BACKGROUND Although breast milk is ideal for newborns,in some cases,it is replaced with cow’s milk,which contains proteins that increase the risk of cow’s milk protein allergy(CMPA).AIM To evaluate CMPA prevalence in Bahrain and compare clinical characteristics of children with immunoglobulin E(IgE)-and non-IgE-mediated CMPA.METHODS This retrospective cohort study examined children with CMPA diagnosed at the pediatric gastroenterology outpatient clinic of the Salmaniya Medical Complex,Bahrain,between 2014 and 2022,and assessed CMPA prevalence.Clinical presentations,laboratory findings,dietary modifications,and outcomes were compared between children with IgE-mediated and non-IgE-mediated CMPA.Categorical variables were compared using Fisher’s exact test or Pearson’sχ2 test,whereas continuous variables were compared using Student’s t-test or the Mann-Whitney U test.RESULTS Of 8332 patients,6022(72.3%)adhered to their appointment.Of them,618(10.3%)were suggested of having CMPA and 595(96.3%)were included;CMPA prevalence was 2%.Most patients were Bahraini(93.8%)and males(55.3%).Non-IgE-mediated type accounted for 77.3%cases and IgE-mediated type,22.7%.IgE-mediated CMPA presented more in boys(P=0.030),and later in life(5.1 months±4.5 months vs 4.2 months±4.2 months,P=0.016,95%CI:0.08-1.73),had more associated diseases(P<0.001);and presented with more cutaneous(P=0.024)and respiratory(P=0.003)manifestations,severe symptoms[rash/dry skin(P=0.031),facial swelling/angioedema(P=0.003),failure to thrive(P=0.013),apparent life-threatening event(P<0.001)],and positive physical findings(P=0.002)than non-IgE-mediated CMPA.Most patients were exclusively fed cow milk formula(50.3%).The amino acid-based formula(AAF)was most prescribed(60.5%)with no difference between the two types(P=0.173).Although breastfeeding was recommended to 49.6%,only 8.2%were exclusively breastfed.IgE-mediated CMPA was associated with a longer follow-up duration than non-IgE-mediated CMPA(17.3 months±14.0 months vs 13.5 months±13.4 months,P=0.005,95%CI:1.1-6.3).CONCLUSION This study revealed a high CMPA prevalence with clinical differences between both types that can influence treatment.AAF was most prescribed,while breastfeeding with dietary modification is rarely applied. 展开更多

BACKGROUND Coronavirus disease 2019(COVID-19)pneumonia with severe septic shock and acute respiratory distress syndrome(ARDS)are critical illnesses for patients following transplant.Intravenous immunoglobulin(IVIG)plays a role in both immune support and inflammation control,especially in immunocompromised patients.This case report describes the first successful experience using IVIG and pulse steroids to manage this critical condition following lung transplantation.CASE SUMMARY A 65-year-old male patient reported a history of chronic obstructive pulmonary disease and poor lung function and received bilateral sequential lung transplantations.Postoperatively,he developed COVID-19 pneumonia,severe septic shock,and ARDS.He recovered from this critical condition after empirical antibiotics administration and veno-venous extracorporeal membrane oxygenation,in addition to IVIG and pulse steroids.CONCLUSION IVIG is a valuable adjunct in managing severe sepsis in lung transplant recipients after COVID-19 infection.We aim,for the first time,to report the success of such a management approach for COVID-19 ARDS and sepsis in the post-lung transplant setting.With further investigations,this is a starting point for wider analysis of such an approach in this setting and consequently helps guide clinical practice for such a challenging patient population moving forward. 展开更多

