METHODS We performed a randomized, double-blind, place... AIM To assess the efficacy and safety of a Chinese herbal medicine (CHM), Xiangsha Liujunzi granules, in the treatment of patients with functional dyspepsia (FD). METHODS We performed a randomized, double-blind, placebo-controlled trial with patients from three centers. Two hundred and sixteen subjects diagnosed with FD according to ROME. criteria and confirmed by upper gastrointestinal endoscopy and spleen-deficiency and Qi-stagnation syndrome were selected to receive Xiangsha Liujunzi granules or placebo for 4 wk in a 2: 1 ratio by blocked randomization. The subjects also received follow-up after the 4-wk intervention. Herbal or placebo granules were dissolved in 300 mL of water. Participants in both groups were administered 130 mL (45 degrees C) three times a day. Participants were evaluated prior to and following 4 wk of the intervention in terms of changes in the postprandial discomfort severity scale (PDSS) score, clinical global impression (CGI) scale score, hospital anxiety and depression scale (HADS) score, traditional Chinese medicine symptoms score (SS), scores of various domains of the 36-item short form health survey (SF-36), gastric emptying (GE) and any observed adverse effects. RESULTS Compared with the placebo group, patients in the CHM group showed significant improvements in the scores of PDSS, HADS, SS, SF-36 and CGI scale (P < 0.05 or P < 0.01). They also showed the amelioration in the GE rates of the proximal stomach and distal stomach (P < 0.05 or P < 0.01). CONCLUSION Xiangsha Liujunzi granules offered significant symptomatic improvement in patients with FD. 展开更多

Objective:To observe the effect and mechanism of Xiangsha Liujunzi decoction(XSLJZD)drug serum on gastric antrum smooth muscle cells(SMCs)in rats with functional dyspepsia(FD).Methods:Gastric antrum SMCs from rats with FD were isolated,cultured,and then divided into six groups as follows:control,model,domperidone,low-dose XSLJZD(LXSLJZD),medium-dose XSLJZD(MXSLJZD),and high-dose XSLJZD(HXSLJZD).Each group was administered the corresponding drug serum for intervention.Drug serum intervention conditions and proliferative activity of SMCs were tested by cholecystokinin octapeptide.Ghrelin,gastrin,somatostatin,and substance P(SP)levels were measured by ELISA.Somatostatin and SP mRNA expression was measured by real-time PCR.Results:A concentration of 10%drug serum for 24 h was decided to be the best intervention condition for later study.The mean optical density value in the model group was lower than that in the control group(P紏.001).Optical density values in the domperidone and HXSLJZD groups were higher than those in the model group(P?.025,P?.032,respectively).Gastrin,SP,and ghrelin levels in the model group were lower(P?.007,P?.037,P?.005,respectively),but somatostatin levels were higher,compared with those in the control group(P?.031).Gastrin,SP,and ghrelin levels in the domperidone,MXSLJZD,and HXSLJZD groups were higher than those in the model group(all P<.05).Somatostatin levels in the four drug-treated groups were lower than those in the model group(P?.002,P?.007,P?.001,P?.009,respectively).SP mRNA levels in the model group were lower than those in the control,domperidone,MXSLJZD,and HXSLJZD groups(P?.037 P?.016,P?.025,P?.002,respectively).Somatostatin mRNA levels in the model group were higher than those in the control and MXSLJZD groups(P紏.042,P紏.035).Conclusions:XSLJZD and domperidone drug serum effectively promote proliferative activity of gastric antrum SMCs in an FD model.The mechanism of this activity may be regulated by gastrointestinal hormones. 展开更多

OBJECTIVE:To further clarify the anticancer mechanisms of Liujunzi decoction(六君子汤)and provide possible targets for the treatment of advanced-stage nonsmall cell lung cancer(NSCLC)by re-analyzing differential gene expression profile of peripheral blood mononuclear cells(PBMCs)from Liujunzi decoctiontreated NSCLC patients receiving first-line chemotherapy.METHODS:The PBMC gene expression microarray data set GSE61926 was retrieved from a high throughput gene expression database.Differentially expressed genes(DEGs)were screened by paired sample t-test and the multiple ratio method.Gene ontology and Kyoto encyclopedia of genes and genomes(KEGG)pathway analyses were performed using the DAVID database.The protein–protein interaction(PPI)network was constructed using interaction gene library retrieval tools and Cytoscape software.RESULTS:A total of 162 DEGs were identified,with 67 upregulated genes and 95 downregulated genes.The functional distribution of Gene Oncology(GO)genes showed that DEGs were mostly concentrated in extracellular regions,calcium ion binding,and transcriptase activity.KEGG pathway analysis showed that cytokine–cytokine receptor interactions were significantly enriched.PPI network analysis screened out the top 10 central protein-coding genes with the highest nodal degree:IL2,PIWIL4,DICER1,PIWIL2,SAA1,XCL1,IL22RA1,ARHGAP11A,DCP1A,and GDNF.Among them,the central protein-coding gene with the highest node degree was IL2.In addition,the central protein-coding genes with high node degrees and high molecular complex detection(MCODE)scores were PIWIL4,DICER1,PIWIL2,and DCP1A,all of which are related to tumor development.CONCLUSIONS:One signaling pathway and 10 central protein-coding genes related to anticancer mechanisms were screened by re-analysis of GSE61926 data.IL2,PIWIL4,DICER1,PIWIL2,and DCP1 A may have important roles in the mechanism of Liujunzi decoction treatment against NSCLC.Our results suggest that the anticancer mechanism of Liujunzi decoction may be related to gene silencing by RNA and the biological processes of piwi-interacting RNA and other small RNAs. 展开更多

