TiO2 nanotubes(NT)has been demonstrated its potential in orthopaedic applications due to its enhanced surface wettability and bio-osteointegration.However,the fretting biocorrosion is the main concern that limited its successfully application in orthopaedic application.In this study,a structure optimised thin TiO2 nanotube(SONT)layer was successfully created on Ti6Al4V bone screw,and its fretting corrosion performance was investigated and compared to the pristine Ti6Al4V bone screws and NT decorated screw in a bone-screw fretting simulation rig.The results have shown that the debonding TiO2 nanotube from the bone screw reduced significantly,as a result of structure optimisation.The SONT layer also exhibited enhanced bio-corrosion resistance compared pristine bone screw and conventionally NT modified bone screw.It is postulated that interfacial layer between TiO2 nanotube and Ti6Al4V substrate,generated during structure optimisation process,enhanced bonding of TiO2 nanotube layer to the Ti6Al4V bone screws that leading to the improvement in fretting corrosion resistance.The results highlighted the potential SONT in orthopaedic application as bone fracture fixation devices. 展开更多

The locking compression plates (LCP) are efficient tools in open reduction and internal fixation (ORIF), especially in osteoporotic bones. Two important factors of screw density and screw position can affect the functionality of the bone plate. Several studies have assessed the influence of the screw configurations on the bone-plate stiffness, but the effects of screw positions on the interfragmentary strain, εIF of LCP construct have not been investigated yet. In this study, finite element method was used to investigate the influence of screws number and position on the interfragmentary strain of LCP-femur system for a mid-shaft fracture. Results of this study showed that by insertion of screws closer to the fracture site, εIF decreases by 2nd degree polynomial function versus screw position, but by adding the screws from the ends of the plate, or by moving and placing the screws towards the fracture site, the reduction of εIF will be linear. Results of this study were compared and are in agreement with some studies in the literature, even though their scope was mostly stability of the bone-implant system, whereas our scope was focused on the interfragmentary strain. 展开更多

Metallic bone screws are clinically used to fix the fractured bone fragments in bone defect treatment;yet they present compromised therapeutic efficacy due to poor osseointegration and tissue support.Here,we develop a novel thermoresponsive shape memory(SMP)bone screw with osteogenesis-angiogenesis coupling for enhanced bone regeneration.The SMP bone screws are prepared by die casting of shape memory polyurethane/hydroxyapatite(PU/HA)composite,coated with L-arginine(Arg)and calcium ions(Ca^(2+)).The SMP bone screw could shrink and be easily reshaped at room temperature(25◦C)and then rapidly recover to its original state(37℃),granting it robust internal fixation capacity(2-fold increase in pull-out force)and beneficial compressive force to nearby tissues.Additionally,the long-term release of L-arginine and calcium ions synergistically activate the nitric oxide-cyclic guanosine monophosphate(NO-cGMP)signaling pathway of native cells.Synergized with its shape memory function,the SMP bone screw activated calcium signaling pathway under the stimulation of mechanical stress,promote the activation of various osteogenic pathways(e.g.PI3K-Akt signaling pathway),and upregulate the NO-cGMP pathway by regulating the influx of calcium ions and arginine to synchronously coordinate osteogenesis and angiogenesis to accelerate bone repair.We envision that our slot-in,snap-back and homeothermal shape memory bone screw,with its easily reshaped and fast stress release properties and osteogenesis-angiogenesis coupling efficacy,can shed new light on the development of clinical bone screws. 展开更多

This paper reports a series of in vitro, ex vivo and in vivo mechanical and corrosion studies of pin and screw prototype made of friction welded pure iron and 316 L type stainless steel aiming to evaluate the applicability of the partially removable bone screws. Results showed that the pin possesses bending, tensile and torsional strengths of1706±147, 666±7 and 0.34±0.03 MPa, respectively. The pin degraded at an average weight loss rate of 17.15×10-5 g cm-2 day-1 and released Fe ions at an average concentration of 2.38 ppm. Plastic deformation induced by torsion increased the corrosion rate of the pin from 0.0014 to 0.0137 mm year-1.The maximum pull-out load of the screw prototypes was3800 N with a calculated failure strength by shear load equal to22.2 k N which is higher than the strength of the cortical bone.Detailed analysis of the rat's blood cells during 60 days of the pin implantation indicated a normal response with low neutrophils/lymphocytes ratio of 0.3-0.5. Iron ion concentration in the rat's blood slightly increased from 55 to 61 ppm without affecting the tissue recovering and healing phase. Histological evaluation confirmed the presence of macrophage cells as a normal response to the released iron particles around the iron section of the pin. 展开更多