Background The objective was to characterize the colostrum proteome of primiparous Holstein cows in association with immunoglobulin G(IgG)content.Immediately after calving,colostrum samples were collected from 18 cows to measure IgG concentration.Based on colostrum IgG content,samples were classified through cluster analysis and were identified as poor,average,and excellent quality.The proteome was assessed with quantitative shotgun proteomics;abundance data were compared among the colostrum types;enrichment analysis of metabolic processes and proteins classes was performed as well.We also tested correlations between this proteome and blood globulin level of cows and passive immunity level of calves.Results On average,428 proteins were identified per sample,which belonged mainly to cellular process,biological regulation,response to stimulus,metabolic process,and immune system process.Most abundant proteins were complement C3(Q2UVX4),alpha-S1-casein(P02662),Ig-like domain-containing protein(A0A3Q1M032),albumin(A0A140T897),polymeric immunoglobulin receptor(P81265),lactotransferrrin(P24627),and IGHG1*01(X167014).Colostrum of excellent quality had greater(P<0.05)abundance of serpin A3-7(A2I7N3),complement factorl(A0A3Q1 MIF4),lipocalin/cytosolic fatty-acid binding domain-containing protein(A0A3Q1 MRQ2),complement C3(E1B805),complement component 4 binding protein alpha(A0AAF6ZHP5),and complement component C6(F1MM86).However,colostrum of excellent quality had lower(P<0.05)abundance of HGF activator(E1BCW0),alpha-S1-casein(P02662),and xanthine dehydrogenase/oxidase(P80457).This resulted in enrichment of the biological processes predominantly for complement activation alternative pathway,complement activation,complement activation classical pathway,humoral immune response,leukocyte mediated immunity,and negative regulation of endopeptidase activity in excellent-quality colostrum.Additionally,some colostrum proteins were found to be correlated with the blood globulin level of cows and with the passive immunity level of calves(P<0.05;r≥0.57).Conclusions This study provides new insights into the bovine colostrum proteome,demonstrating associations between IgG levels and the abundance of other proteins,as well as the enrichment of metabolic processes related to innate immune response.Thus,results suggest that the colostrum proteomic profile is associated with the content of IgG.Future research should deeply explore the association of these findings with pre-calving nutrition status and blood composition of the cow,and with passive immunity transfer to the calf. 展开更多

Molecular simulation study on phase separation of immunoglobulin G

Understanding the liquid-liquid phase separation(LLPS)of immunoglobulin G(IgG)is crucial,as it profoundly influences IgG’s biological activity and stability.In this study,we employed coarse-grained molecular dynamics simulations to systematically investigate the phase separation behavior of IgG.We first constructed two types of IgG models:all-pair IgG model and partial-pair IgG model,and compared the coexistence curve from our simulations with experimental data.Our results showed that the partial-pair IgG model aligns better with the experimental critical temperature and critical density.Using this model,we then calculated the temperature-dependent variations of IgG’s radius of gyration,surface tension,viscosity,etc.More importantly,we demonstrated that variations in the interaction strengths among IgG molecules significantly influence their phase separation behavior.Specifically,a higher standard deviation of interaction strength at different temperatures is found to lead to more stable phase-separated states.Furthermore,we observed that the introduction of repulsive polymers and strongly attractive polymers consistently enhances IgG phase separation,while weakly attractive polymers exhibit a dual regulatory effect on the phase separation.Overall,this study provides valuable insights into the mechanisms governing IgG phase behavior,with potential implications for optimizing biopharmaceutical products. 展开更多

Treatment of immunoglobulin A nephropathy:Current perspective and future prospects

Immunoglobulin(Ig)A nephropathy is the most common type of primary glomerulonephritis globally.It typically manifests with microscopic hematuria and a spectrum of proteinuria,although rapidly progressive glomerulonephritis may occur in rare instances.Deposition of IgA in the mesangium seems to be the underlying disease mechanism.Despite current treatment,IgA nephropathy may progress into end-stage renal disease,indicating the necessity for the development of new therapeutic agents.Lifestyle modifications and anti-proteinuric treatment are recommended,and steroids have shown to be beneficial to high risk groups.Nevertheless,other conventional immunosuppressive agents,such as cyclophosphamide and mycophenolate mofetil,may be considered,despite the lack of sufficient evidence to support their efficacy.A considerable proportion of cases remain unresponsive to these treatments,underscoring the need for novel therapeutic approaches.There are several promising immunosuppressive drugs,such as B-cell lineage depleting agents or complement system inhibitors,that are currently undergoing clinical trials.These therapies may be considered for use in selected cases. 展开更多