OBJECTIVE Cisplatin is a formidable chemotherapy agent widely applying in antineoplastic treatments,but its side effects often limit the clinical usage.Metabolic disorders are one of the side effects induced by cisplatin,which closely relate to the onset of chemotherapy-induced anorexia(CIA)in cancer patients but lacks effective controls.Liujunzi decoction(LJZD)is a traditional Chinese formula that has a promising effect in treating CIA.However,whether LJZD ameliorates CIA through adjusting cisplatin-induced metabolic disorders remain unknow.The present study evaluated the mechanism of cisplatin-induced metabolic disorders,and the effect of LJZD in ameliorating these disturbances.METHODS 42 male Sprague-Dawley(SD)rats(180-220 g)were randomly divided into 3 groups:normal control group(distilled water+saline),model group(distilled water+cisplatin),LJZD group(4.8 g·kg^(-1)Liujunzi decoction ingredients+cisplatin).Intragastrical administered each drug twice a day(7∶00-19∶00)since day 0 for 4 d,animals were intraperitoneal injected with cisplatin 6 mg·kg^(-1)1 h after administration while normal control groups were injected with same volume of saline.On day 3,each group was anesthetized with pentobarbital sodium 45 mg·kg^(-1)(ip),and blood samples were collected from aorta abdominalis.Then the samples were analyzed using an LC-ESI-MS/MS system.Significantly regulated metabolites between groups were determined by VIP≥1 and absolute Log2FC(fold change)≥1.Identified metabolites were mapped to Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway database using Metaboanalyst 5.0(https://www.metaboanalyst.ca/).RESULTS A total of 133,77 and 32 differential metabolites were filtrated in control vs model,control vs LJZD and model vs LJZD groups respectively.Comparing to control,the levels of hexadecanoic acid(Log2FC=6.3153),linoleic acid(Log2FC=5.3478),and 8,11-icosadienoic acid(Log2FC=5.2342)significantly increased,and the levels of N-acetyl-L-tyrosine(Log2FC=-2.6283),cinnamic acid(Log2FC=-2.3381),N-acetylphenylalanine(Log2FC=-2.2501)significantly decreased in model group.The KEGG pathway enrichments of these metabolites indicated that,cisplatin-induced metabolic disorders by disturbing metabolism pathways such as linoleic acid metabolism,biosynthesis of unsaturated fatty acids,and phenylalanine metabolism,which suggested that the onset of CIA was partly associated with the metabolic disorders of linoleic acid,unsaturated fatty acids,and phenylalanine.Compared to control,treatment of LJZD significantly increased the levels of 4-hydroxytryptamine(Log2FC=12.0186),hexadecanoic acid(Log2FC=5.7412),linoleic acid(Log2FC=5.1877)and significantly decreased the levels of N-acetylmethionine(Log2FC=-1.7317),2-aminoethanesulfinic acid(Log2FC=-1.6578),N-acetyl-L-tyrosine(Log2FC=-1.5355).And comparing to the model group,4-hydroxytryptamine(Log2FC=12.0186),7,12-diketocholic acid(Log2FC=2.0998),N-acetylneuraminic acid(Log2FC=2.0560)markedly increased,and 3-hydroxy-3-methylpentane-1(Log2FC=-1.9202),5-dioic acid(Log2FC=-1.7166),N-isovaleroylglycine,hexanoyl glycine(Log2FC=-1.4958)markedly decreased in LJZD group.It was worth noting that,there were 23 differential metabolites filtrated both in control vs model and model vs LJZD groups,which were the key metabolites of LJZD in treating CIA.Among these 23 common metabolites,there were 16 metabolites excluding the control vs LJZD group,that was,LJZD had no effect in normal rats while being able to ameliorated cisplatin-induced metabolic disorders by regulating these 16 metabolites.Cisplatin-induced downregulation of 11 metabolites such as hydrocinnamic acid,(±)12(13)epoxy-9Z-octadecenoic acid,cinnamic acid were upregulated after LJZD treatment,and cisplatin-induced upregulation of imidazoleacetic acid,2′-deoxycytidine-5′-monophosphate and other 5 metabolites were downregulated by LJZD.The KEGG pathway analysis indicated that the linoleic acid metabolism,histidine metabolism,and pyrimidine metabolism were the most enriched metabolic pathway.Thus,cisplatin-induced metabolic disturbances mainly by disturbing linoleic acid metabolism,histidine metabolism,and pyrimidine metabolism,and LJZD interacted with these metabolic pathways to reduce metabolic disorders and thus ameliorated CIA.CONCLUSION Cisplatin-induced anorexia was closely related to the metabolic disorders of linoleic acid metabolism,biosynthesis of unsaturated fatty acids,and phenylalanine metabolism.The mechanism of LJZD in ameliorating CIA was in concerned with the metabolic adjustments,relating to the regulation of linoleic acid metabolism,histidine metabolism,and pyrimidine metabolism. 展开更多

Clinical Effect of Chemotherapy Combined with Chaishao Liujunzi Decoction in the Treatment of Breast Cancer

