AIM To increase evidence-based pain prevention strategy use during routine vaccinations in a pediatric primary care clinic using quality improvement methodology.METHODS Specific intervention strategies(i.e.,comfort po...AIM To increase evidence-based pain prevention strategy use during routine vaccinations in a pediatric primary care clinic using quality improvement methodology.METHODS Specific intervention strategies(i.e.,comfort positioning,nonnutritive sucking and sucrose analgesia,distraction) were identified,selected and introduced in three waves,using a Plan-Do-Study-Act framework.System-wide change was measured from baseline to post-intervention by:(1) percent of vaccination visits during which an evidence-based pain prevention strategy was reported as being used; and(2) caregiver satisfaction ratings following the visit.Additionally,self-reported staff and caregiver attitudes and beliefs about pain prevention were measured at baseline and 1-year post-intervention to assess for possible long-term cultural shifts.RESULTS Significant improvements were noted post-intervention.Use of at least one pain prevention strategy was documented at 99% of patient visits and 94% of caregivers were satisfied or very satisfied with the pain prevention care received.Parents/caregivers reported greater satisfaction with the specific pain prevention strategy used [t(143) = 2.50,P ≤ 0.05],as well as greater agreement that the pain prevention strategies used helped their children's pain [t(180) = 2.17,P ≤ 0.05] and that they would be willing to use the same strategy again in the future [t(179) = 3.26,P ≤ 0.001] as compared to baseline.Staff and caregivers also demonstrated a shift in attitudes from baseline to 1-year post-intervention.Specifically,staff reported greater agreement that the pain felt from vaccinations can result in harmful effects [2.47 vs 3.10; t(70) =-2.11,P ≤ 0.05],less agreement that pain from vaccinations is "just part of the process" [3.94 vs 3.23; t(70) = 2.61,P ≤ 0.05],and less agreement that parents expect their children to experience pain during vaccinations [4.81 vs 4.38; t(69) = 2.24,P ≤ 0.05].Parents/caregivers reported more favorable attitudes about pain prevention strategies for vaccinations across a variety of areas,including safety,cost,time,and effectiveness,as well as less concern about the pain their children experience with vaccination [4.08 vs 3.26; t(557) = 6.38,P ≤ 0.001],less need for additional pain prevention strategies [3.33 vs 2.81; t(476) = 4.51,P ≤ 0.001],and greater agreement that their doctors' office currently offers pain prevention for vaccinations [3.40 vs 3.75; t(433) =-2.39,P ≤ 0.05].CONCLUSION Quality improvement methodology can be used to help close the gap in implementing pain prevention strategies during routine vaccination procedures for children.展开更多

Mycotoxins-tainted food and fodder can be found all over the world, posing a global concern. Consumption of contaminated food and fodder is a typical cause of epidemic outbreaks. Other mycotoxins have been associated with esophageal cancer and neural tube defects (NTDs);the immunotoxin deoxynivalenol (DON) causes diarrhea when combined with trichothecenes, and ochratoxin A (OTA) has been linked to kidney failure. The direct market costs associated with missed trade or lower revenues owing to tainted food or feed could be viewed as the economic repercussions of mycotoxins on human society. It is mostly transmitted throu...Zika virus disease is a disease of public health importance and was declared a “Public Health Emergency of International Concern”, by the World Health Organization on February 1, 2016. It is mostly transmitted through the bite of Aedes aegypti and Aedes albopictus mosquitos. Transmission can also occur through blood transfusion and sexual intercourse. It could cause microcephaly and other neurological problems in newborns and adults, such as Guillain-Barre syndrome. It could also lead to abortion and stillbirth in pregnant women. Prevention methods are essentially targeted at transmission routes, which are mosquito bite, sexual intercourse, and blood transfusion. The study was of observational, descriptive, cross-sectional design, and conducted in a tertiary health institution in the Southeast geopolitical zone of Nigeria. Two hundred and fifty-six female caregivers participated in the study, with most of them being within the childbearing age group, and educated up to tertiary level. The objective was to assess the knowledge of Zika virus infection prevention methods, among female caregivers that bring babies for immunization in a Teaching Hospital, Southeast Nigeria. Out of the eight questions that sought to assess the knowledge level of the respondents, they scored 50% and above only in three, which were on prevention through protection against mosquito bite (60.5%), avoiding unnecessary blood transfusion in endemic areas (53.9%), and avoidance of oily meals not being a preventive measure (51.2%). Respondents in the childbearing age group, and those with higher educational qualifications, generally exhibited higher knowledge level than the other respondents. Scores on questions that sought to assess their knowledge on prevention aspects related to sexual behavior, and vaccination were poor. Sensitization, or awareness creation activities for persons in this study area, ought to be designed to address the observed gaps.展开更多