Lumbar degenerative disc disease(DDD)in the elderly population remains a global health problem,especially in patients with osteoporosis.Osteoporosis in the elderly can cause failure of internal fixation.Cortical bone trajectory(CBT)is an effective,safe and minimally invasive technique for the treatment of lumbar DDD in patients with osteoporosis.In this review,we analyzed the anatomy,biomechanics,and advantages of the CBT technique in lumbar DDD and revision surgery.Additionally,the clinical trials and case reports,indications,advancements and limitations of this technique were further discussed and reviewed.Finally,we concluded that the CBT technique can be a practical,effective and safe alternative to traditional pedicle screw fixation,especially in DDD patients with osteoporosis. 展开更多

Treatment of Thoracolumbar Vertebrate Fracture by Transpedicular Morselized Bone Grafting in Vertebrae for Spinal Fusion and Pedicle Screw Fixation 被引量:16

To enhance the fusion of graft bone in thoracolumbar vertebrae and minimize the postoperative loss of correction, short-segment pedicle screw fixation was reinforced with posterior moselizee bone grafting in vertebrae for spinal fusion in patients with thoracrolumbar vertebrate fractures. Seventy patients with thoracrolumbar vertebrate fractures were treated by short-segment pedicle screw fixation and were randomly divided into two groups. Fractures in group A (n=20) were rein-forced with posterior morselized bone grafting in vertebrae for spinal fusion, while patients group B (n=50) did not receive the morselized bone grafting for bone fusion. The two groups were compared in terms of kyphotic deformity, anterior vertebral height, instrument failure and neurological functions after the treatment. Frankel grading system was used for the evaluation of neurological evaluation and Denis scoring scale was employed for pain assessment. The results showed that the kyphosis correction was achieved in both group A and group B (group A: 6.4 degree; group B: 5.4 degree)/At the end of follow-up, kyphosis correction was maintained in group A but lost in group B (P=0.0001). Postoperatively, greater anterior height was achieved in group A than in group B (P〈0.01). During follow-up study, anterior vertebral height was maintained only in Group A (P〈0.001). Both group A and group B showed good Denis pain scores (P1 and P2) but group A outdid group B in terms of control of severe and constant pain (P4 and P5). By Frankel criteria, the changes in neurological functions in group A was better than those of group B (P〈0.001). It is concluded that reinforcement of short-segment pedicle fixation with morselized bone grafting for the treatment of patients with thoracolumbar vertebrae fracture could achieve and maintain kyphosis correction, and it may also increase and maintain anterior vertebral height. Morselized bone grafting in vertebrae offers immediate spinal stability in patients with thoracolumbar vertebrate fractures, decreases the instrument failure and provides better postoperative pain control than without the morselized bone grafting. 展开更多

Posterior lumbar interbody fusion (PLIF) is a common surgical procedure and widely used in the treatment of lumbar degenerative disc disorders. Traditionally, posterior lumbar interbody fusion is done by using the traditional pedicle screw (PS) which offers great advantages, but at the same time it has some disadvantages which include the risk of superior facet joint violation and muscle damage. Recently, an alternative method of screw insertion via cortical bone trajectory (CBT) has been invented which has less invasive process and can be placed without the drawbacks associated with the traditional pedicle screw. However, it has to remain an interest whether CBT will provide similar or greater clinical outcomes compared to PS in PLIF. So the main aim of this review is to compare the clinical outcomes of cortical bone trajectory and traditional pedicle screw fixation in posterior lumbar interbody fusion based on the articles published on this topic. Compared to the traditional pedicle screw fixation, PLIF with CBT has similar clinical outcome based on pain intensity, ODI status and JOA score, as well as similar fusion rate and radiological evaluated complication such as loosening of screw. In addition PLIF with CBT has advantages of less facet joint violation, less blood loss, less intraoperative muscle damage and perioperative pain. On the basis of this study, we can suggest that PLIF with CBT can be considered as a reasonable alternative to PS in PLIF. 展开更多