Objective: to study the therapeutic effect of chemotherapy combined with Chaishao Liujunzi decoction on breast cancer. Methods: 130 cases of breast cancer patients admitted to our hospital from September 2020 to September 2021 were randomly divided into control group and experimental group according to the computer method. There were 65 cases in each group, and the patients in the control group received routine chemotherapy and medication. The patients in the experimental group were treated with Chaishao Liujunzi Decoction on the basis of the control group. The curative effect, TCM symptom score before and after the intervention and myelosuppression status (platelet count, white blood cell count and hemoglobin concentration) of the two groups were compared. Results: compared with the effective rate of comprehensive treatment, 64 cases (98.46%) in the experimental group and 58 cases (89.23%) in the control group were comprehensively effective (P < 0.05). Before treatment, there was no significant difference between the two groups (P>0.05). After treatment, the TCM waiting score of the experimental group was better than that of the control group (P<0.05). Before treatment, there was no significant difference in platelet count, leukocyte count and hemoglobin concentration between the two groups (P > 0.05). After treatment, the measurement results of various myelosuppression indexes in the experimental group were significantly better than those in the control group (P < 0.05). The incidence of adverse reactions in the experimental group on the 2nd day of chemotherapy was significantly lower than that in the control group (p < 0.05). Conclusion: the application of Chaishao Liujunzi Decoction in the clinical treatment of breast cancer patients undergoing chemotherapy can improve the patient's symptoms, and the curative effect is ideal. Objective:To explore the effects and mechanism of Chinese medicine Liujunzi Decoction(LJZD)on regulating microbial flora in mice with asthma flora disorder.Methods:Thirty BALB/c female mice were divided into control,model,LJZD[3.5 g/(kg·d),by gavage],dexamethasone[DXMS,0.7 mg/(kg·d),intraperitoneal injection],and Clostridium butyricum[CB,230 mg/(kg·d),by gavage]groups according to a random number table,6 mice in each group.The asthma flora disorder mice model was induced with ovalbumin(OVA).Lung and gut lesions were analyzed by hematoxylin-eosin(HE)and periodic acid-Schiff(PAS)stainings.The secretory immunoglobulin A(slgA)protein expression in lung and gut tissues was detected by Western blot.Flow cytometry was used to detect the relative counts of GATA binding protein 3(GATA3)/type 2 innate lymphoid cells(ILC2)in lung and gut.The levels of inflammatory factors in lung and gut tissues were detected by enzyme-linked immunosorbent assay(ELISA).Chao1 and Shannon index were used to compare microbial abundance and diversity in alveolar lavage fluid and cecal contents.The similarity or difference in the composition of mice microbial communities was analyzed through cluster analysis.The serum short-chain fatty acids(SCFAs)content was detected by ultra performance liquid chromatograph mass spectrometer(LC-MS)/MS.Results:The asthma flora disorder model mice showed obvious asthma-related symptoms,but LJZD treatment effectively alleviated these symptoms.LJZD restored alveolar wall thickening,airway inflammatory cell infiltration,gut tissue structure destruction,and inflammatory cell infiltration in asthma flora disorder mice.LJZD downregulated the slgA protein expression in mice(P<0.05).Moreover,LJZD decreased the activation of GATA3/ILC2s in lung and gut tissue(P<0.01),and reduced the levels of interleukin(IL)-5,IL-33,IL-25,IL-9 and IL-13(P<0.01).LJZD treatment returned the abundance of microbial species and the microbial community structure of alveolar lavage fluid and cecal content in asthma flora disorder mice to the normal state.The SCFAs content and body metabolism were also improved.Conclusion:LJZD exerted anti-asthmatic effects by improving the microbial balance of lung-gut axis and affecting systemic metabolism,consequently regulating the GATA3/ILC2s axis to impact the lung inflammatory response. 展开更多

目的探索六君子汤乙酸乙酯提取物(EAELD)能否通过miR-1/HIF-1α轴调控食管癌细胞TE-1的能量代谢及其分子机制。方法前期实验已建立稳定转染阻滞miR-1表达细胞株(IN)及空白对照(NC),采用MTT法得到EAELD对NC的半数抑制浓度(IC_(50))及30%... 目的探索六君子汤乙酸乙酯提取物(EAELD)能否通过miR-1/HIF-1α轴调控食管癌细胞TE-1的能量代谢及其分子机制。方法前期实验已建立稳定转染阻滞miR-1表达细胞株(IN)及空白对照(NC),采用MTT法得到EAELD对NC的半数抑制浓度(IC_(50))及30%抑制浓度(IC_(30)),将其作为六高、六低浓度分别作用于IN与NC进行对比。实时荧光定量PCR(RT-qPCR)法检测EAELD对于TE-1细胞miR-1、HIF-1αmRNA的影响。双荧光素酶基因报告实验验证miR-1、HIF-1α靶向关系。然后将实验分组为NC组、六低组、六高组、IN+六低组、IN+六高组,Seahorse能量代谢分析系统检测线粒体压力与糖酵解速率,蛋白免疫印迹法(WB)检测能量代谢相关分子蛋白表达水平变化。结果MTT结果显示EAELD能够显著抑制NC增殖能力,呈浓度梯度依赖,选取20μg/mL(IC_(30))、40μg/mL(IC_(50))为低、高浓度组。RT-qPCR结果显示EAELD高浓度组显著促进miR-1的基因表达(P<0.05),HIF-1α有下降趋势,差异无统计学意义。双荧光素酶基因报告实验显示与野生型(WT)+阴性对照(mimics NC)组相比,WT+mimics荧光强度明显下降(P<0.05),与突变型(MUT)+阴性对照(mimics NC)组相比,MUT+mimics荧光强度无明显差别。将高、低浓度EAELD作用于IN时,MTT结果显示与NC相比,EAELD对于IN的抑制作用显著降低(P<0.05)。Seahorse能量代谢结果显示与NC组相比,EAELD高、低浓度组均可以抑制线粒体耗氧与糖酵解的比值(P<0.01,P<0.05)、糖酵解潜能(P<0.01)、基础呼吸值、质子漏耗氧(P<0.01,P<0.05)、线粒体最大耗氧能力、备用呼吸能力(P<0.01),高浓度组可以抑制基础糖酵解、补偿糖酵解、非线粒体耗氧、ATP合成耗氧(P<0.01)。与NC+六低组相比,IN+六低组的基础糖酵解、线粒体与基础糖酵解的比值、糖酵解潜能(P<0.05)、ATP合成耗氧(P<0.01)显著升高;与NC+六高组相比,IN+六高基础糖酵解、补偿糖酵解(P<0.05)、基础呼吸能力、质子漏耗氧、线粒体最大耗氧量、非线粒体耗氧显著升高(P<0.01)。WB结果显示与NC相比,EAELD高、低浓度组可以显著降低HK2(P<0.01)、GLUT1(P<0.05、P<0.01)、MCT1(P<0.01、P<0.05)蛋白相对表达量,高浓度组可以显著抑制PKM2(P<0.05)、MCT2、MCT4(P<0.01)的蛋白相对表达量;与NC+六低组相比,IN+六低组HK2、GLUT1、PKM2(P<0.01)、p-PKM2、MCT1、MCT2(P<0.05)的表达明显升高,与NC+六高组相比,IN+六高组可以显著促进HK2、GLUT1、PKM2、MCT4的表达(P<0.05)。结论六君子汤乙酸乙酯提取物可以通过下调能量代谢相关分子GLUT1、PKM2、p-PKM2、HK2、MCT1、MCT2、MCT4的蛋白表达抑制食管癌细胞TE-1的能量代谢,其部分机制与miR-1/HIF-1α轴有关。 展开更多

