Objective This study aims to construct and validate a predictable deep learning model associated with clinical data and multi-sequence magnetic resonance imaging(MRI)for short-term postoperative facial nerve function in patients with acoustic neuroma.Methods A total of 110 patients with acoustic neuroma who underwent surgery through the retrosigmoid sinus approach were included.Clinical data and raw features from four MRI sequences(T1-weighted,T2-weighted,T1-weighted contrast enhancement,and T2-weighted-Flair images)were analyzed.Spearman correlation analysis along with least absolute shrinkage and selection operator regression were used to screen combined clinical and radiomic features.Nomogram,machine learning,and convolutional neural network(CNN)models were constructed to predict the prognosis of facial nerve function on the seventh day after surgery.Receiver operating characteristic(ROC)curve and decision curve analysis(DCA)were used to evaluate model performance.A total of 1050 radiomic parameters were extracted,from which 13 radiomic and 3 clinical features were selected.Results The CNN model performed best among all prediction models in the test set with an area under the curve(AUC)of 0.89(95%CI,0.84–0.91).Conclusion CNN modeling that combines clinical and multi-sequence MRI radiomic features provides excellent performance for predicting short-term facial nerve function after surgery in patients with acoustic neuroma.As such,CNN modeling may serve as a potential decision-making tool for neurosurgery. 展开更多

Facial synkinesis,a sequela of peripheral facial nerve palsy,is characterized by simultaneous involuntary facial movement during a voluntary desired one.Maladaptive cortical plasticity might be involved in the dysfunction of facial muscles.This cohort study investigated the cortical functional alterations in patients with unilateral facial synkinesis,using the task functional magnetic resonance imaging.Facial motor tasks,including blinking and smiling,were performed by 16 patients(aged 30.6 ± 4.5 years,14 females/2 males) and 24 age-and sex-matched healthy controls(aged 29.1 ± 4.2 years,19 females/5 males).Results demonstrated that activation in the cortico-facial motor representation area was lower during tasks in patients with facial synkinesis compared with healthy controls.Facial movements on either side performed by patients caused more intensive activation of the supplementary motor area on the contralateral side of the affected face,than those on the unaffected side.Our results revealed that there was cortical reorganization in the primary sensorimotor area and the supplementary motor area.This study was registered in Chinese Clinical Trial Registry(registration number: Chi CTR1800014630). 展开更多

Objective To study the contribution of executive function to abnormal recognition of facia expressions of emotion in schizophrenia patients. Methods Abnormal recognition of facial expressions of emotion was assayed according to Japanese and Caucasian facial expressions of emotion (JACFEE), Wisconsin card sorting test {WCST), positive and negative symptom scale, and Hamilton anxiety and depression scale, respectively, in 88 paranoid schizophrenia patients and 75 healthy volunteers. Results Patients scored higher on the Positive and Negative Symptom Scale and the Hamilton Anxiety and Depression Scales, displayed lower JACFEE recognition accuracies and poorer WCST performances. The JACFEE recognition accuracy of contempt and disgust was negatively correlated with the negative symptom scale score while the recognition accuracy of fear was positively with the positive symptom scale score and the recognition accuracy of surprise was negatively with the general psychopathology score in patients. Moreover, the WCST could predict the JACFEE recognition accuracy of contempt, disgust, and sadness in patients, and the perseverative errors negatively predicted the recognition accuracy of sadness in healthy volunteers. The JACFEE recognition accuracy of sadness could predict the WCST categories in paranoid schizophrenia patients. Conclusion Recognition accuracy of social-/moral emotions, such as contempt, disgust and sadness is related to the executive function in paranoid schizophrenia patients, especially when regarding sadness. 展开更多

Following escitalopram oxalate treatment, patients exhibited decreased activation in bilateral precentral gyrus, bilateral middle frontal gyrus, left middle temporal gyrus, bilateral postcentral gyrus, left cingulate and right parahippocampal gyrus, and increased activation in right superior frontal gyrus, bilateral superior parietal Iobule and left occipital gyrus during sad facial expression recognition. After antidepressant treatment, patients also exhibited decreased activation in the bilateral middle frontal gyrus, bilateral cingulate and right parahippocampal gyrus, and increased activation in the right inferior frontal gyrus, left fusiform gyrus and right precuneus during happy facial expression recognition. Our experimental findings indicate that the limbic-cortical network might be a key target region for antidepressant treatment in major depressive disorder. 展开更多