Though a bone/metal interface in osseointegration was also formed on the surface of uncoated 317L plate screw after implantation, a smal1 space was seen between the bone and surface of the screw, indicating that the tissue compatibility of titanium-coated 317L plate screw may be better than that of the non-coated screw. 展开更多

背景:与传统椎弓根螺钉相比,皮质骨轨迹螺钉通道全程均为皮质骨,具有更牢靠的生物力学优势,然而安全、准确地置入皮质骨轨迹螺钉对脊柱外科医师技术要求较高。理论上在3D打印个性化导板引导下能有效提高一次性置钉的准确度、简化置钉操... 背景:与传统椎弓根螺钉相比,皮质骨轨迹螺钉通道全程均为皮质骨,具有更牢靠的生物力学优势,然而安全、准确地置入皮质骨轨迹螺钉对脊柱外科医师技术要求较高。理论上在3D打印个性化导板引导下能有效提高一次性置钉的准确度、简化置钉操作、提高手术效率,但目前国内外相关报道较少。目的:探讨应用个性化3D打印导板辅助皮质骨轨迹螺钉置入治疗合并骨质疏松胸腰椎骨折的准确性与安全性。方法:选择2020年1月至2024年1月行皮质骨轨迹螺钉内固定的58例合并骨质疏松的胸腰椎(T_(11)-L_(4))骨折患者,共置入348枚螺钉。根据置钉方案分为2组,3D导板组30例行个性化3D打印导板辅助置钉,共置入螺钉180枚;对照组28例行徒手方式置钉,共计置入螺钉168枚。通过比较两组间手术相关指标、置钉情况、伤椎的复位与脊柱后凸畸形矫正情况以及术后疗效等,评估不同置钉方案的效果和适用性。结果与结论:①3D导板组在切口长度、手术时间、置钉时间、术中透视次数、术中出血量、术后引流量以及住院天数方面均显著优于对照组(P<0.05),但是两组的总费用相比差异无显著性意义(P>0.05);②3D导板组的置钉准确率高于对照组,螺钉对上关节突的侵犯率低于对照组,差异有显著性意义(P<0.05);③术后评估结果显示,两组在伤椎前缘高度百分比、Cobb角、目测类比评分以及Oswestry功能障碍指数方面均较术前有所改善,且差异有显著性意义(P<0.05);组间相比,3D导板组的Oswestry功能障碍指数更低,显示出更佳的胸腰椎功能(P<0.05),而在术后伤椎前缘高度百分比、Cobb角及目测类比评分方面,两组之间无显著性差异(P>0.05);④在围术期及随访期间,两组患者均未出现并发症;⑤提示个性化3D打印导板辅助皮质骨轨迹螺钉置入治疗合并骨质疏松的胸腰椎骨折是一种可行的方式,具有微创、高效、精准及安全等优势,可获得满意的临床效果。 展开更多