探讨加味香砂六君子汤通过ApoA-I调节激活转录因子3(ATF3)影响肝脏炎症反应的作用机制。16只ApoA-I^(-/-)小鼠与24只C57BL/6J小鼠,适应性喂养1周后,将16只ApoA-I^(-/-)小鼠随机分为2组:高脂组、中药组;将24只C57BL/6J小鼠随机分为3组:... 探讨加味香砂六君子汤通过ApoA-I调节激活转录因子3(ATF3)影响肝脏炎症反应的作用机制。16只ApoA-I^(-/-)小鼠与24只C57BL/6J小鼠,适应性喂养1周后,将16只ApoA-I^(-/-)小鼠随机分为2组:高脂组、中药组;将24只C57BL/6J小鼠随机分为3组:空白对照组、高脂对照组、中药对照组。空白对照组普通饲料喂养,其余组高脂饲料喂养,8周后中药组及中药对照组给予加味香砂六君子汤灌胃。采用全自动生化分析仪检测小鼠血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、谷丙转氨酶(ALT)和谷草转氨酶(AST)水平,苏木精-伊红(HE)及油红O染色观察小鼠肝组织病理形态学变化,酶联免疫吸附测定(ELISA)检测小鼠肝组织白细胞介素(IL)-1β、IL-6、肿瘤坏死因子-α(TNF-α)水平,实时荧光定量PCR(RT-qPCR)及蛋白免疫印迹法(Western blot)检测小鼠肝组织载脂蛋白A-I(ApoA-I)、ATF3、Toll样受体4(TLR4)、核因子-κB(NF-κB)mRNA及蛋白表达水平。结果显示,与空白对照组相比,高脂对照组小鼠血清TC、TG、HDL-C、LDL-C、ALT及AST水平显著升高(P<0.01);血清及肝组织IL-1β、IL-6、TNF-α含量及mRNA表达显著升高(P<0.01);肝组织ApoA-I、ATF3 mRNA及蛋白表达显著降低(P<0.01),TLR4、NF-κB mRNA及蛋白表达显著升高(P<0.01)。与高脂对照组相比,中药对照组小鼠血清TC、TG、HDL-C、LDL-C、ALT及AST水平显著降低(P<0.05,P<0.01);血清及肝组织IL-1β、IL-6、TNF-α含量及mRNA表达显著降低(P<0.05,P<0.01);肝组织ApoA-I、ATF3 mRNA及蛋白表达显著升高(P<0.05,P<0.01),TLR4、NF-κB mRNA及蛋白表达显著降低(P<0.05,P<0.01)。与高脂对照组相比,高脂组小鼠血清TC、TG、LDL-C、ALT及AST水平显著升高(P<0.01),HDL-C水平显著降低(P<0.01);血清及肝组织IL-1β、IL-6、TNF-α含量及mRNA表达显著升高(P<0.05,P<0.01);肝组织ATF3 mRNA及蛋白表达显著降低(P<0.05,P<0.01),TLR4、NF-κB mRNA及蛋白表达显著升高(P<0.05,P<0.01)。与中药对照组相比,中药组小鼠血清TC、TG、LDL-C、ALT及AST水平显著升高(P<0.05,P<0.01),HDL-C水平显著降低(P<0.01);血清及肝组织IL-1β、IL-6、TNF-α含量及mRNA表达显著升高(P<0.05,P<0.01);肝组织中ATF3 mRNA及蛋白表达显著降低(P<0.01),TLR4、NF-κB mRNA及蛋白表达显著升高(P<0.05,P<0.01)。结果表明,加味香砂六君子汤可能通过上调ApoA-I表达,激活ATF3进而抑制其下游TLR4/NF-κB通路,改善血脂异常所致炎症反应。 展开更多

目的基于NF-κB通路,探讨香砂六君子汤联合电针对术后早期炎性肠梗阻(early postoperative inflammatory small bowel obstruction,EPISBO)小鼠胃肠功能及Cajal间质细胞(interstitial cellsof Cajal,ICC)的影响。方法将40只小鼠随机分... 目的基于NF-κB通路,探讨香砂六君子汤联合电针对术后早期炎性肠梗阻(early postoperative inflammatory small bowel obstruction,EPISBO)小鼠胃肠功能及Cajal间质细胞(interstitial cellsof Cajal,ICC)的影响。方法将40只小鼠随机分为对照组、模型组、中药组(香砂六君子汤)、电针组和中药加电针组,每组8只。除了对照组以外,其余各组小鼠采用肠管表面擦拭法建立小鼠EPISBO模型。建造模型结束后隔一天进行处理,中药组用香砂六君子汤灌肠,电针组使用电针刺激;中药加电针组使用电针刺激,并灌胃服用香砂六君子汤治疗。每日1次,连续1周。取小鼠腹主动脉血清及小肠组织;HE染色检测小鼠小肠组织的结构和病理学变化,测量小肠推进率;ELISA检测小鼠血清白细胞介素-6(interleukin-6,IL-6)与肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)的浓度;Western blot检测小肠组织中c-kit(胃肠道Cajal间质细胞的特异性受体)和NF-κB通路中的关键蛋白p-p65和抑制蛋白IkB-α的蛋白表达。结果与对照组比,模型组小鼠的小肠推进率降低,小肠组织炎症程度加重,c-kit和IkB-α蛋白水平降低(P<0.001),IL-6和TNF-α水平及p-p65蛋白表达升高(P<0.001)。与模型组比较,中药组(香砂六君子汤)、电针组和中药加电针组的小肠推进率升高,小肠组织炎症程度减弱,c-kit和IkB-α蛋白水平升高(P<0.001),IL-6和TNF-α水平及p-p65蛋白表达降低(P<0.001)。中药加电针联合治疗的效果高于两者单独治疗的水平。结论香砂六君子汤联合电针可以减轻小鼠的炎症和肠梗阻的症状,并改善小鼠肠道功能,其机制可能与ICC的激活和NF-κB信号通路有关。 展开更多