Metal may affect maternal immune function,but few epidemiological studies have reported the associations between multiple-metal exposure and maternal immunoglobulin(Ig)levels.Based on the Hangzhou Birth Cohort Study,1059 participants were included,and eleven metals in whole blood samples and serum IgA,IgG,IgE and IgM levels were measured.Linear regression,quantile-based g-computation(QGC),and Bayesian kernel machine regression(BKMR)models were used to evaluate the associations.Compared with the first tertile of metal levels,arsenic(As)was negatively associated with IgE(β=-0.25,95%confidence interval(CI)=-0.48 to-0.02).Moreover,significant associations of manganese(Mn)with IgA,IgG and IgM were demonstrated(β=0.10,95%CI=0.04 to 0.18;β=0.07,95%CI=0.03 to 0.12;β=0.10,95%CI=0.03 to 0.18,respectively).Cadmium(Cd)were associated with higher levels of IgM.QGC models showed the positive association of the metalmixtures with IgA and IgG,with Mn playing amajor role.Mn and Cd had positive contributions to IgM,while As had negative contributions to IgE.In the BKMR models,the latent continuous outcomes of IgA and IgG showed a significant increase when all the metals were at their 60th percentile or above compared to those at their 50th percentile.Therefore,exposure to metals was associated with maternal Igs,and mainly showed that Mn was associated with increased levels of IgA,IgG and IgM,and As was associated with low IgE levels. 展开更多

BACKGROUND The exact mechanisms underlying diabetic nephropathy(DN)remain incompletely elucidated,prompting researchers to explore new perspectives and identify novel intervention targets in this field.AIM To explore the role and underlying mechanisms of farnesoid X receptor(FXR)in the development of DN by regulating endoplasmic reticulum stress(ERS)molecular chaperone binding immunoglobulin protein(BiP)expression.METHODS Bioinformatics analyses identified potential FXR-binding elements in the BiP promoter.Dual-luciferase and chromatin immunoprecipitation(ChIP)assays confirmed FXR-BiP binding sites.In vitro studies used SV40 MES 13 cells under varying glucose conditions and treatments with FXR modulators[obeticholic acid(INT-747)and guggulsterones]or BiP small interfering RNA.The expression of BiP and ERS-related proteins[protein kinase R-like endoplasmic reticulum kinase(PERK),inositol-requiring enzyme 1(IRE1),activating transcription factor 6(ATF6)]was assessed alongside cell proliferation and extracellular matrix(ECM)synthesis.In vivo studies in DN mice(db/db)examined the effects of FXR activation on renal function and morphology.RESULTS FXR bound to the target sequence in the BiP promoter region,enhancing transcriptional activity,as confirmed by ChIP experiments.FXR expression decreased in SV40 MES 13 cells stimulated with high glucose and in renal tissues of DN mice compared with control.Treatment of SV40 MES 13 cells with the FXR agonist INT-747 significantly increased intracellular BiP expression,whereas silencing the FXR gene led to the downregulation of BiP levels.In vivo administration of INT-747 significantly elevated BiP levels in renal tissues,improved renal function and fibrosis in DN mice,while inhibiting the expression of ERS-related signaling proteins PERK,IRE1,and ATF6.CONCLUSION FXR promotes BiP expression by binding to its promoter,suppressing ERS pathways,and reducing mesangial cell proliferation and ECM synthesis.These findings highlight FXR as a potential therapeutic target for diabetic glomerulosclerosis. 展开更多

