Effectiveness of one-to-one feedback methods of teaching with proper metered-dose inhaler technique and correcting errors in chronic obstructive lung disease patients

Objective:To determine the most common mistakes made during the use of a metered-dose inhaler(MDI),and the effect of the repeated trainings performed with demonstration method by providing one-to-one feedback on these mistakes.Methods:This is a quasi-experimental study with a control group.A total of 100 chronic obstructive pulmonary disease(COPD)patients(50 in the control group and 50 in the experimental group)were included in the research.“Patient Information Form(PIF)”(to determine the descriptive characteristics of patients)through the face-to-face interview method,using“MDI Skill Assessment Form”(MDISAF)(it is composed of 10 skill steps about the use of MDI)through observation method was used.Patients in both the groups were asked to use MDI and their abilities regarding use of MDI were assessed.Then in the intervention group,usage of MDI was explained by a nurse via demonstration and placebo MDI.Trainings were repeated on days 1,3,and 5 as from hospitalization of the patient.In the intervention group,three methods were used in this study:“face-to-face training,”“one-to-one,”and“with feedbacks and repeated.”Routine training regarding use of MDI was given by the nurses in the clinic to patients in the control group.The use of an MDI was assessed using MDISAF before training and after the training on the first,third and fifth days of hospitalization.On the seventh day,the last measurement was performed.Percentage,chi square,and mean were used to assess the data.Results:After repetitive training with one-to-one feedback,several differences between the groups in favor of the experimental group were found in 7 of the 10 skill levels of the MDI.There was a significant difference after“training”between the groups in the third,fourth,fifth,sixth,seventh,eighth and ninth MDI steps in posttest measurement(P<0.05).Evaluating the skills of the groups to use MDI from pretest to posttest,it was determined that while the intervention group made less mistakes in steps in which mistakes were made mostly,the control group continued to make mistakes.Conclusions:Inhaler technique intervention with repeated,face to face,and one-to-one feedback trainings can significantly enhance the MDI techniques in COPD patients.The patients in the intervention group made less mistakes during MDI application and their application skills improved.It may be asser ted that the training provided to the intervention group was effective for using the device correctly,while the training provided in the clinic for the control group was inadequate. 展开更多

This paper mainly studies the problem of using UAVs to provide accurate remote target indication for hypersonic projectiles.Based on the optimal trajectory trends and feedback guidance methods,a new cooperative control algorithm is proposed to optimize trajectories of multi-UAVs for target tracking in approaching stage.Based on UAV kinematics and sensor performance models,optimal trajectory trends of UAVs are analyzed theoretically.Then,feedback guidance methods are proposed under the optimal observation trends of UAVs in the approaching target stage,producing trajectories with far less computational complexity and performance very close to the best-known trajectories.Next,the sufficient condition for the UAV to form the optimal observation configuration by the feedback guidance method is presented,which guarantees that the proposed method can optimize the observation trajectory of the UAV in approaching stage.Finally,the feedback guidance method is numerically simulated.Simulation results demonstrate that the estimation performance of the feedback guidance method is superior to the Lyapunov guidance vector field(LGVF)method and verify the effectiveness of the proposed method.Additionally,compared with the receding horizon optimization(RHO)method,the proposed method has the same optimization ability as the RHO method and better real-time performance. 展开更多

In the realm of acoustic signal detection,the identification of weak signals,particularly in the presence of negative signal-to-noise ratios,poses a significant challenge.This challenge is further heightened when signals are acquired through fiber-optic hydrophones,as these signals often lack physical significance and resist clear systematic modeling.Conventional processing methods,e.g.,low-pass filter(LPF),require a thorough understanding of the effective signal bandwidth for noise reduction,and may introduce undesirable time lags.This paper introduces an innovative feedback control method with dual Kalman filters for the demodulation of phase signals with noises in fiber-optic hydrophones.A mathematical model of the closed-loop system is established to guide the design of the feedback control,aiming to achieve a balance with the input phase signal.The dual Kalman filters are instrumental in mitigating the effects of signal noise,observation noise,and control execution noise,thereby enabling precise estimation for the input phase signals.The effectiveness of this feedback control method is demonstrated through examples,showcasing the restoration of low-noise signals,negative signal-to-noise ratio signals,and multi-frequency signals.This research contributes to the technical advancement of high-performance devices,including fiber-optic hydrophones and phase-locked amplifiers. 展开更多