听神经瘤显微切除术中多模式神经电生理监测的效果

目的探究听神经瘤显微切除术中应用多模式神经电生理监测的效果。方法回顾性选取2021-01—2025-01郴州市第一人民医院及中南大学湘雅医院收治的190例听神经瘤患者,对照组和研究组各95例,对照组行单纯显微切除术,研究组在对照组基础上结... 目的探究听神经瘤显微切除术中应用多模式神经电生理监测的效果。方法回顾性选取2021-01—2025-01郴州市第一人民医院及中南大学湘雅医院收治的190例听神经瘤患者,对照组和研究组各95例,对照组行单纯显微切除术,研究组在对照组基础上结合多模式神经电生理监测,比较2组手术指标、疼痛程度、听力、面神经功能、并发症发生率、GOS评分及生活质量等。结果研究组术中出血量、内听道后唇磨除区域及疼痛视觉模拟评分法(VAS)评分低于对照组(分别为t=4.499、30.273、44.644,P<0.001),手术时间短于对照组(t=3.035,P=0.003),肿瘤全切率高于对照组(90.53%比75.79%,χ^(2)=7.692,P=0.021)。术后1个月,研究组纯音平均听阈(PTA)低于对照组(t=9.283,P<0.001),听力分级明显优于对照组(χ^(2)=22.741,P<0.001)。术后6个月,研究组面神经功能House-Brackmann分级显著优于对照组(χ^(2)=8.311,P=0.040)。研究组并发症发生率低于对照组(3.16%比11.58%,χ^(2)=4.935,P=0.026)。术后6个月,研究组GOS评分及SF-36评分均高于对照组(分别为t=6.584、8.642,P<0.001)。结论听神经瘤显微切除术中应用多模式神经电生理监测可有效缩短手术时间,提升肿瘤全切率,保护患者听力及面神经功能,改善患者短期预后和生活质量。 展开更多

透刺法联合穴位注射治疗特发性面神经麻痹恢复期临床研究

目的:观察透刺法联合穴位注射治疗特发性面神经麻痹恢复期的临床疗效。方法:选取2022年7月—2023年12月丽水市人民医院收治的126例特发性面神经麻痹患者作为研究对象,采用随机数字表法分为对照组和观察组各63例。对照组采用穴位注射治疗... 目的:观察透刺法联合穴位注射治疗特发性面神经麻痹恢复期的临床疗效。方法:选取2022年7月—2023年12月丽水市人民医院收治的126例特发性面神经麻痹患者作为研究对象,采用随机数字表法分为对照组和观察组各63例。对照组采用穴位注射治疗,观察组在对照组基础上联合透刺法治疗,2组均连续治疗2周。比较2组临床疗效、面部肌肉功能、面部神经功能分级、临床症状、生活质量及不良反应发生率。结果:治疗后,观察组总有效率为92.06%(58/63),高于对照组71.43%(45/63)(P<0.05)。治疗后,2组额肌、口轮匝肌、眼轮匝肌、上唇方肌肌肉的M波波幅较治疗前升高,且观察组高于对照组(P<0.05);2组额肌、口轮匝肌、眼轮匝肌、上唇方肌肌肉的运动潜伏期较治疗前缩短,且观察组短于对照组(P<0.05)。治疗后,2组面部神经功能分级均优于治疗前,且观察组优于对照组(P<0.05)。治疗后,2组面瘫Portmann评分较治疗前升高,且观察组高于对照组(P<0.05)。治疗后,2组躯体功能得分较治疗前升高,且观察组高于对照组(P<0.05);2组社会生活功能得分较治疗前下降,且观察组低于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:透刺法联合穴位注射治疗特发性面神经麻痹恢复期具有良好的治疗效果,可改善面部肌肉功能和神经功能,缓解临床症状,提高生活质量,且具有较高的安全性。 展开更多

Electrical response grading versus House-Brackmannscale for evaluation of facial nerve injury after Bell's palsy: a comparative study 被引量:12

OBJECTIVE: There are no convenient techniques to evaluate the degree of facial nerve injury during a course of acupuncture treatment for Bell's palsy. Our previous studies found that observing the electrical response of specific facial muscles provided reasonable correlation with the prognosis of electroacupuncture treatment. Hence, we used the new method to evaluate the degree of facial nerve injury in patients with Bell's palsy in comparison with the House-Brackmann scale. The relationship between therapeutic effects and prognosis was analyzed to explore an objective method for evaluating Bell's palsy. METHODS: The facial nerve function of 68 patients with Bell's palsy was assessed with both electrical response grading and the House-Brackmann scale before treatment. Then differences in evaluation results of the two methods were compared. All enrolled patients received electroacupuncture treatment with disperse-dense wave at 1/100 Hz for 4 weeks. After treatment, correlation analysis was conducted to find the relationship between electrical response and therapeutic effects or prognosis. RESULTS: Checking consistency between electrical response grading and House-Brackmann scale: Kappa value 0.028 (P = 0.578). Correlation analysis: the two methods were correlated with the prognosis, and electrical response grading (rER = 0.789) was better than the House-Brackmann scale (rHB = 0.423). CONCLUSION: Electrical response grading is superior to the House-Brackmann scale in efficacy and reliability, and can conveniently assess the degree of facial nerve injury. The House-Brackmann scale is suitable for the patients with mild facial nerve injury, but its evaluation quality for severe facial nerve injury is poor. 展开更多