BACKGROUND Immunoglobulin G4(IgG4)-related disease(IgG4-RD),a relatively rare immunemediated chronic inflammatory condition characterized by fibrosis,is capable of affecting multiple organs and systems.Epidemiologically,the disease predominantly affects middle-aged and older men in Asian populations,whereas it shows a female predominance in the corresponding age group in the United States.In IgG4-RD,affected tissues and organs may exhibit diffuse or localized swelling,mimicking neoplastic lesions.IgG4-related cholecystitis(IgG4-CC)represents a manifestation involving the gallbladder,with isolated gallbladder involvement without other organ lesions being exceptionally uncommon.CASE SUMMARY A 53-year-old man was admitted to the hospital with abdominal pain.Preoperative evaluations could not exclude gallbladder carcinoma,and surgical intervention was required.Based on intraoperative findings,postoperative pathology,and postoperative serum IgG4 levels,a diagnosis of IgG4-CC was considered.After glucocorticoid therapy,the patient’s general condition substantially improved.CONCLUSION For patients present with space-occupying lesions of the gallbladder,IgG4-RD should be included in the differential diagnosis. 展开更多

Prognostic value of the MEST-C score in long-term outcomes of immunoglobulin A nephropathy patients:Insights from a developing country

BACKGROUND Immunoglobulin A nephropathy is a leading cause of primary glomerulonephritis globally.Predicting disease progression using clinical markers alone is often inadequate.Integrating the Oxford classification may enhance kidney survival predictions,though its relevance in Pakistan remains unexplored.AIM To determine the correlation between MEST-C scores and clinical parameters,as well as their utility in predicting long-term kidney outcomes.METHODS A retrospective analysis was conducted on biopsy-confirmed immunoglobulin A nephropathy cases diagnosed from 1998 to 2019 at the Sindh Institute of Urology and Transplantation,with a minimum follow-up of 12 months.RESULTS Among 118 patients(mean age:29.03±10.58 years),median proteinuria was 2.13 g/day,and mean estimated glomerular filtration rate(eGFR)was 67.82±44.60 mL/minute/1.73 m^(2).Upon admission,26.4%required kidney replacement therapy.Oxford classification components(E1,T1/T2,C1/C2)were significantly linked to proteinuria and eGFR decline(P=0.00).Remission rates were 79.6%,77.9%,77.1%,49.3%,and 33.3%at 6 months,and at 1 year,2 years,5 years,and 10 years,respectively.End-stage kidney disease progression increased over time,reaching 20.3%,22%,22.8%,31.3%,and 33.8%at 6 months,1 year,2 years,5 years,and 10 years,respectively.Three(2.5%)patients died.The median follow-up was 3.5 years;kidney survival rates were 79.6%,77.9%,77.1%,49.3%and 33.3%at 6 months and at 1 year,2 years,5 years,and 10 years,respectively.Higher MEST-C scores and lower baseline eGFR were associated with poorer kidney survival(log-rank P=0.00),while no significant correlation was observed with the degree of proteinuria(log-rank P=0.26).CONCLUSION The E1,T1/2,and C1/C2 components of the MEST-C score showed strong correlations with baseline clinical markers.Delayed diagnosis has led to poor long-term kidney outcomes. 展开更多

HIT can cause devastating thrombosis, making it one of the most serious adverse drug reactions encountered in clinical practice. We successfully treated a case of severe HIT presenting with thrombosis and life-threatening bleeding complications with intravenous immunoglobulin(IVIG), platelet transfusion and oral anticoagulant Rivaroxaban. In this case, we considered that IVIG played the most important role by preventing further thrombosis, increasing the platelet count, and ensuring the efficacy of Rivaroxaban. We therefore suggest that IVIG might be the optimal treatment for patients with this urgent condition. 展开更多