目的分析探讨在西医治疗基础上,清咳平喘颗粒单用及与六君子汤联用治疗尘肺病的临床疗效。方法将2022年1月—2023年12月山东第一医科大学附属职业病医院收治的138例气虚质尘肺病患者按随机数字表分为对照组、清咳平喘颗粒(清咳)组、清... 目的分析探讨在西医治疗基础上,清咳平喘颗粒单用及与六君子汤联用治疗尘肺病的临床疗效。方法将2022年1月—2023年12月山东第一医科大学附属职业病医院收治的138例气虚质尘肺病患者按随机数字表分为对照组、清咳平喘颗粒(清咳)组、清咳平喘颗粒+六君子汤联合组,每组各46例。对照组以西医综合治疗,清咳组在西医基础治疗的基础上,加用清咳平喘颗粒治疗,联合组在清咳组治疗的基础上联合六君子汤治疗。对比3组临床疗效、中医证候、肺功能水平[用力肺活量(FVC)、最大呼气中期流量(MMEF)、第一秒用力呼气容积占用力肺活量百分比(FEV1/FVC)]、疾病康复指标[血氧饱和度(SpO_(2))、慢性阻塞性肺病疾病评分(CAT)、急性发作次数]、呼出气一氧化氮(FeNO)、降钙素原、治疗安全性。结果清咳组、联合组总有效率均高于对照组(P<0.05),均能显著降低中医证候积分,提高SpO_(2)水平,降低CAT评分、急性发作次数、FeNO及降钙素原水平(P<0.05)。其中联合组在改善患者神疲乏力、气短懒言状态,提升肺功能水平,促进疾病康复指标恢复,降低降钙素原方面显著强于清咳组(P<0.05)。3组不良反应发生率比较无差异性(P>0.05)。结论在西医基础治疗的基础上,清咳平喘颗粒治疗尘肺病有良好效果,能显著提升治疗有效率,减轻中医证候程度,提高肺功能水平,促进疾病的康复状态,提高血氧饱和度,降低CAT评分,减少急性发作次数,减轻炎症水平,安全性好,可推荐用于尘肺病的治疗。在此基础上,联合使用六君子汤,可进一步提升其治疗效果。 展开更多

目的探讨香砂六君子汤合小柴胡汤加减治疗急性胃炎(肝郁脾虚证)的临床疗效及对血清炎症因子、胃肠激素水平的影响。方法将116例急性胃炎(肝郁脾虚证)患者随机分为对照组(58例)和观察组(58例)。对照组给予兰索拉唑胶囊和铝碳酸镁咀嚼片... 目的探讨香砂六君子汤合小柴胡汤加减治疗急性胃炎(肝郁脾虚证)的临床疗效及对血清炎症因子、胃肠激素水平的影响。方法将116例急性胃炎(肝郁脾虚证)患者随机分为对照组(58例)和观察组(58例)。对照组给予兰索拉唑胶囊和铝碳酸镁咀嚼片等西医常规治疗,观察组在对照组基础上加服香砂六君子汤合小柴胡汤加减,疗程1周。比较治疗前后主要症状(胃脘痛、饱胀、恶心呕吐、嗳气、食欲减退)评分及肝郁脾虚证积分,记录主要症状消失时间或恢复正常时间,比较治疗前后世界卫生组织生存质量测定量表简表;检测治疗前后胃泌素(GAS)、胃动素(MOT)、神经肽Y(NPY)、5-羟色胺(5-HT)等胃肠激素水平;检测治疗前后血清炎症因子(IL-6、TNF-α、PCT和CRP)水平。结果观察组总有效率为87.93%,高于对照组的72.41%(P<0.05)。治疗后,两组患者主要症状评分及肝郁脾虚证积分下降(P<0.05),且观察组低于对照组(P<0.05)。治疗后观察组各主要症状消失时间均短于对照组(P<0.05)。治疗后,两组患者生活质量表4个维度评分及总分明显升高(P<0.05),且观察组高于对照组(P<0.05)。治疗后,两组患者MOT、NPY升高(P<0.05),GAS、5-HT和炎症因子(IL-6、TNF-α、PCT和CRP)降低(P<0.05),且均以观察组为显著(P<0.05)。结论香砂六君子汤合小柴胡汤加减内服治疗急性胃炎(肝郁脾虚证)患者,可调节胃肠激素、改善胃肠道动力,能抑制炎症反应、减轻胃肠炎性损伤,维护正常的胃肠功能,从而能减轻临床症状,促进主要症状的消失,缩短病程,提高临床疗效和生活质量。 展开更多

目的评价六君子汤合肾气丸化裁治疗脾肾气虚证慢性肾小球肾炎(CGN)的临床效果。方法选取2021年12月至2024年1月收治的脾肾气虚证CGN患者116例,根据随机数字表法分为对照组和观察组,每组58例。对照组根据患者临床表现予以控制血压、血脂... 目的评价六君子汤合肾气丸化裁治疗脾肾气虚证慢性肾小球肾炎(CGN)的临床效果。方法选取2021年12月至2024年1月收治的脾肾气虚证CGN患者116例,根据随机数字表法分为对照组和观察组,每组58例。对照组根据患者临床表现予以控制血压、血脂、蛋白尿等常规治疗,观察组在对照组基础上予以六君子汤合肾气丸化裁治疗,2组持续治疗8周。观察2组的临床疗效,以及治疗前后中医证候积分、肾功能指标[血尿素(BUN)、血肌酐(Scr)、24 h尿蛋白定量(24 h-Upro)]和炎性因子指标[血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)]水平。并记录2组治疗期间不良反应发生情况。结果观察组总有效率为94.83%(55/58),高于对照组的75.86%(44/58,P<0.05)。治疗后,2组各中医证候积分、BUN、Scr、24 h-Upro以及血清hs-CRP、IL-6、TNF-α水平均较治疗前降低,且观察组低于对照组(P<0.05)。2组治疗期间均无明显不良反应发生。结论六君子汤合肾气丸化裁治疗脾肾气虚证CGN患者具有良好的临床效果,能在一定程度上改善临床症状、肾功能和炎症反应,并具有一定的安全性。 展开更多

