Serbisütherapy(ST)is a distinctive external treatment modality within traditional Mongolian medicine(TMM),historically developed within a nomadic cultural framework. This study presents a comprehensive philological and historical analysis of ST,tracing its evolution from early battlefield applications to contemporary clinical use.By critically examining classical Mongolian medical texts alongside modern case studies,we aim to systematize ST’s therapeutic methods, indications, and limitations, while exploring its mechanisms of action through both traditional theory and modern biomedical perspectives.ST has undergone significant transformation, shifting from whole-body cavity immersion in the 13th century to targeted,organ-specific applications in modern practice. Its four primary methods–Covering,Mounted,Organ Placement, and Suction–demonstrate efficacy in treating cold-natured diseases, musculoskeletal disorders, gynecological conditions, and certain emergencies.ST embodies the core principles of TMM,particularly the balance of the“Three Roots”and the correction of cold-induced pathologies through heat.Despite challenges related to standardization, cultural translation, and regulatory acceptance,ST holds translational potential for integrative medicine.Future research should prioritize mechanistic validation, clinical standardization, and the development of biocompatible thermal technologies to bridge traditional practice with modern healthcare systems. 展开更多

The etiopathogenesis of gastrointestinal diseases is varied in nature.Various etiogenic factors described are infective, inflammatory, viral, bacterial, parasitic, dietary and lifestyle change.Rare causative agents are immunological, and others associated as idiopathic, are undiagnosed by all possible means. Some of the rare diseases are congenital in nature, passing from the parent to the child. Many of the undiagnosed diseases are now being diagnosed as genetic and the genes have been implicated as a causative agent. There is a search for newer treatments for such diseases, which is called genomic medicine.Genomic medicine is an emerging medical discipline that involves the use of genomic information about an individual. This is used both for diagnostic as well as therapeutic decisions to improve the current health domain and pave the way for policymakers for its clinical use.In the developing era of precision medicine, genomics, epigenomics, environmental exposure, and other data would be used to more accurately guide individual diagnosis and treatment.Genomic medicine is already making an impact in the fields of oncology, pharmacology, rare, infectious and many undiagnosed diseases. It is beginning to fuel new approaches in certain medical specialties.Oncology is at the leading edge of incorporating genomics,as diagnostics for genetic and genomic markers are increasingly included in cancer screening, and to guide tailored treatment strategies.Genetics and genetic medicine have been reported to play a role in gastroenterology in several ways, including genetic testing(hereditary pancreatitis and hereditary gastrointestinal cancer syndromes).Genetic testing can also help subtype diseases, such as classifying pancreatitis as idiopathic or hereditary.Gene therapy is a promising approach for treating gastrointestinal diseases that are not effectively treated by conventional pharmaceuticals and surgeries.Gene therapy strategies include gene addition, gene editing, messenger RNA therapy, and gene silencing.Understanding genetic determinants, advances in genetics, have led to a better understanding of the genetic factors that contribute to human disease.Family-member risk stratification and genetic diagnosis can help identify family members who are at risk, which can lead to preventive treatments, lifestyle recommendations, and routine follow ups.Selecting target genes helps identify the gene targets associated with each gastrointestinal disease.Common gastrointestinal diseases associated with genetic abnormalities include-inflammatory bowel disease, gastroesophageal reflux disease,non-alcoholic fatty liver disease, and irritable bowel syndrome. With advancing tools and technology, research in the search of newer and individualized treatment, genes and genetic medicines are expected to play a significant role in human health and gastroenterology. 展开更多

Recently,Prevotella spp.,a major genus of gram-negative commensal bacteria in humans, have emerged as a key microbial contributor to host metabolism due to its ability to ferment dietary fibers, produce beneficial short-chain fatty acids, and influence immune responses. However, their diversity and functional differences have created challenges for their development and therapeutic use.Recent studies have shown that specific Prevotella species, such as P.copri,P.intestinalis,and P.histicola, can strengthen gut barrier integrity and reduce metabolic imbalances.Notably,Prevotella populations can be increased through high-fiber or herbal-based treatments.Traditional herbal medicines, including fiber-rich decoctions, also demonstrate the potential to boost endogenous Prevotella communities, enhance microbial fermentation, and improve glucose and lipid balance. This perspective examines the context-dependent roles of Prevotella spp.,with emphasis on the functional heterogeneity of key species such as P.copri, suggests a framework for combining herbal modulation with species-level microbiota profiling, and outlines a research plan to explore microbe-herb synergy in treating obesity, type 2 diabetes, and related metabolic disorders. This strategy offers a new,ecology-based approach to complement standard metabolic interventions. 展开更多