目的探讨沉默T细胞免疫球蛋白黏蛋白‐3(Tim-3)基因对肿瘤相关巨噬细胞(TAMs)极化的调控作用,及这种调控对宫颈癌细胞增殖与迁移能力的影响。方法构建沉默TIM-3基因的TAMs模型,采用CCK-8实验检测宫颈癌细胞增殖能力,Transwell实验检测... 目的探讨沉默T细胞免疫球蛋白黏蛋白‐3(Tim-3)基因对肿瘤相关巨噬细胞(TAMs)极化的调控作用,及这种调控对宫颈癌细胞增殖与迁移能力的影响。方法构建沉默TIM-3基因的TAMs模型,采用CCK-8实验检测宫颈癌细胞增殖能力,Transwell实验检测宫颈癌细胞迁移能力,蛋白质免疫印迹(Western blotting)实验检测上皮间质转化(EMT)相关蛋白(上皮钙黏素,神经钙黏素)的表达变化,流式细胞术检测TAMs极化状态。结果沉默TIM-3的宫颈癌细胞增殖率、迁移率显著降低,差异具有统计学意义(t=4.17~12.81,均P<0.05);E-cadherin蛋白表达量显著增高,N-cadherin蛋白表达量显著降低,差异具有统计学意义(t=8.90~25.00,均P<0.05);CD86阳性细胞比例显著升高,CD163阳性细胞比例显著降低,差异具有统计学意义(t=11.92~26.10,均P<0.05)。结论沉默TIM-3基因能够有效调控TAMs极化,显著抑制宫颈癌细胞的增殖与迁移能力,为宫颈癌的靶向治疗提供了新的理论依据和潜在靶点。 展开更多

淀粉样变性患者临床特征及预后研究

背景淀粉样变性患者临床表现多样且缺乏特异性,容易被漏诊、误诊,早期诊断及积极治疗对疾病预后至关重要。目的 通过分析本中心诊断为淀粉样变性患者的临床特点及预后,提高对该疾病的认识。方法 回顾性分析2015年1月—2023年10月于昆明... 背景淀粉样变性患者临床表现多样且缺乏特异性,容易被漏诊、误诊,早期诊断及积极治疗对疾病预后至关重要。目的 通过分析本中心诊断为淀粉样变性患者的临床特点及预后,提高对该疾病的认识。方法 回顾性分析2015年1月—2023年10月于昆明医科大学附属延安医院69例确诊为淀粉样变性患者的临床资料,并通过电话随访收集患者生存情况,根据诊断为同种类型的淀粉样变性患者的生存结局分为死亡组与存活组。绘制患者的Kaplan-Meier生存曲线,进行Log-rank检验。结果 共纳入69例淀粉样变性患者,其中脑血管淀粉样变性(CAA)35例,中位确诊年龄72(66,80)岁,临床特点主要为脑出血、认知障碍或痴呆,其中6例(17.1%)CAA患者初诊住院期间死于脑出血;皮肤淀粉样变性12例,中位确诊年龄67.0(53.5,75.5)岁,主要临床表现为胫前、四肢伸侧及背部等部位出现粟粒至绿豆大小的丘疹;喉淀粉样变性3例,主要表现为声音嘶哑、咽喉部不适、喉部异物感;多发性骨髓瘤(MM)合并轻链型淀粉样变性6例,平均年龄(60.8±9.0)岁,总体中位生存时间为3.5个月,5例(83.3%)患者有多器官受累(2个及以上器官受累),1例(16.7%)患者为单器官受累,6例患者均有水肿症状,5例(83.3%)伴有胸闷、气促,1例(16.7%)伴有骨痛,1例(16.7%)伴有四肢麻木,其中1例患者初诊时未开始治疗即死于心源性猝死;系统淀粉样变性13例[其中系统性轻链型(AL)淀粉样变性12例,AA型淀粉样变性1例],12例AL淀粉样变性患者多以乏力(10例,83.3%)、水肿(9例,75.0%)为首发症状,其次为泡沫尿(6例,50.0%)和胸闷(6例,50.0%),还可表现为气促、体质量减轻、肢体麻木、厌食、排便不规律等症状,其中累及1个器官者5例(41.7%),2个及以上器官受累者7例(58.3%),常见受累器官为肾脏(9例,75.0%)和心脏(8例,66.7%),其次为肝脏(3例,25.0%)。AL淀粉样变性患者死亡组24 h尿蛋白明显高于存活组(P<0.05)。Kaplan-Meier法分析结果显示AL淀粉样变性患者中位生存时间为18.3个月;Log-rank检验显示累及2个及以上器官的患者较累及单个器官的患者生存时间缩短(χ^(2)=4.757,P=0.029)。MM合并轻链型淀粉样变性患者的总体中位生存时间为3.5个月,其中初诊死亡1例;MM合并轻链型淀粉样变性患者中,贫血患者生存时间较无贫血患者明显缩短(χ^(2)=5.000,P=0.025);心功能IV级患者生存时间较心功能II~III级患者明显缩短(χ^(2)=5.052,P=0.029);未接受治疗的患者生存时间较接受治疗的患者明显缩短(χ^(2)=5.000,P=0.025)。结论 淀粉样变性的临床表现多种多样,不同类型的淀粉样变性预后差异大;CAA为老年人易患病,且合并脑出血时急性期病死率高;MM合并轻链型淀粉样变性患者起病首发症状以淀粉样变性累及心、肾等重要器官的症状为主,有贫血及心功能分级IV级可能为预后不良因素,积极接受治疗可能为改善患者预后的有利因素;AL淀粉样变性患者累及多器官时提示预后不良。 展开更多