目的探讨超短脉宽皮秒联合纳米微针导入谷氨精华液治疗面部黄褐斑的效果。方法选取2024年6月至2025年5月娄底市中心医院治疗的92例面部黄褐斑患者,按治疗方法不同分为两组,对照组46例采取超短脉宽皮秒治疗,观察组46例采取纳米微针导入... 目的探讨超短脉宽皮秒联合纳米微针导入谷氨精华液治疗面部黄褐斑的效果。方法选取2024年6月至2025年5月娄底市中心医院治疗的92例面部黄褐斑患者,按治疗方法不同分为两组,对照组46例采取超短脉宽皮秒治疗,观察组46例采取纳米微针导入谷氨精华液联合超短脉宽皮秒治疗,对两组治疗效果进行比较。结果治疗前,MASI评分、黑色素指数、PGA评分组间比较,差异无统计学意义(P>0.05),经治疗后,观察组上述指标均更低,差异有统计学意义(P<0.05);治疗前,皮肤屏障功能相关指标组间比较,差异无统计学意义(P>0.05),治疗后,观察组肤角质层含水量、表皮皮脂含量明显更高,经皮水分丢失量明显更低,差异有统计学意义(P<0.05);两组患者主观满意度比较,观察组更高,差异有统计学意义(P<0.05);两组均未发生严重不良反应。结论纳米微针导入谷氨精华液联合超短脉宽皮秒能够显著改善面部黄褐斑的皮损状态,降低皮损部位的黑色素水平,可修复皮肤屏障功能,提高患者满意度,也具有较高安全性,值得推广应用。 展开更多

Using the conflict principle of the Stroop task, “effect of interference in color naming”, the present study proposes to create a computational version of the emotional Stroop task, called TREFACE, for its acronyms in Portuguese “Teste de Reconhecimento de Expressões Faciais com Conflito Emocional” (Facial Expression Recognition Test with Emotional Conflict). In this protocol, four fixed styles of presentation were generated according to the condition: Congruent Word Reading, Incongruent Word Reading, Congruent Recognition of Face Expression and Incongruent Recognition of Face Expression, counterbalanced in terms of each facial expression, word and gender of the photo character. Forty-two healthy volunteers completed the task. Results revealed that a task associated with word reading allows better performance than a task associated with face recognition. It was also identified that in the congruent condition, there is an advantage in terms of the correct responses. Additionally, the data regarding recognition of face expression showed greater difficulty when the image was not congruent with the word. In general, the results suggest that the emotional attribute can compromise the ability to recognize the faces, reaching the functioning of mechanisms such as cognitive control and regulation of emotions. Thus, the TREFACE paradigm can be considered a good assessment tool for monitoring emotional conflict, in addition to presenting itself as a new instrument in Portuguese language for assessing emotional working memory in healthy individuals and, eventually, in different pathologies that affect the functioning of cortical areas related to executive functions. 展开更多

Based upon motion capture,a semi-automatic technique for fast facial animation was implemented. While capturing the facial expressions from a performer,a camera was used to record her /his front face as a texture map. The radial basis function( RBF) technique was utilized to deform a generic facial model and the texture was remapped to generate a personalized face.Partitioning the personalized face into three regions and using the captured facial expression data,the RBF and Laplacian operator,and mean-value coordinates were implemented to deform each region respectively. With shape blending,the three regions were combined together to construct the final face model. Our results show that the technique is efficient in generating realistic facial animation. 展开更多