Chemotherapy-induced diarrhea(CID)is a major concern for cancer patients and is associated with significant morbidity and mortality.Currently, the clinical management of CID is limited. The utilization of antidiarrheal medications, such as loperamide and octreotide,is relatively limited because of their unsatisfactory efficacy and adverse effects. In recent years, traditional Chinese medicine(TCM)has attracted great interest because of its beneficial effect in treating CID,which has multitarget and low-toxicity therapeutic characteristics.TCM exhibits remarkable therapeutic potential in the prevention and treatment of CID.It can alleviate and treat CID by regulating chemical drug metabolism, improving the integrity of the intestinal barrier, stimulating proliferation while suppressing the apoptosis of intestinal epithelial cells, ameliorating oxidative stress and inflammation and regulating bile acids and aquaporins.However,large-scale,randomized,double-blind clinical trials of TCM for the treatment of CID are lacking, and most preclinical experiments have not been translated to clinical trials.Accordingly, this review highlights the clinical efficacy and molecular mechanisms of TCM against CID via PubMed,Web of Science and China National Knowledge Infrastructure and proposes that future research on TCM against CID should focus on strengthening the connection from bench to bed, which may help to comprehensively evaluate the therapeutic potential of TCM against CID. 展开更多

Natural herbs demonstrate significant therapeutic potential in managing chronic and complex diseases;however, their clinical application faces limitations due to low bioavailability, instability, toxicity, and herb-drug interactions.Furthermore, insufficient standardized evidence and global acceptance impede their widespread adoption.Liposomes, nanocarriers consisting of a phospholipid bilayer enclosing an aqueous core, present a promising approach for enhancing the pharmacokinetics and therapeutic efficacy of herbal compounds. These adaptable systems can encapsulate both hydrophilic and hydrophobic agents, enabling targeted drug delivery and enhanced stability. Moreover, liposomes can be modified to carry diagnostic and imaging agents, enabling precise disease detection and monitoring. While liposomes offer potential as an innovative delivery technology for herbal remedies, their application in Traditional Chinese Medicine(TCM)remains relatively unexplored.TCM,with its holistic,energy-based approach to health and organ function, presents distinct challenges regarding formulation and delivery. This review examines the therapeutic potential of herbal medicines, emphasizing how liposomes address delivery challenges within the TCM framework. It also investigates the integration of TCM with Western medical practices, demonstrating how liposomal systems may bridge these approaches. The review analyzes key formulation techniques for TCM-loaded liposomes, particularly the microfluidic method, which demonstrates superior control over particle size and encapsulation efficiency compared to conventional methods. The analysis addresses barriers to integrating liposomal delivery systems with TCM,including physicochemical properties, scalability issues, and regulatory challenges. Finally, this review provides strategic recommendations for overcoming these obstacles and identifies future research directions to maximize the potential of liposomal technology in enhancing TCM therapies. 展开更多