First of all, according to the first-order approximation method and the stabilization condition of the linear system, one linear feedback controller is structured to control the chaotic system without time-delay, its chaotic behavior is eliminated and stabilized to its equilibrium. After that, based on the first-order approximation method, the Lyapunov stability theorem, and the matrix inequality theory, the other linear feedback controller is structured to control the chaotic system with time-delay and make it stabilized at its equilibrium. Finally, two numerical examples are given to illustrate the correctness and effectiveness of the two linear feedback controllers. 展开更多

For the appearance of the additive perturbation of controller gain when the controller parameter has minute adjustment at the initial running stage of system,to avoid the adverse effects,this paper investigates the mixed H_2/H_∞ state feedback attitude control problem of microsatellite based on extended LMI method.Firstly,the microsatellite attitude control system is established and transformed into corresponding state space form.Then,without the equivalence restriction of the two Lyapunov variables of H_2 and H∞performance,this paper introduces additional variables to design the mixed H_2/H_∞ control method based on LMI which can also reduce the conservatives.Finally,numerical simulations are analyzed to show that the proposed method can make the satellite stable within 20 s whether there is additive perturbation of the controller gain or not.The comparative analysis of the simulation results between extended LMI method and traditional LMI method also demonstrates the effectiveness and feasibility of the proposed method in this paper. 展开更多

The phase difference between the two intensities in birefringence feedback changes with the angle between the optic axis of the birefringence element and laser original polarization. The phase difference is highly sensitive to the relative position of the optic axis and the laser original polarization. This method is used to highly precisely determine the optic axis azimuth, and is able to distinguish between the fast axis and the slow axis of the birefringence element. 目的探讨基于病历书写的三段反馈法对实习医学生临床思维能力的培养效果。方法采用前瞻性随机对照设计,纳入2023年1月—2024年12月在西安交通大学第二附属医院肾病内科轮转的五年制临床医学专业实习医学生99名,随机分为三段反馈组(n=50... 目的探讨基于病历书写的三段反馈法对实习医学生临床思维能力的培养效果。方法采用前瞻性随机对照设计,纳入2023年1月—2024年12月在西安交通大学第二附属医院肾病内科轮转的五年制临床医学专业实习医学生99名,随机分为三段反馈组(n=50)与传统模式组(n=49)。其中,三段反馈组采用“即时纠错-逻辑追问-示范重构”模式开展病历书写教学,传统模式组则采用病历集中批改模式实施教学。评估阶段分别于入科时、出科时,依据《内科住院病历书写质量评分标准》对两组实习医学生的病历书写质量进行盲法评估。结果出科评估结果在病历书写质量总分方面,三段反馈组(90.02±2.15)显著高于传统模式组(86.22±2.24),差异有统计学意义(P<0.001)。在临床思维相关要素评分方面,三段反馈组在主诉(P<0.001)、体格检查(P=0.001)、病历摘要(P<0.001)、诊断(P<0.001)等关键项目得分亦显著高于传统模式组。结论三段反馈法通过构建“即时纠错-逻辑追问-示范重构”闭环训练体系,能显著提升实习医学生的病历书写质量及临床思维能力。 展开更多

同步发电机(SG)和跟网型变流器并网系统在弱电网下会存在稳定性问题,而构网型变流器并网系统在强电网下会存在稳定性问题,但对这三种电源接入电网后强弱电网失稳的特性缺少统一认识。为此,该文对SG和非SG电源并网系统的强弱电网小扰动... 同步发电机(SG)和跟网型变流器并网系统在弱电网下会存在稳定性问题,而构网型变流器并网系统在强电网下会存在稳定性问题,但对这三种电源接入电网后强弱电网失稳的特性缺少统一认识。为此,该文对SG和非SG电源并网系统的强弱电网小扰动失稳机理进行比较分析。首先,用特征值分析法对三种电源并网系统的同步环二阶模型和考虑外环(分别对应励磁调节和暂态电势、直流电压控制、端电压控制)的四阶模型的小扰动稳定性进行比较,确定了三种电源并网系统强弱电网失稳现象均与外环控制有关;其次,分别构建三种电源并网系统的类Heffron-Phillips模型,通过以同步环为主导环路、外环控制为附加支路的统一视角进行分析,基于反馈支路传递函数分解法,将三种电源并网系统不同结构的反馈传递函数G(s)分解为统一结构的二阶振荡环节H_(1)(s)和剩余环节H_(2)(s),统一分析系统强弱电网失稳的附加负阻尼转矩机理;最后,通过Matlab/Simulink仿真和硬件在环实验验证上述理论分析结果的有效性,并对SG和非SG电源接入强弱电网系统小扰动稳定的更加一般化统一认识加以讨论。 展开更多