TREFACE (Test for Recognition of Facial Expressions with Emotional Conflict) is a computerized model for investigating the emotional factor in executive functions based on the Stroop paradigm, for the recognition of emotional expressions in human faces. To investigate the influence of the emotional component at the cortical level, the electroencephalographic (EEG) recording technique was used to measure the involvement of cortical areas during the execution of certain tasks. Thirty Brazilian native Portuguese-speaking graduate students were evaluated on their anxiety and depression levels and on their well-being at the time of the session. The EEG recording was performed in 19 channels during the execution of the TREFACE test in the 3 stages established by the model-guided training, reading, and recognition—both with congruent conditions, when the image corresponds to the word shown, and incongruent condition, when there is no correspondence. The results showed better performance in the reading stage and in congruent conditions, while greater intensity of cortical activation in the recognition stage and in incongruent conditions. In a complementary way, specific frontal activations were observed: intense theta frequency activation in the left extension representing the frontal recruitment of posterior regions in information processing;also, activation in alpha frequency in the right frontotemporal line, illustrating the executive processing in the control of attention, in addition to the dorsal manifestation of the prefrontal side, for emotional performance. Activations in beta and gamma frequencies were displayed in a more intensely distributed way in the recognition stage. The results of this mapping of cortical activity in our study can help to understand how words and images of faces can be regulated in everyday life and in clinical contexts, suggesting an integrated model that includes the neural bases of the regulation strategy. 展开更多

目的:基于面部美观及面神经功能探讨良性腮腺肿瘤患者采用改良美容术式腮腺切除术进行治疗的效果。方法:选取2021年1月-2023年3月达州市中心医院接受治疗良性腮腺肿瘤患者85例作为研究对象,并分为A组(42例)和B组(43例),分组依据为治疗... 目的:基于面部美观及面神经功能探讨良性腮腺肿瘤患者采用改良美容术式腮腺切除术进行治疗的效果。方法:选取2021年1月-2023年3月达州市中心医院接受治疗良性腮腺肿瘤患者85例作为研究对象,并分为A组(42例)和B组(43例),分组依据为治疗方法的不同。采用传统腮腺切除术对A组患者进行治疗,采用改良美容术式腮腺切除术对B组患者进行治疗。两组术后均随访1年。比较两组术后1年的临床疗效、面部美观满意度及复发率,术前、术后3 d的细胞因子,随访期间的并发症。结果:术后1年,B组总有效率为93.02%,高于A组的69.05%(P<0.05)。术后1个月,两组面神经功能分级比较没有差异(P>0.05)。与术前比较,术后3 d,两组血清白细胞介素6(IL-6)、白细胞介素1β(IL-1β)、促肾上腺皮质激素(Adrenocorticotrophic hormone,ACTH)、皮质醇(Cortisol,COR)水平均升高,但相比于A组,B组更低;两组血清糖链抗原125(CA125)、β_(2)微球蛋白(β_(2)-microglobulin,β_(2)-MG)水平均降低,且相比于A组,B组更低(P<0.05)。随访期间,B组并发症发生率低于A组(11.63% vs.30.95%,P<0.05)。术后1年,B组面部美观满意度为97.67%,高于A组的78.57%(P<0.05),而两组复发率比较差异无统计学意义(P>0.05)。结论:改良美容术式腮腺切除术可显著降低良性腮腺肿瘤患者肿瘤标志物水平,有效减轻患者炎症反应及应激反应,提高患者面部美观满意度,减少并发症发生,进一步可有效提高临床疗效,患者术后面神经功能恢复良好,且不会增加复发情况。 展开更多

Objectives: The first objective of this paper is to show the improved binary outcomes resulting from using MARIE as a diagnostic instrument that allows valid and reliable visual recognition of facial emotional expressions (VRFEE) in an objective and quantitative manner. The second objective is to demonstrate mathematical modeling of binary responses that allow the measurement of categorical dimension, sensitivity, camber, equilibrium points, transition thresholds, etc. The final objective is to illustrate the use of this test for 1) testing a homogeneous sample of healthy young participants;and 2) applying this method to a sample of 12 participants with early Alz-heimer disease compared to a matched control sample of healthy elderly participants. Design: Transforming the binary outcomes of MARIE in mathematical variables (experiment 1), allowing verification of a disorder of VRFEE in early Alzheimer’s disease (experiment 2). Measures: Comparison of numerical variables and graphic representations of both samples. Results: The objective measurement of VRFEE is possible in a healthy population. The application of this methodology to a pathological population is also made possible. The results support the current literature. Conclusion: The combination of the mathematical method with the diagnostic instrument MARIE shows its power and ease of use in clinical practice and research. Its application in many clinical conditions and in clinical research can be useful for understanding brain function. This method improves 1) the inter-examiner comparison and standardizes the quantification of VRFEE for use by multiple researchers;2) the follow-up of a sample over time;3) the comparison of two or more samples. This method is already available in clinical work for refining the diagnosis of Alzheimer’s Disease (AD) in our department. 展开更多
