摘要
[目的]探讨人文护理在Ⅲ期Kümmell病后路椎板减压椎弓骨水泥钉固定围手术期的作用。[方法]2023年12月—2024年11月在本科行后路椎板减压椎弓骨水泥钉固定术治疗Ⅲ期Kümmell病的患者50例纳入本研究。采用随机数字表法,将患者分为两组。其中,25例围手术期给予人文护理,另外25例围手术期给予常规护理。比较两组患者临床资料。[结果]两组患者均顺利完成后路椎板减压椎弓骨水泥钉固定术,无严重并发症。随时间推移,两组的VAS评分、ODI指数均显著下降(P<0.05);相应时间点,人文组的VAS评分均显著低于常规组(P<0.05);人文组的ODI指数均低于常规组,术后72 h时[%,(22.2±7.8)vs(30.6±6.8),P=0.001]和术后3个月[%,(6.0±2.3)vs(10.8±3.9),P<0.001]两组间的差异均有统计学意义。随时间推移,两组HAMA评分均显著降低(P<0.05);相应时间点人文组的HAMA评分均显著低于常规组(P<0.05)。随时间推移,两组的Barthel指数均呈负向波峰改变(P<0.05);术后72 h时[分,(47.1±5.6)vs(34.7±4.0),P<0.001]和出院时[分,(63.0±6.2)vs(53.0±8.8),P<0.001]人文组的Barthel指数均显著高于常规组。[结论]在Ⅲ期Kümmell病后路椎板减压椎弓骨水泥钉固定术围手术期采用人文护理,可有效缓解患者住院期间心理焦虑,减轻围术期疼痛,提高围术期舒适度,加快患者围术期功能康复。
[Objective]To explore the role of humanistic nursing care in the perioperative period of posterior laminectomy and pedicle screw fixation with bone cement for stageⅢKümmell's disease.[Methods]A total of 50 patients who undergoing posterior laminectomy and pedicle screw fixation with bone cement for stageⅢKümmell's disease were included in this study in our department from January 2021 to June 2023.The patients were randomly divided into two groups using a random number table.Of them,25 patients received humanistic nursing care during the perioperative period,while the other 25 patients received conventional nursing care.The clinical status of the patients was evaluated.[Results]All patients had the operations performed successfully without serious complications.The VAS and ODI scores were decreased significantly in both groups over time(P<0.05).The humanistic group proved significantly superior to the conventional group in terms of the VAS scores at all matching time points(P<0.05),and ODI score 72 hours postoperatively[%,(22.2±7.8)vs(30.6±6.8),P=0.001]and 3 months after operation[%,(6.0±2.3)vs(10.8±3.9),P<0.001].With time elapsed,the HAMA scores were decreased significantly in both groups(P<0.05);which in the humanistic group were significantly lower than those in the conventional group at all corresponding time points(P<0.05).In addition,the Barthel index in both groups showed a negative peak change over time(P<0.05),which in humanistic group was significantly higher than that in the conventional group 72 hours postoperatively[points,(47.1±5.6)vs(34.7±4.0),P<0.001]and at discharge[points,(63.0±6.2)vs(53.0±8.8),P<0.001].[Conclusion]The application of humanistic nursing care in the perioperative period of posterior laminectomy and pedicle screw fixation with bone cement for stageⅢKümmell's disease does effectively alleviate the psychological anxiety during hospitalization,reduce perioperative pain,improve perioperative comfort,and accelerate the functional recovery.