Test results and comprehensive evaluation analysis demonstrated that 72% Bt·monosultap WP and 8%lambda-cyhalothrin·emamectin benzoate SA by manual spraying and unmanned aerial vehicle spraying had good prevention and control effect on strains and internodes damaged by Tryporyza intacta Snellen in middle and late stage, which were ideal new biological formulations with high effi-ciency and low risk for prevention and control of T. intacta in middle and late stage, and could be popularized in sugarcane area. 72% Bt·monosultap WP at the dose of 3 000 g/hectare and 8% lambda-cyhalothrin·emamectin benzoate SA at the dose of 750 mL/hectare could be sprayed in mid September at the peak occurrence period of the forth and fifth generations of T. intacta and Proceras venosatus. Agents were diluted with 900 kg water per hectare, and manually sprayed with electric knapsack sprayer; or agents were diluted with 15 kg of Haoyang aerial control special addi-tives and water per hectare, and sprayed with unmanned aerial vehicle. 文摘AIM To increase evidence-based pain prevention strategy use during routine vaccinations in a pediatric primary care clinic using quality improvement methodology.METHODS Specific intervention strategies(i.e.,comfort positioning,nonnutritive sucking and sucrose analgesia,distraction) were identified,selected and introduced in three waves,using a Plan-Do-Study-Act framework.System-wide change was measured from baseline to post-intervention by:(1) percent of vaccination visits during which an evidence-based pain prevention strategy was reported as being used; and(2) caregiver satisfaction ratings following the visit.Additionally,self-reported staff and caregiver attitudes and beliefs about pain prevention were measured at baseline and 1-year post-intervention to assess for possible long-term cultural shifts.RESULTS Significant improvements were noted post-intervention.Use of at least one pain prevention strategy was documented at 99% of patient visits and 94% of caregivers were satisfied or very satisfied with the pain prevention care received.Parents/caregivers reported greater satisfaction with the specific pain prevention strategy used [t(143) = 2.50,P ≤ 0.05],as well as greater agreement that the pain prevention strategies used helped their children's pain [t(180) = 2.17,P ≤ 0.05] and that they would be willing to use the same strategy again in the future [t(179) = 3.26,P ≤ 0.001] as compared to baseline.Staff and caregivers also demonstrated a shift in attitudes from baseline to 1-year post-intervention.Specifically,staff reported greater agreement that the pain felt from vaccinations can result in harmful effects [2.47 vs 3.10; t(70) =-2.11,P ≤ 0.05],less agreement that pain from vaccinations is "just part of the process" [3.94 vs 3.23; t(70) = 2.61,P ≤ 0.05],and less agreement that parents expect their children to experience pain during vaccinations [4.81 vs 4.38; t(69) = 2.24,P ≤ 0.05].Parents/caregivers reported more favorable attitudes about pain prevention strategies for vaccinations across a variety of areas,including safety,cost,time,and effectiveness,as well as less concern about the pain their children experience with vaccination [4.08 vs 3.26; t(557) = 6.38,P ≤ 0.001],less need for additional pain prevention strategies [3.33 vs 2.81; t(476) = 4.51,P ≤ 0.001],and greater agreement that their doctors' office currently offers pain prevention for vaccinations [3.40 vs 3.75; t(433) =-2.39,P ≤ 0.05].CONCLUSION Quality improvement methodology can be used to help close the gap in implementing pain prevention strategies during routine vaccination procedures for children.
文摘Fungi that attack field crops and contaminate agricultural commodities produce mycotoxins when conditions are favorable. These are specialized metabolites made of poisonous chemical compounds created by bacteria, fungi, or plants that aren’t involved in the organism’s usual growth, development, or reproduction. They have an adversarial hold on humans, wildlife, and the agricultural sector, resulting in mysterious ailments and economic disruptions. Mycotoxins-tainted food and fodder can be found all over the world, posing a global concern. Consumption of contaminated food and fodder is a typical cause of epidemic outbreaks. Other mycotoxins have been associated with esophageal cancer and neural tube defects (NTDs);the immunotoxin deoxynivalenol (DON) causes diarrhea when combined with trichothecenes, and ochratoxin A (OTA) has been linked to kidney failure. The direct market costs associated with missed trade or lower revenues owing to tainted food or feed could be viewed as the economic repercussions of mycotoxins on human society. This review describes frequent groups of mycotoxins in detail, their impact on global health, their impact on the socio-economy;the methods of detection and prevention of these mycotoxins.
文摘Zika virus disease is a disease of public health importance and was declared a “Public Health Emergency of International Concern”, by the World Health Organization on February 1, 2016. It is mostly transmitted through the bite of Aedes aegypti and Aedes albopictus mosquitos. Transmission can also occur through blood transfusion and sexual intercourse. It could cause microcephaly and other neurological problems in newborns and adults, such as Guillain-Barre syndrome. It could also lead to abortion and stillbirth in pregnant women. Prevention methods are essentially targeted at transmission routes, which are mosquito bite, sexual intercourse, and blood transfusion. The study was of observational, descriptive, cross-sectional design, and conducted in a tertiary health institution in the Southeast geopolitical zone of Nigeria. Two hundred and fifty-six female caregivers participated in the study, with most of them being within the childbearing age group, and educated up to tertiary level. The objective was to assess the knowledge of Zika virus infection prevention methods, among female caregivers that bring babies for immunization in a Teaching Hospital, Southeast Nigeria. Out of the eight questions that sought to assess the knowledge level of the respondents, they scored 50% and above only in three, which were on prevention through protection against mosquito bite (60.5%), avoiding unnecessary blood transfusion in endemic areas (53.9%), and avoidance of oily meals not being a preventive measure (51.2%). Respondents in the childbearing age group, and those with higher educational qualifications, generally exhibited higher knowledge level than the other respondents. Scores on questions that sought to assess their knowledge on prevention aspects related to sexual behavior, and vaccination were poor. Sensitization, or awareness creation activities for persons in this study area, ought to be designed to address the observed gaps.
文摘To analyze the type of Algerian’s gallstones and put forward relative measures of prevention and treatment, The NICOLET 170SX FT-IR spectrometer was used to measure the infra-red spectrum and analyze the feature of spectrum of Algerian’s gallstones. The results shown that Algerian have cholesterol gallstones. Reasons to form Algerian’s multiple cholesterol gallstones were investigated. We have put forward five suggestions to prevent and treat cholelithiasis.