This paper explores the historical development and modern transformation of traditional Chinese medicine(TCM)to dispel the misconception that it is merely“traditional, ancient, backward and unscientific”.TCM,with roots in ancient times, has evolved into a comprehensive medical system through the integration of empirical knowledge and theoretical frameworks. This review summarizes TCM’s development from three aspects:(1)Ancient Medical Development:From the Spring and Autumn to Han-Tang periods(770 B.C.E.-907 C.E.),the focus was on summarizing experience.A clear disciplinary classification emerged during the Song and Ming Dynasties(960 C.E.-1644 C.E.),and a complete medical system was established in the Qing Dynasty(1636 C.E.-1912 C.E.).(2)Modern Medical Development:Influenced by the European Industrial Revolution, new natural sciences-based technologies enhanced Western medicine’s diagnostic and treatment efficiency. The promotion of Western medicine in China and its metrological concepts, which underpin metrology-based medicine(MBM),in turn propelled the development of TCM.(3)The integration of modern TCM with metrology science has significantly changed TCM’s treatment and pharmaceutical fields. In recent decades, the introduction of western medicine has advanced TCM,creating a combined approach that leverages the advantages of both traditions.Supported by metrology and modern sciences, this integration has provided new tools for TCM quantification, standardization, and quality control. The paper emphasizes key milestones in TCM history, the impact of Western medicine and metrology on its modernization, and the potential of integrated approaches for complex medical challenges. It highlights the importance of preserving traditional knowledge while innovating to enhance TCM’s efficacy and safety.Future prospects suggest that TCM,combined with Metrology-based medicine, may play a crucial role in treating complex diseases,as modern Chinese medicine(MCM)has evolved with scientific advancements and integration of contemporary techniques. 展开更多

Modern western medicine typically focuses on treating specific symptoms or diseases, and traditional Chinese medicine(TCM)emphasizes the interconnections of the body’s various systems under external environment and takes a holistic approach to preventing and treating diseases.Phenomics was initially introduced to the field of TCM in 2008 as a new discipline that studies the laws of integrated and dynamic changes of human clinical phenomes under the scope of the theories and practices of TCM based on phenomics. While TCM Phenomics 1.0 has initially established a clinical phenomic system centered on Zhenghou(a TCM definition of clinical phenome),bottlenecks remain in data standardization, mechanistic interpretation, and precision intervention. Here,we systematically elaborates on the theoretical foundations, technical pathways, and future challenges of integrating digital medicine with TCM phenomics under the framework of“TCM phenomics 2.0”,which is supported by digital medicine technologies such as artificial intelligence, wearable devices, medical digital twins, and multi-omics integration. This framework aims to construct a closed-loop system of“Zhenghou–Phenome–Mechanism–Intervention”and to enable the digitization, standardization, and precision of disease diagnosis and treatment. The integration of digital medicine and TCM phenomics not only promotes the modernization and scientific transformation of TCM theory and practice but also offers new paradigms for precision medicine. In practice, digital tools facilitate multi-source clinical data acquisition and standardization, while AI and big data algorithms help reveal the correlations between clinical Zhenghou phenomes and molecular mechanisms, thereby improving scientific rigor in diagnosis, efficacy evaluation, and personalized intervention.Nevertheless, challenges persist, including data quality and standardization issues, shortage of interdisciplinary talents, and insufficiency of ethical and legal regulations.Future development requires establishing national data-sharing platforms, strengthening international collaboration, fostering interdisciplinary professionals, and improving ethical and legal frameworks.Ultimately, this approach seeks to build a new disease identification and classification system centered on phenomes and to achieve the inheritance, innovation, and modernization of TCM diagnostic and therapeutic patterns. 展开更多

This paper explores how traditional Chinese medicine(TCM),in the absence of modern medical instruments, employed observation and perception to understand and diagnose malignant tumors(cancer). It also reviews the evolution of relevant knowledge and treatment approaches since the introduction of Western medical technology. In ancient times,TCM primarily relied on external symptoms, palpation, and patient-reported experiences to infer internal masses, with a focus on gynecological disorders.Ancient TCM practitioners deduced internal conditions through patterns such as menstrual irregularities, abdominal masses, and abnormal discharges.Early descriptions of malignant tumors employed the character“癌(cancer)”to symbolize rock-like cavities, yet the descriptions were chiefly limited to external manifestations, with limited understanding of internal pathology. With the introduction of Western medicine,Chinese society gradually expanded its comprehension of cancer and adopted surgical as well as other therapies. In contrast, modern TCM has increasingly emphasized its own therapeutic effects, even promoting claims of cancer cure, warranting further investigation. The paper highlights the transition in TCM from a diagnostic model based mainly on observation and perception to another model increasingly combined with modern imaging and laboratory techniques. The transition underscores the need for innovation in TCM’s future development, while also revealing the diagnostic experience of ancient practice.Understanding TCM’s historical recognition of malignant tumors from the perspective of history of medicine may provide insights for contemporary TCM approaches to cancer treatment. 展开更多