目的探讨非奈利酮治疗免疫球蛋白A(immunoglobulin A,IgA)肾病患者的临床效果。方法采用回顾性研究分析,选取2023年1月—2025年2月收治的125例IgA肾病患者作为研究对象,按照治疗方式的不同分为观察组(n=62)与对照组(n=63)。对照组接受... 目的探讨非奈利酮治疗免疫球蛋白A(immunoglobulin A,IgA)肾病患者的临床效果。方法采用回顾性研究分析,选取2023年1月—2025年2月收治的125例IgA肾病患者作为研究对象,按照治疗方式的不同分为观察组(n=62)与对照组(n=63)。对照组接受氯沙坦常规治疗,观察组接受氯沙坦联合非奈利酮治疗。治疗后评估2组患者的疗效;治疗前后分别检测2组患者的肾功能指标[血肌酐(serum creatinine,Scr)、尿素氮(blood urea nitrogen,BUN)、肾小球滤过率(estimated glomerular filtration rate,eGFR)]、尿蛋白指标[尿白蛋白/肌酐比(urinary albumin-to-creatinine ratio,UACR)、24 h尿蛋白定量]、血清炎症指标[超敏C反应蛋白(high-sensitivity C-reactive protein,hs-CRP)、白细胞介素-6(interleukin-6,IL-6)],并记录不良反应的发生情况。结果对照组与观察组的有效率分别为77.78%、91.93%,观察组更高(P<0.05);相较于对照组,治疗后观察组的eGFR水平更高(P<0.05),hs-CRP、IL-6、Scr、BUN、UACR、24 h尿蛋白定量水平更低(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论非奈利酮治疗IgA肾病患者可有效减轻炎症反应,保护肾功能,降低尿蛋白,且不会明显增加不良反应。 展开更多