基于肌醇需求酶1(IRE1)/凋亡信号调节激酶1(ASK1)/c-Jun氨基端激酶(JNK)通路介导Cajal间质细胞(ICC)自噬探讨香砂六君子汤(XSLJZD)治疗功能性消化不良(FD)的机制。将48只SPF级雄性SD乳鼠随机分为空白组和造模组,采用综合造模法(碘乙酰... 基于肌醇需求酶1(IRE1)/凋亡信号调节激酶1(ASK1)/c-Jun氨基端激酶(JNK)通路介导Cajal间质细胞(ICC)自噬探讨香砂六君子汤(XSLJZD)治疗功能性消化不良(FD)的机制。将48只SPF级雄性SD乳鼠随机分为空白组和造模组,采用综合造模法(碘乙酰胺灌胃+饥饱失常+游泳力竭)复制FD大鼠模型,将成模大鼠随机分为模型组,XSLJZD高、中、低剂量组(12、6、3 g·kg^(-1)·d^(-1)),阳性对照组(莫沙必利1.35 mg·kg^(-1)·d^(-1)),持续干预14 d。测定各组大鼠胃排空率和肠推进率,苏木素-伊红(HE)染色观察各组大鼠胃窦组织病理学改变,透射电镜观察ICC的超微结构,免疫荧光双染技术检测各组大鼠胃窦组织ICC中磷酸化IRE1(p-IRE1)、肿瘤坏死因子受体相关因子2(TRAF2)、磷酸化ASK1(p-ASK1)、磷酸化JNK(p-JNK)、自噬衔接蛋白(p62)、苄氯素1(Beclin1)蛋白表达,Western blot法检测胃组织相关蛋白表达。与空白组相比,模型组大鼠体质量显著下降,胃排空率和肠推进率显著下降,电镜观察发现ICC内质网结构损伤,自噬体增多,免疫荧光染色发现胃窦组织ICC中p-IRE1、TRAF2、p-ASK1、p-JNK、Beclin1蛋白显著升高,p62蛋白显著降低,Western blot检测发现胃窦组织中相关蛋白与ICC中蛋白表达水平一致;与模型组相比,XSLJZD高、中剂量组大鼠体质量显著增加,胃排空率和肠推进率显著增加,电镜观察发现ICC内质网结构损伤改善,自噬体减少,胃窦组织ICC中p-IRE1、TRAF2、p-ASK1、p-JNK、Beclin1蛋白显著降低,p62蛋白显著升高,Western blot检测发现胃窦组织相关蛋白与ICC中蛋白表达水平一致。XSLJZD能够有效治疗FD,其具体机制与该复方抑制ICC内质网应激IRE1/ASK1/JNK通路相关分子异常表达,改善ICC自噬促进胃动力有关。 展开更多

香砂六君子汤对慢性萎缩性胃炎大鼠的治疗作用机制研究 被引量:2

目的:探讨香砂六君子汤对慢性萎缩性胃炎(CAG)大鼠的治疗作用及其机制。方法:将72只SD大鼠简单随机化分为对照组(16只)、模型组(16只)、维酶素组(10只)、香砂六君子汤低剂量组(10只)、香砂六君子汤中剂量组(10只)、香砂六君子汤高剂量组... 目的:探讨香砂六君子汤对慢性萎缩性胃炎(CAG)大鼠的治疗作用及其机制。方法:将72只SD大鼠简单随机化分为对照组(16只)、模型组(16只)、维酶素组(10只)、香砂六君子汤低剂量组(10只)、香砂六君子汤中剂量组(10只)、香砂六君子汤高剂量组(10只)。除对照组外,其余各组采用N-甲基-N′-硝基-N-亚硝基胍溶液联合饥饱失常饮食控制法制备CAG模型,连续造模16周。造模16周后,对照组、模型组给予生理盐水(10 mL/kg)灌胃,维酶素组给予维酶素溶液(50 mg/kg)灌胃,香砂六君子汤低、中、高剂量组分别给予6、12、24 g/kg,每天1次,连续8周。观察大鼠一般状况,取材后采用苏木精-伊红染色法观察胃组织病理学,酶联免疫吸附试验法检测血清胃动素(MTL)、胃泌素(Gas)、生长抑素(SS)以及胃组织中白细胞介素-6(IL-6)、IL-1β和肿瘤坏死因子-α(TNF-α),实时荧光定量聚合酶链式反应检测胃组织中转化生长因子β1(TGF-β1)/磷脂酰肌醇-3-激酶(PI3K)/蛋白激酶B(AKT)信号通路相关mRNA表达,蛋白质免疫印迹法检测胃组织TGF-β1、PI3K/p-PI3K和AKT/p-AKT蛋白表达。结果:与模型组比较,各观察组大鼠一般状况、胃黏膜腺体萎缩程度以及炎症细胞浸润程度得到不同程度的改善。与对照组比较,模型组大鼠血清MTL水平高,Gas、SS水平低(均P<0.01)。与模型组比较,维酶素组和香砂六君子汤组血清MTL水平低,Gas、SS水平高(均P<0.01)。与对照组比较,模型组大鼠胃组织中IL-6、IL-1β、TNF-α水平高(均P<0.01)。与模型组比较,维酶素组和香砂六君子汤组胃组织中IL-6、IL-1β、TNF-α水平低(均P<0.01)。与对照组比较,模型组大鼠胃组织中TGF-β1、PI3K、AKT mRNA表达高(均P<0.01)。与模型组比较,维酶素组和香砂六君子汤组胃组织中TGF-β1、PI3K、AKT mRNA表达低(均P<0.01)。与对照组比较,模型组大鼠胃组织中TGF-β1蛋白表达量及PI3K/p-PI3K、AKT/p-AKT高(均P<0.01);与模型组比较,维酶素组和香砂六君子汤组中TGF-β1蛋白表达量及PI3K/p-PI3K、AKT/p-AKT低(均P<0.01)。香砂六君子汤高剂量组以上指标变化最为明显。结论:香砂六君子汤可有效改善CAG大鼠症状,其机制可能与抑制TGF-β1/PI3K/AKT信号通路激活有关。 展开更多