文摘According to different planting test data,meteorological data,field planting survey and expert interview data in recent years,frost prevention and drought resistance effect of potato under different planting methods were studied using agro-meteorological disaster risk index analysis method. The results showed that the risk index of potato with the full-film planting was small due to good thermal insulation and soil moisture conservation performance. The risk index of potato with half-film planting( flat mulching,ridging planting and furrow planting) was moderate,while that of ridge tillage without mulching,double rows,full of stars and flat planting was large,and their frost prevention and drought resistance effect were poor. It provides support and reference for understanding the frost and drought conditions of potatoes under different planting methods and carrying out insurance weather services for farmers and production managers.
文摘Background:To analyze the medication and methods of ancient traditional Chinese medicine for the prevention of plague and seasonal pathogen in order to reduce the spread of infectious diseases such as flu and protect the susceptible people.Methods:Search the medication and the methods for preventing plague and seasonal pathogen in the Chinese Medical Code,classify and summarize the methods,perform frequency analysis and efficacy cluster analysis on Chinese medicines of involved medicines.Results:The methods of preventing ancient plague were as follows:burning and fumigating Chinese medicines,medicating nostrils,wearing bags filled with Chinese medicines,oral Chinese medicines and applying medicines to skin.The 3 main ways for drugs to work were external medication,nostril inhalation,and internal absorption.The Chinese medicines for preventing plague are mostly warm in nature and pungent in flavour,belonging to the spleen,stomach,liver,lung meridian.The 3 categories of drugs obtained by cluster analysis are C1:drugs for facilitating lung,relieving exterior and dissipating cold,such as Cangzhu(Atractylodis rhizoma)-Qianghuo(Notopterygh rhizoma et radix)-Xixin(Asari radix et rhizoma)-Baizhi(Angelicae dahuricae radix)-Fangfeng(Saposhnikoviae radix)-Zaojiao(Gleditsiae sinensis fructus);C2:drugs for promoting blood circulation and removing blood stasis,such as Xionghuang(Realgar)-Zhusha(Cinnabaris)-Dahuang(Rhei radix rhizoma)-Chuanxiong(Chuanxiong rhizoma)-Jiegeng(Platycodonis radix);C3:drugs for replenishing Qi(it is the most basic element that constitutes human body and maintains life activities in traditional Chinese medicine)and tonifying the spleen,such as Gancao(Glycyrrhizae radix et rhizoma)-Dazao(Jujubae fructus)-Ganjiang(Zingiberis rhizoma)-Rougui(Cinnamomi cortex)-Gansong(Nardostachyos radix et rhizoma)-Chenpi(Citri reticulatae pericarpium)-Baizhu(Atractylodis macrocephalae rhizoma).Conclusion:Through analysis the medication and methods of ancient traditional Chinese medicine for the prevention of plague,we can provide reference for modern research from the original plague prevention methods,and make an important contribution to the use of traditional Chinese medicine in the prevention and treatment of epidemic infectious diseases in modern clinic.