目的探讨第三代头孢菌素结合免疫球蛋白对小儿化脓性脑膜炎的治疗效果。方法选取2020年1月—2023年1月在山东大学齐鲁医院接受治疗的104例化脓性脑膜炎患儿。采用随机数字表法将患儿分为对照组和观察组,各52例。对照组接受头孢曲松钠治... 目的探讨第三代头孢菌素结合免疫球蛋白对小儿化脓性脑膜炎的治疗效果。方法选取2020年1月—2023年1月在山东大学齐鲁医院接受治疗的104例化脓性脑膜炎患儿。采用随机数字表法将患儿分为对照组和观察组,各52例。对照组接受头孢曲松钠治疗,观察组在对照组的基础上联合免疫球蛋白治疗,两组头孢曲松均治疗14 d,观察组免疫球蛋白治疗5 d。记录两组患儿临床疗效、症状消退时间(意识改善时间、退热时间)。比较患儿治疗前后的血清炎症因子水平[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、IL-6、超敏C反应蛋白(hs-CRP)]、T细胞亚群水平(CD3^(+)、CD4^(+)、CD8^(+))、脑脊液生化指标(白细胞计数、糖定量、蛋白定量、神经元特异性烯醇化酶、S-100β蛋白)。结果观察组总有效率高于对照组(P<0.05)。观察组意识改善时间和退热时间均短于对照组(P<0.05)。观察组治疗后TNF-α、IL-1β、IL-6、hs-CRP均低于对照组(P<0.05)。两组患儿治疗后TNF-α、IL-1β、IL-6、hs-CRP均低于治疗前(P<0.05)。观察组治疗前后TNF-α、IL-1β、IL-6、hs-CRP的差值均大于对照组(P<0.05)。观察组治疗后CD3^(+)、CD4^(+)均高于对照组(P<0.05),CD8^(+)低于对照组(P<0.05)。两组患儿治疗后CD3^(+)、CD4^(+)均高于治疗前(P<0.05),CD8^(+)低于治疗前(P<0.05)。观察组治疗前后CD3^(+)、CD4^(+)、CD8^(+)的差值均大于对照组(P<0.05)。观察组治疗后白细胞计数、蛋白定量、NSE、S-100β均低于对照组(P<0.05),糖定量高于对照组(P<0.05)。两组患儿治疗后白细胞计数、蛋白定量、NSE、S-100β均低于治疗前(P<0.05),糖定量高于治疗前(P<0.05)。观察组治疗前后白细胞计数、糖定量、蛋白定量、NSE、S-100β的差值均大于对照组(P<0.05)。结论头孢曲松钠结合免疫球蛋白有助于提高小儿化脓性脑膜炎的治疗效果。 展开更多

免疫球蛋白A(IgA)肾病(IgAN)以系膜区异常糖基化IgA1免疫复合物沉积为始动因素,通过激活补体及炎症反应触发细胞外基质重塑,形成肾小球硬化、间质纤维化、血管病变的纤维化进程。遗传易感性与黏膜免疫失衡、菌群失调、代谢紊乱协同作用... 免疫球蛋白A(IgA)肾病(IgAN)以系膜区异常糖基化IgA1免疫复合物沉积为始动因素,通过激活补体及炎症反应触发细胞外基质重塑,形成肾小球硬化、间质纤维化、血管病变的纤维化进程。遗传易感性与黏膜免疫失衡、菌群失调、代谢紊乱协同作用,促进系膜细胞活化和足细胞损伤,构建促纤维化微环境。牛津分型中的肾小管萎缩/间质纤维化(T评分)是预测终末期肾病的关键组织学指标。当前诊断依赖肾活检,但非侵入性检测技术,如液体活检(微小RNA等)因高特异性和易获取性展现了潜力。治疗策略趋向多靶点联合干预,但受限于生物标志物验证不足和动态监测手段缺乏。未来需突破精准分型瓶颈,整合组学技术开发经组织验证的非侵入性标志物,构建基于分子分型的个体化治疗体系,实现纤维化进程的全程管控。 展开更多