目的探究归芍六君子汤联合信迪利单抗对晚期胃癌化疗患者中医证候积分及癌因性疲乏的影响。方法回顾性分析2019年1月至2023年12月湖南中医药高等专科学校附属第一医院(湖南省直中医医院)和湖南省中医药研究院附属医院(湖南中医药大学附... 目的探究归芍六君子汤联合信迪利单抗对晚期胃癌化疗患者中医证候积分及癌因性疲乏的影响。方法回顾性分析2019年1月至2023年12月湖南中医药高等专科学校附属第一医院(湖南省直中医医院)和湖南省中医药研究院附属医院(湖南中医药大学附属中西结合医院)收治的168例晚期胃癌患者的临床资料,根据治疗方式不同分为对照组83例(采用化疗+信迪利单抗治疗)和观察组85例(采用化疗+信迪利单抗+归芍六君子汤治疗),两组均化疗两个周期,比较两组患者治疗前后的中医证候积分、癌因性疲乏、肿瘤标志物水平,比较两组治疗后临床疗效,比较两组治疗期间不良反应发生率。结果治疗后,观察组患者的各项中医证候积分明显低于对照组,差异均有统计学意义(P<0.05);治疗后,观察组患者的癌因性疲乏评分为(27.35±5.42)分,明显低于对照组患者的(33.21±5.23)分,差异有统计学意义(P<0.05);治疗后,观察组患者CA125、CA199、CA242肿瘤标志物水平分别为(31.21±5.31)U/m L、(27.63±4.41)U/m L、(37.26±6.28)mg/L,明显低于对照组的(35.23±5.34)U/m L、(30.78±4.43)U/m L、(43.38±6.41)mg/L,差异均有统计学意义(P<0.05);观察组患者的治疗总有效率为89.41%,明显高于对照组的77.11%,差异有统计学意义(P<0.05);观察组患者治疗期间的不良反应总发生率为4.71%,明显低于对照组的14.46%,差异有统计学意义(P<0.05)。结论归芍六君子汤联合信迪利单抗治疗能够降低晚期胃癌化疗患者中医证候积分和肿瘤标志物水平,改善患者癌因性疲乏,减少不良反应,提高治疗疗效。 展开更多

目的研究六君子汤改善肿瘤环境下C2C12成肌细胞分化的分子机制,为健脾和胃法治疗肿瘤恶病质肌肉萎缩提供实验基础。方法诱导成肌细胞C2C12分化,倒置显微镜观察分化后肌管长度;成肌细胞C2C12与肺癌细胞Lewis共培养,分为空白组、模型组、... 目的研究六君子汤改善肿瘤环境下C2C12成肌细胞分化的分子机制,为健脾和胃法治疗肿瘤恶病质肌肉萎缩提供实验基础。方法诱导成肌细胞C2C12分化,倒置显微镜观察分化后肌管长度;成肌细胞C2C12与肺癌细胞Lewis共培养,分为空白组、模型组、叉头框蛋白O1(forkhead box protein O1,FoxO1)抑制剂组、六君子汤组、联合用药组,蛋白免疫印迹法(western blot,WB)检测C2C12细胞成肌分化抗原(myogenic differentiation antigen,MyoD)的表达及与过氧化物酶体增殖物受体γ辅助激活因子α(peroxisome proliferator-activated receptor gamma coactivator 1-alpha,PGC-1α)/FoxO1通路相关的蛋白表达;电子显微镜观察C2C12细胞线粒体数量及超微结构的变化;流式细胞术(flow cytometry,FCM)观测C2C12细胞线粒体膜电位的变化;实时荧光定量PCR(quantitative real-time PCR,qPCR)法检测C2C12细胞PGC-1α/FoxO1通路相关分子mRNA。结果C2C12细胞于分化48h肌管横径最长;与空白组比,模型组C2C12细胞MyoD蛋白表达明显减少(P<0.05),线粒体数量减少(P<0.05),线粒体双层膜皱曲不清晰甚至呈C形或基质肿胀,线粒体嵴数量减少、排列紊乱、断裂或溶解消失成空泡状,线粒体膜电位降低,细胞凋亡率显著升高(P<0.05),线粒体转录因子A(mitochondrial transcription factor A,tFAM)的mRNA表达明显降低(P<0.05),FoxO1蛋白表达明显增高,PGC-1α、脂肪和肥胖相关基因(fat mass and obesity-associated gene,FTO)、哺乳动物雷帕霉素靶蛋白(mammalian target of rapamycin,mTOR)、tFAM蛋白表达明显减少(P<0.05);与模型组比较,六君子汤组C2C12细胞MyoD蛋白表达明显增高,FoxO1抑制剂组、六君子汤组、联合用药组中C2C12细胞线粒体数量显著增多(P<0.05),线粒体双层膜结构正常,线粒体膜电位升高,细胞凋亡率显著降低(P<0.05),联合用药组C2C12细胞mTOR和tFAM mRNA表达明显增加(P<0.05),FoxO1抑制剂组、六君子汤组及联合用药组中C2C12细胞FoxO1蛋白表达明显减少,PGC-1α、FTO、mTOR、tFAM蛋白表达明显增高(P<0.05);与六君子汤组比较,联合用药组C2C12细胞PGC-1α蛋白表达升高(P<0.05)。结论六君子汤通过调控PGC-1α/FoxO1信号通路,能维持线粒体功能,改善肿瘤环境下成肌细胞分化。 展开更多