背景:髋臼顶部骨缺损是人工髋翻修过程中常见而又复杂的挑战,因为该部位的骨缺损会影响翻修术中髋臼杯的初始稳定性以及中长期稳定性。目的:评估髋臼阻挡螺钉联合颗粒打压植骨技术重建髋关节翻修过程中髋臼顶部骨缺损的效果。方法:选择2... 背景:髋臼顶部骨缺损是人工髋翻修过程中常见而又复杂的挑战,因为该部位的骨缺损会影响翻修术中髋臼杯的初始稳定性以及中长期稳定性。目的:评估髋臼阻挡螺钉联合颗粒打压植骨技术重建髋关节翻修过程中髋臼顶部骨缺损的效果。方法:选择2018年3月至2022年12月佛山市中医院收治的11例人工全髋关节置换后髋臼杯松动合并髋臼顶部PaproskyⅢA型骨缺损患者,均采用髋臼阻挡螺钉联合颗粒打压植骨技术重建骨缺损。记录并对比翻修前和翻修后末次随访的髋关节Harris评分、满意度评分以及双下肢长度差;通过X射线片评估翻修后髋臼杯以及髋臼阻挡螺钉的状态。结果与结论:①11例患者随访时间为20-72个月;②截止到术后末次随访,所有患者的髋关节Harris评分、满意度评分以及双下肢长度差与翻修术前比较,均有显著改善(P<0.001);③末次随访X射线片提示所有患者髋臼杯周缘都有良好的骨长入,没有髋臼杯再次松动的情况发生;④所有髋臼杯阻挡螺钉在术后末次随访X射线片上均没有发生移位或者断裂;⑤结果表明,髋臼阻挡螺钉联合颗粒打压植骨是重建髋关节翻修过程中髋臼顶部骨缺损一种简便而有效的技术,不仅有助于人工髋关节翻修过程中髋臼杯初始稳定性的实现,也有利于加强髋臼杯的中远期稳定性。 展开更多

背景:骨质疏松症导致的骨小梁结构稀少不足以维持螺钉的把持力,常导致内固定失败,可通过增加螺钉直径和长度、改变螺钉表面涂层、骨水泥强化钉道等措施来增加螺钉的把持力。骨水泥强化皮质骨轨迹置钉技术和采用新型变径螺钉的改良皮质... 背景:骨质疏松症导致的骨小梁结构稀少不足以维持螺钉的把持力,常导致内固定失败,可通过增加螺钉直径和长度、改变螺钉表面涂层、骨水泥强化钉道等措施来增加螺钉的把持力。骨水泥强化皮质骨轨迹置钉技术和采用新型变径螺钉的改良皮质骨轨迹置钉技术已被证实有良好的固定效果,目前需要对两者相关力学性能进行分析比较。目的:利用有限元分析法比较骨水泥强化皮质骨轨迹置钉技术、皮质骨轨迹置钉技术和采用新型变径全皮质骨螺纹螺钉的改良皮质骨轨迹置钉技术在腰椎内固定治疗中的力学性能。方法:基于3具骨质疏松椎体标本的CT扫描数据构建L4腰椎模型,并在改良皮质骨置钉技术中应用了创新的变径全皮质骨螺钉(改良皮质骨轨迹置钉组),螺钉全长45 mm,直径为5.5-4.3 mm变径;与未强化皮质骨轨迹置钉组(直径为5.5 mm、长度为40 mm)和骨水泥强化皮质骨轨迹置钉组(直径为5.5 mm、长度为40 mm、中心开孔直径为1 mm)进行对比。通过测定轴向拔出力、螺钉稳定性(上、下、左、右载荷位移比)、椎体活动度比较各组螺钉的固定强度。结果与结论:①轴向抗拔出力:强化皮质骨轨迹置钉组>改良皮质骨轨迹螺钉组(P=0.0246),且强化皮质骨轨迹置钉组、改良皮质骨轨迹置钉组均>未强化皮质骨轨迹置钉组(P=0.0001,P=0.00264);②螺钉稳定性:在下方施加载荷时,3组螺钉的载荷位移比值为强化皮质骨轨迹置钉组>未强化皮质骨轨迹置钉组、改良皮质骨轨迹螺钉组(均P<0.05),且改良皮质骨轨迹螺钉组>未强化皮质骨轨迹螺钉组(P<0.05);③椎体活动度:在5种工况下骨水泥强化皮质骨轨迹置钉组<改良皮质骨轨迹螺钉组,但差异无显著性意义(P>0.05),且骨水泥强化皮质骨轨迹置钉组、改良皮质骨轨迹螺钉组均<未强化皮质骨轨迹螺钉组;④与改良皮质骨轨迹螺钉组比较,骨水泥强化皮质骨轨迹置钉组的螺钉载荷位移比、腰椎活动度等相关力学性能均有提升,但差异均无显著性意义(P>0.05);⑤提示骨水泥强化皮质骨轨迹置钉技术与新型变径全皮质骨螺钉相比,具有更好的生物力学性能,且骨水泥强化皮质骨轨迹置钉技术可能是临床骨质疏松症患者内固定治疗更为合适的置钉选择。 展开更多