文摘Wireless Sensor Networks(WSNs)gained wide attention in the past decade,thanks to its attractive features likeflexibility,monitoring capability,and scalability.It overcomes the crucial problems experienced in network man-agement and facilitates the development of diverse network architectures.The existence of dynamic and adaptive routing features facilitate the quick formation of such networks.Butflexible architecture also makes it highly vulnerable to dif-ferent sorts of attacks,for instance,Denial of Service(DoS).Grey Hole Attack(GHA)is the most crucial attack types since it creates a heavy impact upon the components of WSN and eventually degrades the performance of network.In cur-rent study,a simple attack detection,prevention and reduction approach is pro-posed.This is to secure the WSN from GHA and other such attacks by warning and blocking the malicious suspensions and by examining the storage table.Instead of blocking the entire host,the presented approach specifically elim-inates the malicious nodes.Further,in case of no malicious traffic detection,the host gets unblocked.In current study,the researchers simulated the model under MATLAB environment and the outcomes showed an enhanced performance and increased utilization of Central Processing Unit(CPU)and packet delivery ratio.
文摘With the increasing incidence of salivary gland tumors, partial parotidectomy has become more common, leading to a rise in the occurrence of parotid fistula post-surgery. Parotid fistula can cause redness and erosion of the surrounding skin, resulting in delayed wound healing, and it is a common postoperative complication that troubles both patients and healthcare providers. Various methods for the prevention and treatment of parotid fistula exist, each with its own advantages and disadvantages, typically requiring different approaches based on the type of fistula. The overall strategies for prevention and treatment can be divided into two categories: pharmacological and non-pharmacological treatments. Pharmacological options include anticholinergics, botulinum toxin injections, and hypertonic saline, while non-pharmacological options include low-dose radiotherapy, bandage compression, and biomembrane therapy. These methods can effectively prevent and treat parotid fistula, thereby reducing the concerns of both patients and healthcare providers.

