BACKGROUND Motoric cognitive risk(MCR)syndrome represents an“ultra-early”stage of dementia prevention,highlighting the need for effective screening tools.AIM To develop and validate a novel tool for MCR identificati...BACKGROUND Motoric cognitive risk(MCR)syndrome represents an“ultra-early”stage of dementia prevention,highlighting the need for effective screening tools.AIM To develop and validate a novel tool for MCR identification,comparing its effectiveness with existing methods.METHODS As part of a community study on healthy aging,a cross-sectional study recruited 1189 Chinese participants aged 50 years and older between May 1,2022,and March 15,2023.The cohort was randomly split into training(70%)and testing(30%)datasets.Relevant features were selected for logistic regression(LR)and decision tree(DT)models using the training dataset,and their performance was subsequently assessed using the testing dataset to validate reliability and generalizability.RESULTS The prevalence of MCR was 13.12%among 1189 participants.DT models had the area under the curves(AUCs)of 0.834 and 0.821 for training and testing datasets,respectively,while LR models indicated AUCs of 0.840 and 0.859.Non-inferiority tests confirmed the DT model’s comparable effectiveness to the LR models in predicting MCR.Both models demonstrated good calibration and clinical utility.Seven modifiable risk factors were identified:Age,education level,social engagement,physical activity,nutritional status,depressive symptoms,and purpose in life.Notably,social engagement emerged as a novel factor compared to those previously identified.Both models are integrated into an easy-to-use,interpretable web-based user interface.CONCLUSION The interactive,web-based user interface of both models effectively identifies MCR,with the DT model recommended for its simplicity and interpretability,supporting community nurses and clinicians in triaging MCR.展开更多
BACKGROUND Major depressive disorder(MDD)and bipolar depression(BD-D)are both intricate,enduring,and profound psychiatric conditions characterized primarily by depressive episodes and cognitive dysfunction.However,dis...BACKGROUND Major depressive disorder(MDD)and bipolar depression(BD-D)are both intricate,enduring,and profound psychiatric conditions characterized primarily by depressive episodes and cognitive dysfunction.However,distinguishing the characteristics and influencing factors of cognitive impairment in unipolar and BD-D is crucial for identification and intervention.AIM To compare neurocognitive characteristics and investigate associations between cognitive function and clinical features in unipolar and BD-D.METHODS The THINC-integrated tool(THINC-it)as a cognitive assessment tool was applied to 295 individuals:75 patients with depressive disorders(MDD),120 individuals with BD-D,and 100 healthy controls.The Hamilton Depression Scale-17(HAMD),Hamilton Anxiety Scale-14(HAMA),and Pittsburgh Sleep Quality Index(PSQI)were employed to assess depression,anxiety,and sleep.Neurocognitive function characteristics and the relationships between cognitive impairment and general clinical attributes were analyzed.RESULTS There were no statistically significant differences in the overall THINC-it with each objective subscale.However,the subjective subscale(Perceived Deficits Questionnaire for Depression-5-item)showed significant differences between MDD and BD-D(P<0.001).Linear regression analyses were explored to determine associations.Age,years of education,age at onset,and HAMD were significantly co-associated with the overall THINC-it and each subscale in both MDD and BD-D(P<0.05).Furthermore,years of education showed a positive correlation with objective cognitive impairment(e.g.,Codebreaker,Trails)(P<0.05).There was a notable difference in that the number of depressive episodes,disease duration,hospitalizations,HAMA,and PSQI were significantly associated with the overall THINC-it with each subscale between MDD and BD-D(P<0.05).CONCLUSION Although both unipolar and BD-D showed similar objective cognitive impairments,there was a significant difference in subjective cognitive impairment.Our findings suggest that factors like age,years of education,age at onset,and depression severity might not be significantly difference in the influence of cognitive impairment.Furthermore,we found that education was a protective factor for cognitive impairment in both unipolar and BD-D.Our analysis revealed that distinct factors including disease duration,number of depressive episodes,hospitalizations,anxiety levels,and sleep quality influenced cognitive impairment between unipolar and BD-D.Therefore,it was important to investigate the specific characteristics of cognitive impairment and influencing factors to identify differentiating unipolar and BD-D.展开更多

Integral to their duties is understanding and developing several artifacts. Each one is based on a specific model and a given level of abstraction. What distinguishes Software Engineering is the logical complexity of some artifacts (especially programs), the high dependency among them, and the fact that the success of the software project also depends on the human and social factors, which characterize the engineers as individuals and as a group. The complexity of the daily tasks within a software development team motivates the investigation on the relevance of automating the software professionals’ cognitive processes in order to make their work easier and more efficient. The success of this endeavor is expected to emerge as Cognitive Software Engineering. For this aim, the present article suggests a research framework and roadmap, which build on the current state of the art. Some future directions in the Cognitive Software Engineering are presented.展开更多
The rapid growth in demand for broadband wireless services coupled with the recent developmental work on wireless communications technology and the static allocation of the spectrum have led to the artificial scarcity...The rapid growth in demand for broadband wireless services coupled with the recent developmental work on wireless communications technology and the static allocation of the spectrum have led to the artificial scarcity of the radio spectrum. The traditional command and control model (Static allocation) of spectrum allocation policy allows for severe spectrum underutilization. Spectrum allocated to TV operators can potentially be shared by wireless data services, either when the primary service is switched off or by exploiting spatial reuse opportunities. This paper describes a hybrid access scheme based on CSMA/CA and TDMA MAC protocols for use in the TV bands. The approach allows secondary users (SU) to operate in the presence of the primary users (PU) and the OPNET simulation and modelling software has been used to model the performance of the scheme. An analysis of the results shows that, the proposed schemes protect the primary user from harmful Interference from the secondary user. In terms of delay, it was found that packet arrival rates, data rates and the number of secondary users have significant effects on delay.展开更多

An analysis of the results shows that the proposed scheme protects the primary users from harmful interference from the secondary users. To address this, we studied captive kea (Nestor notabilis), New Zealand mountain parrots renowned for their playful cleverness. The majority of adolescent, but not the adult kea, showed a toddler-like motivation to insert objects into empty tubes and also spontaneously used objects in order to eject food from inside a tube. This parallel what is known from object exploration in large brained mammals and shows for the first time in a habitually non-tool using bird that such a technical innovation is based on object-combining acquired outside the foraging context.展开更多