目的对比研究分析椎旁肌间隙(Wiltse)入路下后路椎弓根钉棒复位固定术与后正中小切口非融合手术治疗胸腰段脊柱骨折(TSF)患者的效果。方法回顾性分析郑州市骨科医院2023年2月至2025年2月收治的104例TSF患者资料,按手术方案分两组。以接... 目的对比研究分析椎旁肌间隙(Wiltse)入路下后路椎弓根钉棒复位固定术与后正中小切口非融合手术治疗胸腰段脊柱骨折(TSF)患者的效果。方法回顾性分析郑州市骨科医院2023年2月至2025年2月收治的104例TSF患者资料,按手术方案分两组。以接受Wiltse入路下后路椎弓根钉棒固定术治疗的52例患者列为A组,以接受后正中小切口非融合手术治疗的52例患者列为B组。对比两组手术相关指标,术前、术后3个月视觉模拟评分法(VAS)、Oswestry功能障碍指数(ODI)、伤椎后凸Cobb角、伤椎前缘高度(AVH)百分比、骨代谢水平[β-胶原降解产物(β-CTX)、骨保护素(OPG)、骨钙素(BGP)],并发症发生率。结果A组手术及住院时长较B组短,术中出血量及术后引流量较B组少(P<0.05);A组术后3个月VAS、ODI评分较B组低(P<0.05);A组术后3个月伤椎后凸Cobb角较B组低,AVH百分比较B组高(P<0.05);A组术后3个月血清β-CTX水平较B组低,OPG、BGP较B组高(P<0.05);A组并发症发生率[1.92%(1/52)]较B组[15.38%(8/52)]低(P<0.05)。结论相较于后正中小切口非融合手术治疗TSF患者,经Wiltse入路下后路椎弓根钉棒固定术治疗更有助于优化手术相关指标,降低并发症风险,减轻术后疼痛,恢复脊柱正常结构及稳定性,改善肢体功能障碍及机体骨代谢水平。 展开更多

目的评估脊柱内镜下胸、腰椎结核病灶清除术联合自体髂骨椎间植骨融合及经皮椎弓根螺钉内固定术的临床应用效果。方法回顾性分析2020年6月至2023年12月柳州市人民医院收治的44例胸、腰椎结核患者资料,其中男24例,女20例;年龄20~77岁,平... 目的评估脊柱内镜下胸、腰椎结核病灶清除术联合自体髂骨椎间植骨融合及经皮椎弓根螺钉内固定术的临床应用效果。方法回顾性分析2020年6月至2023年12月柳州市人民医院收治的44例胸、腰椎结核患者资料,其中男24例,女20例;年龄20~77岁,平均(54.00±12.00)岁。根据手术方式不同分为两组,其中试验组22例,采用脊柱内镜辅助下病灶清除+自体髂骨椎间植骨融合+经皮椎弓根螺钉固定术;对照组22例,采用常规后路开放病灶清除及内固定术。对比两组手术相关指标、手术前后疼痛视觉模拟评分(visual analogue scale,VAS)、Oswestry功能障碍指数(oswestry disability index,ODI)、红细胞沉降率(erythrocyte sedimentation rate,ESR)、C反应蛋白(C-reactive protein,CRP)、Cobb角以及术后并发症发生情况。结果术后随访≥12个月。试验组的手术出血量、手术时间、住院时间均少于对照组(P<0.05)。术后1、3、6、12个月,试验组VAS均优于对照组(P<0.05);术后1、6个月,试验组ODI均优于对照组(P<0.05);术后1、6、12个月,试验组Cobb角均优于对照组(P<0.05);术后6、12个月,试验组ESR、CRP均优于对照组(P<0.05)。试验组术后出现下肢麻木3例,无二次手术;对照组出现脓肿及愈合不良3例(行二次手术),下肢麻木、感觉减退2例,下肢肌无力1例。结论脊柱内镜下病灶清除、自体髂骨椎间植骨融合联合经皮椎弓根螺钉内固定治疗胸、腰椎结核,创伤小、恢复快、疗效良好,是一种有效且安全的方法。 展开更多