文摘Modal parameters can accurately characterize the structural dynamic properties and assess the physical state of the structure.Therefore,it is particularly significant to identify the structural modal parameters according to the monitoring data information in the structural health monitoring(SHM)system,so as to provide a scientific basis for structural damage identification and dynamic model modification.In view of this,this paper reviews methods for identifying structural modal parameters under environmental excitation and briefly describes how to identify structural damages based on the derived modal parameters.The paper primarily introduces data-driven modal parameter recognition methods(e.g.,time-domain,frequency-domain,and time-frequency-domain methods,etc.),briefly describes damage identification methods based on the variations of modal parameters(e.g.,natural frequency,modal shapes,and curvature modal shapes,etc.)and modal validation methods(e.g.,Stability Diagram and Modal Assurance Criterion,etc.).The current status of the application of artificial intelligence(AI)methods in the direction of modal parameter recognition and damage identification is further discussed.Based on the pre-vious analysis,the main development trends of structural modal parameter recognition and damage identification methods are given to provide scientific references for the optimized design and functional upgrading of SHM systems.

文摘In 1999,China released its first edition of the“Guidelines for the Prevention and Treatment of Hypertension”,which was subsequently updated in 2005,2010,and 2018,establishing a diagnostic,assessment,grading,stratification,prevention,intervention,and management framework for hypertension tailored to the unique characteristics of the Chinese population.Over the past two decades,these guidelines have been widely implemented in clinical practice across China and have played a significant role in the management of hypertension and chronic diseases,the development of grassroots guidelines,the formulation of clinical pathways,and the implementation of health insurance policies.

文摘COVID-19 has been discovered since December 2019 and has become a global pandemic recently,killing hundreds of thousands of people in 216 countries and regions around the world.COVID-19 has caused more than 4000 deaths in China,seriously affecting national health and national economy.Since the outbreak of COVID-19,National Health Commission of the People's Republic of China has used the method of sino-western medicine to prevent and control COVID-19,effectively curbed the spread of the epidemic.But the TCM methods in the scheme are all oral prescriptions,less involving non-drug methods.Therefore,I searched the COVID-19 control methods published in various parts of the country,this paper plans to discuss how to improve immunity from the perspective of non-drug methods such as moxibustion,diet therapy,self massage,physical exercise skills and traditional music,hoping can helps the susceptible populations,infected,discharged and medical staff in COVID-19 epidemic.

文摘To screen new biological formulations, accurate and efficient application technology, field test was conducted with 72% Bacillus thuringiensis(Bt)·monosultap WP, 8% lambda-cyhalothrin·emamectin benzoate SA and 3.6% lambda-cyhalothrin·Bt SA through manual spraying and unmanned aerial vehicle spraying. Test results and comprehensive evaluation analysis demonstrated that 72% Bt·monosultap WP and 8%lambda-cyhalothrin·emamectin benzoate SA by manual spraying and unmanned aerial vehicle spraying had good prevention and control effect on strains and internodes damaged by Tryporyza intacta Snellen in middle and late stage, which were ideal new biological formulations with high effi-ciency and low risk for prevention and control of T. intacta in middle and late stage, and could be popularized in sugarcane area. 72% Bt·monosultap WP at the dose of 3 000 g/hectare and 8% lambda-cyhalothrin·emamectin benzoate SA at the dose of 750 mL/hectare could be sprayed in mid September at the peak occurrence period of the forth and fifth generations of T. intacta and Proceras venosatus. Agents were diluted with 900 kg water per hectare, and manually sprayed with electric knapsack sprayer; or agents were diluted with 15 kg of Haoyang aerial control special addi-tives and water per hectare, and sprayed with unmanned aerial vehicle. The control effect against borer-damaged strain rate was above 81.3% and that against borer-damaged internode rate was above 88.6%.