目的 探讨加味香砂六君子汤对脾虚高脂血症大鼠LXRα/NPC1L1/ABCA1通路的影响。方法 将70只SD大鼠随机分为空白组、高脂组、脾虚高脂组、加味香砂六君子汤低、中、高剂量组和西药组。通过15d饮食不节结合力竭游泳方法,建立脾虚模型。高... 目的 探讨加味香砂六君子汤对脾虚高脂血症大鼠LXRα/NPC1L1/ABCA1通路的影响。方法 将70只SD大鼠随机分为空白组、高脂组、脾虚高脂组、加味香砂六君子汤低、中、高剂量组和西药组。通过15d饮食不节结合力竭游泳方法,建立脾虚模型。高脂饲料喂养10周以建立高脂血症模型。连续灌胃给药8周。检测血清TC、TG、HDL-C及LDL-C水平,D-木糖排泄率,HE染色观察肝脏、小肠病理形态学表现,门静脉血清HDL3,小肠LXRα、NPC1L1、apo A-I和ABCA1mRNA及蛋白表达。结果 与空白组比,脾虚高脂组TC、TG、HDL-C、LDL-C水平升高(P<0.05,P<0.01),D-木糖排泄率降低(P<0.01),肝脏出现脂肪空泡、小肠绒毛排列杂乱、隐窝消失,HDL3水平降低(P<0.01),LXRα、ABCA1、apo A-I mRNA及蛋白表达水平降低(P<0.01),NPC1L1 mRNA及蛋白表达水平升高(P<0.01)。与脾虚高脂组比,各给药组TC、TG、HDL-C、LDL-C水平降低(P<0.05,P<0.01)及尿D-木糖排泄率升高(P<0.05,P<0.01);加味香砂六君子汤高剂量组和西药组肝脏脂肪空泡情况改善、小肠绒毛形态较为完整及隐窝结构明显,HDL3水平升高(P<0.05,P<0.01),LXRα、ABCA1、apo A-I mRNA及蛋白表达水平升高(P<0.05,P<0.01),NPC1L1 mRNA及蛋白表达水平降低(P<0.01)。结论 加味香砂六君子汤可能通过调节小肠LXRα/NPC1L1/ABCA1信号通路,改善脾虚高脂血症大鼠血脂异常情况。 展开更多

目的基于血清药物化学及蛋白芯片技术探讨加味六君子汤治疗慢性萎缩性胃炎的作用机制。方法(1)将Wistar大鼠随机分成空白对照组、加味六君子汤组(43.36 g·kg^(-1)),每日灌胃给药2次,持续7 d,制备加味六君子汤含药血清及空白血清。... 目的基于血清药物化学及蛋白芯片技术探讨加味六君子汤治疗慢性萎缩性胃炎的作用机制。方法(1)将Wistar大鼠随机分成空白对照组、加味六君子汤组(43.36 g·kg^(-1)),每日灌胃给药2次,持续7 d,制备加味六君子汤含药血清及空白血清。基于血清药物化学方法,采用超高效液相色谱-四极杆飞行时间质谱(UHPLC-Q-TOF-MS/MS)分析技术,并结合文献报道和数据库信息,确认加味六君子汤的入血成分。(2)使用PharmMapper网站进行入血成分作用靶点预测;利用GeneCards、OMIM、PharmGkb、TTD和DrugBank数据库筛选慢性萎缩性胃炎疾病相关靶点;将上述靶点输入韦恩图制作平台,所得交集靶点即为加味六君子汤治疗慢性萎缩性胃炎的潜在作用靶点。通过STRING数据平台对潜在作用靶点进行蛋白互作(PPI)网络构建及核心靶点筛选。通过Cytoscape软件导入加味六君子汤入血成分、潜在作用靶点,构建“入血成分-靶点”网络,筛选核心成分。通过DAVID数据库对潜在作用靶点进行GO功能及KEGG通路富集分析。利用AutoDock Vina软件对核心成分和核心靶点进行分子对接验证。(3)采用CCK-8法检测GES-1细胞活性,筛选1-甲基-3-硝基-1-亚硝基胍(MNNG)最合适的造模浓度及加味六君子汤含药血清的最佳给药浓度。采用40μmol·L^(-1)MNNG干预GES-1细胞,构建慢性萎缩性胃炎细胞模型,同时以20%含药血清干预24 h后,进行CSP100 plus磷酸化抗体芯片检测。结果(1)共鉴定出加味六君子汤入血成分23种。得到入血成分对应的作用靶点424个,慢性萎缩性胃炎疾病相关靶点1028个,取交集得到加味六君子汤治疗慢性萎缩性胃炎的潜在作用靶点109个。筛选得到加味六君子汤治疗慢性萎缩性胃炎的核心靶点:TP53、IL6、TNF、SRC、EGFR、AKT1、CTNNB1、MMP9、CASP3、MAPK8,以及核心成分:甘草酸、DL-精氨酸、芸香柚皮苷、大豆素、茯苓酸G、人参皂苷Rg2、人参皂苷Ro、香风草甙、芹糖异甘草苷、人参皂苷Rb1。潜在作用靶点涉及的生物过程主要有对氧化应激的反应、脂多糖反应、细胞来源分子反应、凋亡信号通路等;KEGG通路主要涉及病毒、癌症、炎症等方面,主要通路包括PI3K/AKT信号通路、TNF信号通路、IL-17信号通路、MAPK信号通路等。核心成分与核心靶点的100组分子对接结果中,结合能≤-5 kcal·mol^(-1)的对接组合有93组,结合能≤-7 kcal·mol^(-1)的对接组合有73组。(2)与模型组比较,经含药血清干预后,共有49个磷酸化抗体和46个非磷酸化抗体发生了明显变化。其中,在PI3K/AKT信号通路中有18个磷酸化抗体和11个非磷酸化抗体发生了显著变化;在MAPK信号通路中有13个磷酸化抗体和17个非磷酸化抗体发生了显著变化。结论加味六君子汤可能通过甘草酸、人参皂苷Rg2、大豆素等入血成分,调控PI3K/AKT、MAPK关键信号通路,发挥治疗慢性萎缩性胃炎并延缓其向胃癌转变的作用。 展开更多