新型变径螺钉在腰椎改良皮质骨轨迹中的有限元分析

背景:骨质疏松患者松质骨不足以维持螺钉把持力,脊柱内固定失败率高。腰椎传统椎弓根螺钉技术和皮质骨轨迹置钉技术仍有不足,改良皮质骨轨迹置钉技术力学性能较好,但目前尚无与其特点相匹配的螺钉。目的:通过有限元分析法比较3种新型变... 背景:骨质疏松患者松质骨不足以维持螺钉把持力,脊柱内固定失败率高。腰椎传统椎弓根螺钉技术和皮质骨轨迹置钉技术仍有不足,改良皮质骨轨迹置钉技术力学性能较好,但目前尚无与其特点相匹配的螺钉。目的:通过有限元分析法比较3种新型变径全皮质骨螺纹螺钉与非变径全皮质骨螺纹螺钉在改良皮质骨轨迹技术中的性能。方法:根据1具成人骨质疏松尸体标本(男性,63岁)的CT数据建立L1-L5腰椎及骶骨模型,在改良皮质骨轨迹技术中分别使用3种新型变径全皮质骨螺纹螺钉(单螺纹螺钉,全长45 mm,粗杆直径5.5 mm,细杆直径4.35 mm,分别在2/3处、1/2处、1/3处进行均匀变径,变径段长8 mm)与非变径全皮质骨螺纹螺钉(单螺纹螺钉,全长45 mm、直径5.5 mm)在腰椎L4-L5进行置钉,未置钉对照组与4组置钉模型组进行有限元分析对比。通过计算腰椎活动度、内固定系统应力、L4-L5椎体应力、L4-L5椎间盘应力比较各组螺钉在改良皮质骨轨迹技术下的力学性能。结果与结论:在前屈、后伸、侧弯、轴向旋转工况下,①2/3变径螺钉组相比对照组,腰椎整体关节活动度分别降低14.16%,23.75%,8.44%及7.17%;非变径螺钉组相比对照组,分别降低14.45%,23.97%,8.52%及7.07%;3种新型变径螺钉与非变径螺钉水平相近;②2/3变径螺钉组相比非变径螺钉组,内固定系统应力分别减少3.94%、增加20.60%、增加1.16%、减少8.59%;1/2变径螺钉组相比非变径螺钉组,内固定系统应力分别增加2.06%、减少0.05%、减少0.14%、减少5.98%;1/3组相比非变径螺钉组,内固定系统应力分别增加4.16%、增加13.96%、增加5.07%、减少4.19%;③4种置钉组椎体应力相比对照组均有升高,3种变径螺钉组相比非变径螺钉组没有明显升高;④4种置钉组固定节段椎间盘应力相比对照组均有显著降低,3种变径螺钉组相比非变径螺钉组基本相同。提示在改良皮质骨轨迹技术中,新型变径螺钉可以提供不弱于非变径螺钉的固定效果。 展开更多

Bone screws are devices used to fix implants or bones to bones.However,conventional screws are mechanically fixed with thread and often face long-term failure due to poor osseointegration.To improve osseointegration,screws are evolving from solid and smooth to porous and rough.Additive manufacturing(AM)offers a high degree of manufacturing freedom,enabling the preparation of predesigned screws that are porous and rough.This paper provides an overview of the problems currently faced by bone screws:long-term loosening and screw breakage.Next,advances in osseointegrated screws are summarized hierarchically(sub-micro,micro,and macro).At the sub-microscale level,we describe surface-modification techniques for enhancing osseointegration.At the micro level,we summarize the micro-design parameters that affect the mechanical and biological properties of porous osseointegrated screws,including porosity,pore size,and pore shape.In addition,we highlight three promising pore shapes:triply periodic minimal surface,auxetic structure with negative Poisson ratio,and the Voronoi structure.At the macro level,we outline the strategies of graded design,gradient design,and topology optimization design to improve the mechanical strength of porous osseointegrated screws.Simultaneously,this paper outlines advances in AM technology for enhancing the mechanical properties of porous osseointegrated screws.AM osseointegrated screws with hierarchical design are expected to provide excellent long-term fixation and the required mechanical strength. 展开更多