文摘Objective Alzheimer’s disease(AD)presents a significant global health challenge with a steadily increasing prevalence and impact.This comprehensive review aimed to delve into the epidemiology,pathophysiology,pharmacological therapies,emerging research,challenges,and future directions of AD.Major findings from recent studies were synthesised,encompassing insights into the global prevalence,molecular pathology,high-risk factors,and therapeutic interventions,including cholinesterase inhibitors,glutamate receptor antagonists,and antibodies against Aβand tau proteins.Additionally,emerging research areas such as gene therapy and lipid nanoparticles were highlighted.This review emphasised the urgent need for ongoing research on novel therapeutic avenues and addressing the challenges in AD diagnosis,treatment,and care.Future perspectives underscore the potential of disease-modifying treatments,personalised medical approaches,and holistic interventions to alleviate the growing burden of AD on individuals,families,and healthcare systems worldwide.By fostering collaboration and innovation,we can strive towards a future where effective prevention,early detection,and personalised treatments enhance the lives of millions affected by AD globally.
文摘The problem of gastric cancer(GC)prevention remains relevant for a long time.Various methods of population serological screening of atrophic gastritis and precancerous changes in the gastric mucosa have been created at present.Modern endoscopic and morphological methods of verification of the diagnosis of precancerous diseases and changes in the gastric mucosa have been introduced into the practice of gastroenterologists and oncologists.GC risk stratification systems allow the formation of risk groups that require population screening.Practical hints for population serological screening of atrophic gastritis,endoscopic and morphological verification of precancerous changes and diseases of the stomach recommend using it:When developing state programs for the prevention of stomach cancer;when implementing preventive measures for stomach cancer by doctors of all specialties;the authors also offer the possibility of use by anyone over the age of 40,provided that they seek methodological help from their doctor;in the work of health schools in any medical and preventive institutions.The use of an assessment system of certain risk factor signatures with prognostic value would add significant assistance to preventive measures against GC.

文摘The separation-of-variable(SOV)methods,such as the improved SOV method,the variational SOV method,and the extended SOV method,have been proposed by the present authors and coworkers to obtain the closed-form analytical solutions for free vibration and eigenbuckling of rectangular plates and circular cylindrical shells.By taking the free vibration of rectangular thin plates as an example,this work presents the theoretical framework of the SOV methods in an instructive way,and the bisection–based solution procedures for a group of nonlinear eigenvalue equations.Besides,the explicit equations of nodal lines of the SOV methods are presented,and the relations of nodal line patterns and frequency orders are investigated.It is concluded that the highly accurate SOV methods have the same accuracy for all frequencies,the mode shapes about repeated frequencies can also be precisely captured,and the SOV methods do not have the problem of missing roots as well.

文摘Soil improvement is one of the most important issues in geotechnical engineering practice.The wide application of traditional improvement techniques(cement/chemical materials)are limited due to damage ecological en-vironment and intensify carbon emissions.However,the use of microbially induced calcium carbonate pre-cipitation(MICP)to obtain bio-cement is a novel technique with the potential to induce soil stability,providing a low-carbon,environment-friendly,and sustainable integrated solution for some geotechnical engineering pro-blems in the environment.This paper presents a comprehensive review of the latest progress in soil improvement based on the MICP strategy.It systematically summarizes and overviews the mineralization mechanism,influ-encing factors,improved methods,engineering characteristics,and current field application status of the MICP.Additionally,it also explores the limitations and correspondingly proposes prospective applications via the MICP approach for soil improvement.This review indicates that the utilization of different environmental calcium-based wastes in MICP and combination of materials and MICP are conducive to meeting engineering and market demand.Furthermore,we recommend and encourage global collaborative study and practice with a view to commercializing MICP technique in the future.The current review purports to provide insights for engineers and interdisciplinary researchers,and guidance for future engineering applications.