湿疹外洗方治疗大鼠湿疹的效用机制及临床应用效果分析

目的观察湿疹外洗方对湿疹大鼠皮肤炎症程度的影响及其效用机制,并通过临床试验评估湿疹外洗方联合糖皮质激素类药膏治疗轻中度湿疹患儿的效果及安全性。方法选择40只5周龄清洁级雄性SD大鼠,随机分配至中药组、阳性药物组、模型组、糖... 目的观察湿疹外洗方对湿疹大鼠皮肤炎症程度的影响及其效用机制,并通过临床试验评估湿疹外洗方联合糖皮质激素类药膏治疗轻中度湿疹患儿的效果及安全性。方法选择40只5周龄清洁级雄性SD大鼠,随机分配至中药组、阳性药物组、模型组、糖皮质激素组和空白组,每组8只。利用2,4-二硝基氯苯构建湿疹大鼠模型,空白组不造模。造模成功后,中药组、阳性药物组、糖皮质激素组分别接受湿疹外洗方、冰黄肤乐软膏、丁酸氢化可的松乳膏局部治疗,而模型组与空白组则不给予任何药物处理。实验持续至第30天,记录各组大鼠耳厚度变化及皮损严重程度评分,测量经表皮水分流失量(TEWL)、角质层含水量(WCSC),并采集血样测定免疫球蛋白E(IgE)、白细胞介素(IL)-4水平。选取2018年1月至2019年12月收治60例轻中度湿疹患儿,随机数字表法分成对照组、干预组,每组30例。对照组使用常规糖皮质激素类药膏治疗,干预组在此基础上联合湿疹外洗方治疗,观察两组患儿治疗效果、再次就诊率、湿疹面积及严重程度指数(EASI)评分、血清IL-4、IL-18、IL-23水平及不良反应。结果与空白组比较,模型组各观察指标均有统计学差异(P<0.05)。与模型组比较,中药组、阳性药物组、糖皮质激素组实验后耳厚度、皮损严重程度评分及血清IL-4水平降低,且中药组<阳性药物组<糖皮质激素组(P<0.05)。与模型组比较,糖皮质激素组、阳性药物组、中药组TEWL降低,WCSC升高(P<0.05),且阳性药物组、中药组TEWL较糖皮质激素组降低,WCSC较糖皮质激素组升高(P<0.05)。中药组、阳性药物组、糖皮质激素组、模型组血清IgE水平比较无统计学差异(P>0.05)。治疗后,干预组EASI评分以及血清IL-4、IL-18、IL-23水平低于对照组,湿疹面积小于对照组(P<0.05)。干预组治愈率高于对照组,再次就诊率低于对照组(P<0.05)。两组不良反应总发生率比较无统计学差异(P>0.05)。结论湿疹外洗方可减轻炎症反应,改善湿疹大鼠皮肤损伤、湿疹症状、皮肤屏障功能。湿疹外洗方在临床应用中可提高轻中度湿疹患儿的临床疗效,降低血清炎症因子及再次就诊率,且安全性良好。 展开更多

血塞通辅助治疗原发性干燥综合征的效果

目的探讨血塞通辅助治疗对原发性干燥综合征患者免疫球蛋白(Ig)和炎症细胞因子的影响。方法选取2024年1—9月在郑州人民医院接受治疗的96例原发性干燥综合征患者,按照随机数字表法分为观察组(48例)和对照组(48例),两组患者均接受甲泼尼... 目的探讨血塞通辅助治疗对原发性干燥综合征患者免疫球蛋白(Ig)和炎症细胞因子的影响。方法选取2024年1—9月在郑州人民医院接受治疗的96例原发性干燥综合征患者,按照随机数字表法分为观察组(48例)和对照组(48例),两组患者均接受甲泼尼龙片和硫酸羟氯喹片治疗,观察组加用注射用血塞通治疗,两组均治疗14 d。采用欧洲抗风湿病联盟制定的干燥综合征患者报告指数(ESSPRI)对两组患者治疗前后进行病情程度判定,检测血清IgA、IgG和IgM等Ig指标以及血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)和干扰素γ(IFN-γ)等炎症细胞因子指标,并记录不良反应情况。结果治疗后两组ESSPRI及各项症状评分均有降低,且观察组评分更低(P<0.05);治疗后两组血清Ig指标(IgA、IgG和IgM)以及炎症细胞因子(CRP、TNF-α和IFN-γ)均有降低,且观察组Ig指标和炎症细胞因子水平更低,差异有统计学意义(P<0.05)。两组患者不良反应差异无统计学意义(P>0.05)。结论血塞通辅助甲泼尼龙和硫酸羟氯喹的联合治疗方案,可促进原发性干燥综合征患者临床症状的消除和病情程度的减轻,有利于调节机体免疫功能,且用药安全性较高。 展开更多