[目的]探讨人文护理在Ⅲ期Kümmell病后路椎板减压椎弓骨水泥钉固定围手术期的作用。[方法]2023年12月—2024年11月在本科行后路椎板减压椎弓骨水泥钉固定术治疗Ⅲ期Kümmell病的患者50例纳入本研究。采用随机数字表法,将患者分为两组。其中,25例围手术期给予人文护理,另外25例围手术期给予常规护理。比较两组患者临床资料。[结果]两组患者均顺利完成后路椎板减压椎弓骨水泥钉固定术,无严重并发症。随时间推移,两组的VAS评分、ODI指数均显著下降(P<0.05);相应时间点,人文组的VAS评分均显著低于常规组(P<0.05);人文组的ODI指数均低于常规组,术后72 h时[%,(22.2±7.8)vs(30.6±6.8),P=0.001]和术后3个月[%,(6.0±2.3)vs(10.8±3.9),P<0.001]两组间的差异均有统计学意义。随时间推移,两组HAMA评分均显著降低(P<0.05);相应时间点人文组的HAMA评分均显著低于常规组(P<0.05)。随时间推移,两组的Barthel指数均呈负向波峰改变(P<0.05);术后72 h时[分,(47.1±5.6)vs(34.7±4.0),P<0.001]和出院时[分,(63.0±6.2)vs(53.0±8.8),P<0.001]人文组的Barthel指数均显著高于常规组。[结论]在Ⅲ期Kümmell病后路椎板减压椎弓骨水泥钉固定术围手术期采用人文护理,可有效缓解患者住院期间心理焦虑,减轻围术期疼痛,提高围术期舒适度,加快患者围术期功能康复。 展开更多

目的观察在斜外侧腰椎椎间融合术(OLIF)中应用明胶海绵-重组人骨形态发生蛋白-2(rh BMP-2)复合活性骨修复材料“三明治”封装植骨技术,在防止活性骨修复材料漏出及促进骨融合方面的安全性与有效性。方法回顾性分析2017年11月至2023年7... 目的观察在斜外侧腰椎椎间融合术(OLIF)中应用明胶海绵-重组人骨形态发生蛋白-2(rh BMP-2)复合活性骨修复材料“三明治”封装植骨技术,在防止活性骨修复材料漏出及促进骨融合方面的安全性与有效性。方法回顾性分析2017年11月至2023年7月四川大学华西医院收治的834例行L_(4-5)节段OLIF的腰椎退行性疾病患者的临床资料。其中男380例(45.6%),女454例(54.4%),年龄23~89(63.3±11.8)岁。记录手术时间、术中出血量及住院时间。采用视觉模拟量表(VAS)评分及Oswestry功能障碍指数(ODI)评估临床疗效。于术前、术后1 d、术后12个月及术后24个月通过矢状位腰椎三维CT测量椎间隙高度(DH)。采用Marchi分级系统评估融合器沉降程度,采用Bridwell分级标准评估椎间融合情况。结果所有患者手术均顺利完成,未发生植骨材料移位、漏出或相关神经压迫并发症。平均手术时间为(87.2±11.8)min,平均术中出血量为(33.3±7.4)m L,平均住院时间为(5.3±1.5)d。术后随访24~90个月,中位随访时间为28(24,43)个月。与术前相比,术后3个月、12个月及24个月的腰痛与腿痛VAS评分以及ODI均持续显著改善,差异有统计学意义(P<0.05)。术后影像学测量显示,手术节段DH在术后1 d恢复,术后12个月维持良好。术后24个月影像学融合率为93.8%(BridwellⅠ~Ⅱ级),融合器沉降率为22.9%。结论在OLIF中应用明胶海绵-rh BMP-2复合活性骨修复材料“三明治”封装植骨技术显示出良好的安全性和可行性,具有操作简便、手术时间短、并发症少等优点,在保证微创手术效率的同时,获得了良好的临床疗效。 展开更多