Aims and objective:A theoretical model based on guided postoperative rehabilitation with feedback was developed for patients with lumbar intervertebral disc protrusion to explore the effects of feedback-based health education.Patients were assessed in terms of disease knowledge,general self-efficacy,medical behaviour,and the Oswestry disability index(ODI).During postoperative rehabilitation,nursing staff provided health education regarding lumbar intervertebral disc protrusion.Design:The CONSORT 2010 statement extension for cluster randomized trials checklist(Campbell et al.,2012)was used to guide reporting outcomes(File S1).Methods:The methods were based on previous research on the feedback method of health education.A cluster randomized controlled trial design was used.From March 2019 to January 2020,patients who underwent spinal surgery at a first-class tertiary hospital were enrolled,and 76 met the inclusion criteria for lumbar disc disease(lumbar disc herniation,LDH).Patients were divided into an experimental group and a control group,with 38 patients in each group.Feedback-based health education was conducted for the experimental group,and routine health education was provided for the control group.Disease knowledge,general self-efficacy,compliance behaviour,and ODI scores were compared between the two groups.The intervention effect was compared between groups by means of t tests.SPSS 23.0 was used for statistical analysis,and P<0.05 was considered statistically significant.The research was reported according to CONSORT statement.Results:1.Comparison of baseline data between the two groups before the intervention.Before the intervention,there was no statistically significant difference between the two groups in terms of general demographic data,disease-related data,disease knowledge,general self-efficacy,compliance behaviour,or ODI(P>0.05).2.Comparison of relevant indicators between the two groups of patients before and after the intervention.(1)One,four and 12 weeks after surgery,the disease knowledge,general self-efficacy and postoperative compliance behaviour of the experimental group were significantly higher than those of the control group(P<0.05).(2)At 1,4 and 12 weeks after surgery,the ODI of the experimental group was significantly lower than that of the control group(P<0.05).Conclusions:Feedback-based health education is conducive to improving disease knowledge,general self-efficacy and compliance behaviour and reducing ODI scores in patients with LDH.Relevance to clinical practice:This study provides nurses worldwide with a reference for health education during postoperative rehabilitation for lumbar disc herniation. 展开更多

目的构建基于Donabedian三维质量结构模型和360度反馈评价的临床护理教师岗位胜任力评价标准,为测评临床护理教师岗位胜任力提供参考。方法以360度反馈评价为理论基础,并结合Donabedian三维质量评价结构模型,采用文献分析、质性访谈初... 目的构建基于Donabedian三维质量结构模型和360度反馈评价的临床护理教师岗位胜任力评价标准,为测评临床护理教师岗位胜任力提供参考。方法以360度反馈评价为理论基础,并结合Donabedian三维质量评价结构模型,采用文献分析、质性访谈初步确定评价指标条目,采用德尔菲法对17名专家进行2轮函询,通过层次分析法确定各指标权重。结果函询专家权威系数为0.851,第1轮和第2轮函询问卷的有效回收率分别为81.82%和94.44%,一、二、三级指标的变异系数分别为0.037~0.059和0.029~0.059,Kendall协调系数W分别为0.164~0.283、0.257~0.346(均P<0.05),其一致性比率值均<0.1。最终形成的临床护理教师岗位胜任力评价指标包括3个一级指标即结构、过程和结果指标,7个二级指标和43个三级指标。结论本研究构建的临床护理教师岗位胜任力评价指标科学合理,维度和指标分层明确,可为评价临床护理教师岗位胜任力提供依据。 展开更多

A strategy for time-delayed feedback control optimization of quasi linear systems with random excitation is proposed.First,the stochastic averaging method is used to reduce the dimension of the state space and to derive the stationary response of the system.Secondly,the control law is assumed to be velocity feedback control with time delay and the unknown control gains are determined by the performance indices.The response of the controlled system is predicted through solving the Fokker-Plank-Kolmogorov equation associated with the averaged Ito equation.Finally,numerical examples are used to illustrate the proposed control method,and the numerical results are confirmed by Monte Carlo simulation. 展开更多

An algorithm is proposed which uses the method of a feasible direction matrix. The algorithm only requires the solution of an algebraic Riccati equation (ARE) and makes the H∞norm of the closedloop transfer function matrix from disturbance inputs to controlled outputs less than a given constant which ensure the stability of the overall controlled system at each iteration. The given example shows that the convergence of the algorithm is satisfactory. 展开更多
