生子女群体锻炼动机的影响因素,以期为中学体育课程改革和独生子女体质健康提供理论参考与启示。研究发现:初中独生子女与非独生子女在父母教养方式、身体自我效能感和锻炼动机上存... 运用文献资料法、专家访谈法、逻辑分析法等研究初中独生子女群体锻炼动机的影响因素,以期为中学体育课程改革和独生子女体质健康提供理论参考与启示。研究发现:初中独生子女与非独生子女在父母教养方式、身体自我效能感和锻炼动机上存在显著差异。初中独生子女父母情感温暖、父母拒绝和父母过度保护显著高于非独生子女群体;身体自我效能感显著低于非独生子女群体;锻炼动机中的能力动机和乐趣动机显著低于非独生子女群体。外貌动机和社交动机显著高于非独生子女群体。父母教养方式、身体自我效能感、父母支持和锻炼动机呈显著正相关。父母教养方式是初中独生子女锻炼动机的前端要素;身体自我效能感是初中独生子女锻炼动机的内在动力;父母支持是初中独生子女锻炼动机的外部动力。建议:家校联袂,合力共育;独生子女父母采用积极的教养方式,注重提高初中独生子女内部动机水平。优化中学体育课程,体育教学中注重提高独生子女的身体自我效能水平。开展亲子活动,父母支持体育消费,逐步提高独生子女运动的自主性。 展开更多

The Code for Seismic Design of Buildings(GB50011-2010)in 2016 and the method of seismic performance-based design for high-rise buildings in the Guide for Performance-based Design of High-Rise Buildings(TBI2017)are compared.In view of the characteristics and limitations of the seismic performance index set by the Sino-US seismic code,a“three-index”performance index system and evaluation process considering the displacement angle of the structural interlayer,the plastic damage degree of components and the plastic strain of material is put forward;combining the example of time-history analysis of a out-of-code high-rise building under the rare earthquakes is verified.The results show that the method of seismic performance evaluation by using deformation control index in Sino-US seismic code is relatively simple;however,both are lacking in the setting of specific components and the whole structure level respectively.The"three-index"system can comprehensively and quantitatively evaluate the seismic performance of out-of-code high-rise buildings. 展开更多

Anti-obesity effects of Lactiplantibacillus plantarum BHP03 on high-fat diet mice and its regulatory function on intestinal microbiota

Obesity is defined as abnormal or excessive accumulation of body fat,which always causes serious harm to body health.In this work,an anti-obesity strain,Lactiplantibacillus plantarum BHP03(L.plantarum),was explored.This strain showed good reproductive and acid production abilities.And its simulated intestinal fluid tolerance was better than that of Lacticaseibacillus rhamnosus GG(LGG).The in vitro lipid-lowering indicators(inhibition rate of pancreatic lipase,bile-acid salt binding capacity)of L.plantarum BHP03 were at the same level as LGG,but L.plantarum BHP03 exhibited better ability to clear cholesterol.Further,L.plantarum BHP03 was used to intervene high-fat diet mice by oral administration.The results showed that high(10^(9) CFU/day)and low(10^(8) CFU/day)doses of L.plantarum BHP03 could alleviate excessive weight gain caused by high-fat diet and improve the symptoms of hepatorenal enlargement and liver fatty lesions.In addition,high dose L.plantarum BHP03 significantly reduced the content of the serum total cholesterol and low-density lipoprotein cholesterol in mice.Moreover,high dose L.plantarum BHP03 enhanced the species richness of intestinal flora in mice fed with high-fat diet,decreasing the abundance of harmful bacteria such as Staphylococcus and increasing the abundance of beneficial bacteria such as Dubosiella,Akkermansia,Bifidobacterium and Lactobacillus.Therefore,it could be concluded that high dose L.plantarum BHP03 was more effective in preventing obesity and balancing gut microbiota.These findings may provide a theoretical basis for the application of lactobacilli to develop func-tional foods related to anti-obesity. 展开更多
